Efficacy of Japanese traditional(kampo) medicine for treating chemotherapy-induced peripheral neuropathy: A retrospective case series study

AIM To investigate Japanese traditional(kampo) medicine's effectiveness on cancer chemotherapy-induced peripheral neuropathy(CIPN), we carried out this retrospective *** By searching our outpatient database of 3154 patients who consulted our outpatient clinic of Japanese-Oriental(kampo) medicine at Chiba University Hospital from November 2005 to December 2010, a total of 281 patients diagnosed with cancer were identified. Twentyfour patients out of the 281 patients identified met the following three conditions and were eligible for further investigation of the effectiveness of kampo treatment: At least one course of cancer chemotherapy had been administered; numbness and pain appeared after the chemotherapy; and CIPN was diagnosed before they were given kampo *** The 24 patients included 6 males and 18 females and ranged in age from 39 to 86(mean 61.2 ± 11.5) years old. kampo formulas were individually chosen by kampo expert doctors based on kampo-specific diagnostics. Beneficial outcomes were obtained by kampo treatment in 20 out of the 24 cases(83.3%). Nine out 20 cases had a major response(the numbness and pain showed improvement or reduction by 50% or more), with 7 of 9 cases showing a more than 70% symptom reduction. Eleven out of 20 cases showed a minor response(less than 50% symptom reduction), and 4 out of the 24 cases had no beneficial response. The most frequently used formula was goshajinkigan(GJG), followed by hachimijiogan(HJG) and keishibukuryogan. Thirteen of the 24 cases(54.2%) were prescribed aconite rootcontaining formulas including GJG and HJG. Aconite root has "warming" effects and ameliorates pain and numbness; 21 out of 24 cases(87.5%) in total used warming formulas such as aconite root-containing formulas to reduce *** Our current study suggested that kampo formulas chosen based on kampo-specific diagnostics could be for treating CIPN that is refractory to conventional medicine.

Blood Stasis Syndrome in Japan and Its Molecular Biological Analysis

Blood stasis syndrome is one of the pathological concepts of Oriental traditional medicine. In Oriental traditional medicine, blood is thought of as not only blood but also as a living component of the body. In fact, blood stasis syndrome is related to not just circulation disorders but dermatological and gynecological and other diseases. In Japan, the concept of blood stasis syndrome is based on the past literature, for instance, Synopsis of Golden Chamber （Jin Kui Yao Lue）, etc. There are many signs of this syndrome, such as a dry mouth, fullness of the abdomen and rough skin. However, the levels of importance of these signs had been unclear. Therefore, in order to determine the levels of seriousness, a scoring system of blood stasis syndrome was made based on multivariate analysis by Dr. Terasawa （Terasawa＇s Blood Stasis Score）. Using the scoring system, we have studied blood stasis syndrome mainly related to blood circulation using modem techniques of analysis. From the results, we found that patients with blood stasis syndrome showed hemorheological abnormalities, and an improvement in these abnormalities was shown after administration of removing-blood stasis formulae. Furthermore, we have studied blood stasis syndrome from the point of view of molecular biology. We searched for the specific protein expression in blood stasis syndrome by proteomic analysis, and found no specific protein expression. However, there may be a possibility of developing a diagnostic algorithm for blood stasis by construction of a decision tree. During the past few years, as one of the molecular biological factors affecting blood stasis syndrome, we have been studying hypoxia inducible factor, which is located in the upstream of many genes. Above all, blood stasis syndrome is more than just circulatory deficit but encompasses the pathological concept of constant multilateral change in the living body.

Boiogito Increases the Synthesis and Secretion of Adiponectin by Promoting Differentiation in Cultured Human Adipocytes

Boiogito (BOT) ameliorates insulin resistance and diabetes in several animal models;however, the underlying mechanisms for these in vivo effects remain unclear. Thiazolidine derivatives, which are peroxisome proliferator-activated receptor γ (PPARγ) agonists for the treatment of type II diabetes, promote adiponectin production by inducing adipocyte differentiation, thereby reducing insulin resistance. This study aimed to evaluate the effect of BOT on adipocyte differentiation using cultured human visceral preadipocytes (HVPAds) compared with the thiazolidine derivative troglitazone (TRG). We investigated the effects of BOT (0.125 - 1 mg/mL) and TRG (10 μM) on the differentiation of adipocytes treated with or without tumor necrosis factor-α (TNF-α: 5 ng/mL). On day 14 of culture, the following adipocyte differentiation marker levels were measured: intracellular lipids, extracellular (i.e., medium) adiponectin, and intracellular differentiation-related genes (PPARγ, CCAAT/enhancer binding protein, adiponectin, differentiation cluster 36, glucose transporter type 4). BOT and TRG increased factors associated with differentiation including lipid, adiponectin, and differentiation-related gene expression levels compared with the controls. The increases in these differentiation markers were inhibited by the PPARγ antagonist GW9662 (20 μM). Furthermore, TNF-α decreased all differentiation marker levels. The decreases in differentiation markers were inhibited by BOT and TRG;however, these inhibitory effects were blocked by GW9662. The results suggest that BOT increases the synthesis and secretion of adiponectin by promoting differentiation similar to TRG. This study is the first to demonstrate that adipocyte differentiation-promoting activity is a mechanism for the beneficial effects of BOT on diabetes and insulin resistance.

Conscious Slower Breathing Predominates Parasympathetic Activity and Provides a Relaxing Effect, in Healthy Japanese Adult Women

Background: The optimal breathing pattern (BP) to effectively regulate autonomic nervous activity is yet to be determined. Objective: We aimed to clarify the effects of four BPs (BP-1, BP-2, BP-3, and BP-4) on autonomic nervous activity and mood changes. Methods: Eleven healthy adult female volunteers performed each BP in a sitting position for 5 min in a resting state. The time required for one breathing for BP-1 (30 breaths/min), BP-2 (20 breaths/min), BP-3 (15 breaths/min), and BP-4 (10 breaths/min) were 2 s, 3 s, 4 s, and 6 s, respectively. The inspiratory/expiratory time of one breathing was 1 s/1 s, 1 s/2 s, 2 s/2 s, and 2 s/4 s. The high-frequency component (HF) and low-frequency component (LF)/HF ratio during and before (control) performing a BP were calculated from heart rate variability data recorded using the wearable biometric information tracer M-BIT. Three mood changes, which are, “pleasure—unpleasure”, “relaxation—tension”, and “sleepiness—arousal”, in the subjects were assessed using the visual analog scale (VAS) before and after performing a BP. Results: Slower breathing induced an increase in HF power and a reduction in LF/HF ratio, indicating increased parasympathetic activity and decreased sympathetic dominance. Furthermore, VAS revealed that slower breathing increased the tendency to feel “pleasure”, “relaxation”, and “sleepiness”. Conclusion: Our results suggest that slower breathing predominates parasympathetic activity in the autonomic nervous system, resulting in a relaxing effect. This result may help lay the foundation for deriving breathing methods that efficiently regulate an individual’s autonomic activity.

Effects of a Traditional Japanese medicine Goshajinkigan, Tokishigyakukagoshuyushokyoto on the Warm and Cold Sense Threshold and Peripheral Blood Flow

The purpose of this study was to investigate the effects of a traditional Japanese medicine Goshajinkigan (TJ-107) and Tokishigyakukagoshuyushokyoto (TJ-38) on warm sense threshold, cold sense threshold and the peripheral blood flow. 31 healthy volunteers (control group: 9people, TJ-107 group: 12 people, TJ-38group:10 people) were examined. Drugs administered 2.5 g a dose. Analysis was before and after 1 hour dosage. The warm and cold sense threshold in the thenar of the non-handedness site of these subjects was measured using a thermostimulator (Intercross-200, Intercross Co., Tokyo, Japan). The peripheral blood flow in the finger of the non-handedness site of these subjects was measured using a full-field laser perfusion imager (FLPI, Moor Instruments Ltd., England). Control: The vehicle had no significant effect on the warm sense threshold, cold sense threshold and the peripheral blood flow. TJ-107: The warm sense threshold and cold sense threshold were significantly decreased, and the reaction latency of cold sense was significantly shortening. The peripheral blood flow was significantly increased second and third finger at 115.6%, 119.3%, respectively. TJ-38: The cold sense threshold and the reaction latency of cold sense were significantly increased. The peripheral blood flow was significantly increased second and third finger with 114.3%, 112.8%, respectively. These results suggest that TJ-107 and TJ-38 have effects on the changed warm sense threshold, cold sense threshold and increased peripheral blood flow.

Assessment of Biological Reaction to Whole Body Vibration Training by Evaluating Changes in Salivary Components and Cutaneous Blood Flow

Aim: Whole body vibration (WBV) is thought to improve blood flow and autonomic balance and thereby induce a relaxation effect, which suggests its use for stress management. However, the relaxation effect of WBV training has not been objectively evaluated thus far. The purpose of this study was to determine the biological response to WBV training by measuring peripheral blood flow and salivary components using non-invasive techniques. Methods: Participants included 10 healthy volunteers (7 men, 3 women;mean age 33.8 ± 2.3) who provided oral consent and served as their own control. Each participant performed 15 types of stretching exercises for 10.5 min on the Power Plate? and cutaneous blood flow and salivary components were measured before and after the exercise. One week later, all participants performed the same exercise regimen for 10.5 min on a non-vibratory plate, and blood flow measurement and salivary tests were performed in a similar manner. Cutaneous blood flow was measured in the 4th digit for 1 min using the laser speckle flowgraphy. Saliva samples were evaluated for cortisol levels and α-amylase activity. To determine the effects of stretching exercises on the Power Plate? vs a non-vibratory plate, the differences in pre- and post-exercise peripheral blood flow, salivary cortisol levels, and salivary α-amylase activity were statistically evaluated by the t-test. Results: Mean blood flow before and after the exercise on the Power Plate? was 122.0 ± 54.2 and 156.7 ± 51.2, respectively;on a non-vibratory plate, blood flow was 136.6 ± 47.9 and 146.3 ± 38.3, respectively. The differences in pre-exercise and post-exercise values of the two training methods were not significant (p = 0.215). Mean cortisol levels before and after the exercise on the Power Plate? were 266.6 ± 125 and 204.9 ± 61.6, respectively;on a non-vibratory plate, the levels were 439.0 ± 121.7 and 425.8 ± 118.8, respectively. Effect of 528 Hz Music on the Endocrine System and Autonomic Nervous System

This study examined the stress-reducing effect on the endocrine system and the autonomic nervous system of music with a frequency of 528 Hz, which has recently attracted attention as a “healing” type of music. Nine healthy participants (one man and eight women, aged 26 - 37 years) listened to 528 Hz and standard 440 Hz music on separate days. We measured salivary biomarkers of stress (cortisol, chromogranin A, and oxytocin) before and after exposure to music, and continuously recorded the activity of the autonomic nervous system. The Profile of Mood State, 2nd edition, was also administered as a subjective indicator of stress. In the 528 Hz condition, mean levels of cortisol significantly decreased, chromogranin A tended to decrease, and oxytocin significantly increased after music exposure. However, no significant change was observed in any salivary biomarkers in the 440 Hz condition. The ratio of low frequency to high frequency autonomic nervous system activity significantly decreased after exposure to both types of music, and the coefficient of variation of R-R intervals also significantly decreased, but only after exposure to 528 Hz music. Tension-anxiety and Total Mood Disturbance scores were significantly reduced after exposure to 528 Hz music, while there was no significant difference following 440 Hz music. These results suggest that the influence of music on the autonomic nervous system and endocrine system varies depending on the frequency of the music, and furthermore, that 528 Hz music has an especially strong stress-reducing effect, even following only five minutes of exposure.
