Background Little is known about the association between stressors(especially positive stressors)during pregnancy and postpartum depression and *** We investigated the association between positive and negative stress events during different stages of pregnancy and postpartum mental health outcomes among low-income pregnant women with symptoms of anxiety in Pakistan and evaluated whether an intervention based on cognitive behavioural therapy(CBT)had a regulatory *** Participants were 621 pregnant Pakistani women with mild *** the Pregnancy Experience Scale-Brief Version,six scores were created to assess positive and negative *** performed a multivariate linear regression to examine whether these six scores,measured both at baseline and in the third trimester,were associated with postpartum anxiety and depressive *** effect of the intervention on this relationship was examined by adding an interaction term to the regression *** Hassles frequency measured in the third trimester was positively associated with depression(B=0.22,95%confidence interval(Cl):0.09 to 0.36)and anxiety(B=0.19,95%Cl:0.08to 0.30).At the same timepoint,uplifts intensity was negatively associated with symptoms of depression(B=-0.82,95%Cl:1.46 to-0.18)and anxiety(B=-0.70,95%Cl:-1.25 to-0.15),whereas hassles intensity was positively related to symptoms of depression(B=1.02,95%Cl:0.36 to 1.67)and anxiety(B=0.90,95%Cl:0.34 to 1.47).The intensity ratio of hassles to uplifts reported in the third trimester was positively related to both depression(B=1.40,95%Cl:0.59 to 2.20)and anxiety(B=1.26,95%Cl:0.57 to 1.96).The intervention strengthened the overall positive effects of uplfts and the negative effects of *** experiences at baseline during early pregnancy to mid-pregnancy were not associated with mental health *** Stressors in the third trimester but not earlier in pregnancy were associated with postpartum symptoms

In this editorial,we comment on Yin et al’s recently published Letter to the *** particular,we focus on the potential use of glucagon-like peptide 1 receptor agonists(GLP-1RAs)alone,but even more so in combination therapy,as one of the most promising therapies in metabolic dysfunction-associated steatotic liver disease(MASLD),the new definition of an old condition,non-alcoholic fatty liver disease,which aims to better define the spectrum of steatotic *** is well known that GLP-1RAs,having shown outstanding performance in fat loss,weight loss,and improvement of insulin resistance,could play a role in protecting the liver from progressive *** clinical trials have shown that,among GLP-1RAs,semaglutide is a safe,well-studied therapeutic choice for MASLD patients;however,most studies demonstrate that,while semaglutide can reduce steatosis,including steatohepatitis histological signs(in terms of inflammatory cell infiltration and hepatocyte ballooning),it does not improve *** of therapies with different but complementary mechanisms of action are considered the best way to improve efficiency and slow disease progression due to the complex pathophysiology of the *** particular,GLP-1RAs associated with antifibrotic drug therapy,dual glucose-dependent insulinotropic polypeptide(GIP)/GLP-1RA or GLP-1 and glucagon RAs have promoted greater improvement in hepatic steatosis,liver biochemistry,and non-invasive fibrosis tests than ***,although to date there are no definitive indications from international drug agencies,there is the hope that soon the therapeutic lines in the most advanced phase of study will be able to provide a therapy for MASLD,one that will certainly include the use of GLP-1RAs as combination therapy.

Uganda has been implementing the Global health Security Agenda(GHSA)since 2015 to build its capacity according to World health Organization(WHO)Benchmarks on International health Regulations *** country remains prone to outbreaks,with more than 20 disease outbreaks reported in the past five years,including Ebola virus disease,Crimean-Congo haemorrhagic fever,Marburg haemorrhagic fever,measles,yellow fever,coronavirus disease 2019(COVID-19),and *** resistance(AMR)is an ongoing *** scored capacity level 3 on infection prevention and control(IPC)and antimicrobial stewardship(AMS)in the 2017 Joint External Evaluation(JEE)*** gaps were being addressed after a self-assessment in *** paper describes the technical assistance approaches provided to Uganda by the Medicines,Technologies,and Pharmaceutical services Program,funded by the United States(U.S.)Agency for International Development,and implemented by Management Sciences for *** program,through a One health approach,supported systematic capacity strengthening based on the JEE's capacity advancement framework for global health security,specifically relating to *** program's interventions impacted 32 WHO benchmark actions(7 for AMR multisectoral coordination,16 for IPC,and 9 for AMS),contributing to Uganda's strengthened GHSA *** success built on the AMR platform,the program trained 745 health workers in IPC for the Ebola virus and provided support for simulation exercises by eight district IPC *** program also worked with the Ministry of health to coordinate IPC for the COVID-19 response in five health regions,covering 45 districts and reaching 5,452 health workers at 858 health facilities.

Association of Hypertension and Plant-based Dietary Patterns on Cognitive Impairment: A Nationwide Cohort Study in China

This study investigated the association of hypertension with cognitive impairment and determined whether better adherence to plant-based diet (PBD) patterns may modify the association. We analyzed 4485 elderly individuals with normal cognition at baseline from the Chinese Longitudinal healthy Longevity Survey (2011-2018). Cognitive function was assessed by the Mini-mental State Examination (MMSE)， and cognitive impairment was defined as an MMSE score below 18 points. Diet patterns were assessed using the overall plant-based diet index (PDI)， healthful plant-based diet index (hPDI)， and unhealthful plant-based diet index (uPDI) derived from a simplified food frequency questionnaire (FFQ). Blood pressure was measured through physical examination. Cox proportional hazard regression and restricted cubic spline were performed. Participants with hypertension had an increased risk of cognitive impairment compared with normal group. The associations between hypertension and cognitive impairment were more pronounced among participants with lower PDI than those with higher PDI， among participants with lower hPDI than those with higher hPDI， and among participants with higher uPDI than those with lower uPDI. A healthful PBD pattern may attenuate hypertension-induced cognitive impairment， while an unhealthful PBD pattern may exacerbate hypertension-induced cognitive impairment in elderly. Adherence to healthful PBD pattern could be used to prevent or mitigate adverse neurological effects in the hypertensive population.

1 Background Congenital heart disease(CHD)is the most common major congenital anomaly,affecting approximately one in every 100 live births[1].Among congenital anomalies,66%of preventable deaths are due to CHD,and 58%of the avertable morbidity and mortality due to congenital anomalies would result from scaling congenital heart surgery services[2].Every year,nearly 300,000 children and adults die from CHD,the majority of whom live in low-and middle-income countries(LMICs)[3].Approximately 49%of all individuals with CHD will require surgical or interventional care at some point in their lifetime[4];as a result of advances in access to and the delivery of such services,over 95%of children born with CHD in high-income countries now live into adulthood[3].Here,adults have surpassed children in the number of CHD cases at a ratio of 2:1[5].
