Nutrient drink test: A promising new tool for irritable bowel syndrome diagnosis

BACKGROUND Irritable bowel syndrome(IBS) is a highly prevalent condition. It is diagnosed on the basis of chronic symptoms after the clinical and/or investigative exclusion of organic diseases that can cause similar symptoms. There is no reproducible noninvasive test for the diagnosis of IBS, and this raises diagnostic uncertainty among physicians and hinders acceptance of the diagnosis by *** gastrointestinal(GI) syndromes often present with overlapping upper and lower GI tract symptoms, now believed to be generated by visceral hypersensitivity. This study examines the possibility that, in IBS, a nutrient drink test(NDT) provokes GI symptoms that allow a positive differentiation of these patients from healthy *** To evaluate the NDT for the diagnosis of *** This prospective case-control study compared the effect of two different nutrient drinks on GI symptoms in 10 IBS patients(patients) and 10 healthy controls(controls). The 500 kcal high nutrient drink and the low nutrient 250 kcal drink were given in randomized order on separate days. Symptoms were assessed just before and at several time points after drink ingestion. Global dyspepsia and abdominal scores were derived from individual symptom data recorded by two questionnaires designed by our group, the upper and the general GI symptomquestionnaires, respectively. Psycho-social morbidity and quality of life were also formally assessed. Effects of extract from Ginkgo biloba on carbon tetrachloride-induced liver injury in rats

AIM： To study the effects of extract from Ginkgo biloba （EGb） containing 22% flavonoid and 5% terpenoid on chronic liver injury and liver fibrosis of rats induced by carbon tetrachloride （CCh）. METHODS： All rats were randomly divided into control group, CCl4-treated group, colchicine-treated group and EGb-protected group. Chronic liver injury was induced in experimental groups by subcutaneous injection of CCh and fed with chows premixed with 79.5% corn powder, 20% lard and 0.5% cholesterol （v/v）. EGb-protected group was treated with EGb （0.5 g/kg body weight per day） for 7 wk. At the end of wk 8, all the rats were killed. Liver function, liver fibrosis, oxidative stress and expression of transforming growth factor β1 （TGF-β1） a-smooth muscle actin （α-SMA） and type I collagens in liver were determined. In addition, pathology changes of liver tissue were observed under light microscope. RESULTS： The levels of alanine aminotransferase （ALT）, aspartate aminotransferase （AST） and albumin （AIb） in EGb-protected group were notably improved as compared with the CCL4-treated group （P 〈 0.01）. The contents of serum hyaluronic acid （HA）, type III procollagen （PCⅢ）, type IV collagen （CIV） and the expression of hepatic tissue TGF-β1, α-SMA and type I collagen in EGb-protected group were significantly lower than those in CCL4-treated groups （P 〈 0.05, P 〈 0.01）. The degrees of liver fibrosis in EGb-protected groups were lower than those in CCL4-treated groups （6.58 ±1.25 vs 9.52 ± 2.06, P 〈 0.05）. Compared to the CCL4-treated group, the levels of plasma glutathoine peroxidase （Se-GSH-Px）, superoxide dismutase （SOD） and malondialdehyde （MDA） were strikingly improved also in EGb-protected group （P 〈 0.05, P 〈 0.01）. CONCLUSION： EGb resists oxidative stress and thereby reduces chronic liver injury and liver fibrosis in rats with liver injury induced by CCl4.

Introduction: Human immunodeficiency virus (HIV) infection is a public health problem of concern. Anti-retroviral therapy (ART) is associated with multiple side effects. This study aimed at identifying the different hepatic manifestations of antiretroviral therapy and the responsible molecules. Patients and Methods: This was an eight months period prospective descriptive study, from January 1st to August 31st, 2015, conducted in the department of Gastroenterology and Internal Medicine at the Brazzaville University Teaching Hospital. Study participants were treatment-na?ve HIV patients who were initiated on ART treatment during the study period. Patients with liver disease, liver cytolysis prior to initiation of therapy, and those with alternative therapy that may cause hepatotoxicity were excluded. The sample size was 110 patients. Results: The age was ranging from 25 to 70 years with a mean age of 47.5 ± 7.5 years. During the six months of follow-up, the alarming hepatic signs were observed in 26.36% of cases (n = 29) in the 3rd month of treatment. There was no observed alarming sign in the 6th month of follow-up. The cytolytic pattern was observed in 54.55% of cases (n = 60) in the 3rd month. The cholestatic pattern was observed in 6.36% of cases (n = 7) in the 3rd month. Triple therapy combination of Zidovudine, Lamivudine and Nevirapine (AZT + 3TC + NVP) was the most used in 57.27% (n = 63) with a statistically significant p value to the occurrence of cytolytic pattern (p Conclusion: Drug induced liver toxicity occurs in a significant number of patients starting ART. The prevalence of hepatic events was high at the third month of treatment and the triple therapy of Zidovudine, Lamivudine and Nevirapine (AZT + 3TC + NVP) was the most incriminated.
