Objective： CareHPV is a human papillomavirus （HPV） DNA test for low-resource settings （LRS）. This study assesses optimum triage strategies for careHPV-positive women in LRS. Methods： A total of 2,530 Chinese women were concurrently screened for cervical cancer with visual inspection with acetic acid （VIA）, liquid-based cytology and HPV testing by physician- and self-collected careHPV, and physician-collected Hybrid Capture 2 （HC2）. Screen-positive women were referred to colposcopy with biopsy and endocervical curettage as necessary. HPV-positivity was defined as _〉1.0 relative light units/cutoff （RLU/CO） for both careHPV and HC2. Primary physician-HC2, physician-careHPV and self-careHPV and in sequential screening with cytology, VIA, or increased HPV test-positivity performance, stratified by age, were assessed for cervical intraepithelial neoplasia （CIN） grade 2/3 or worse （CIN2/3＋） detection. Results： The sensitivities and specificities of primary HPV testing for CIN2＋ were： 83.8%, 88.1% for physician- careHPV; 72. 1%, 88.2% for self-careHPV; and 97.1%, 86.0% for HC2. Physician-careHPV test-positive women with VIA triage had a sensitivity of 30.9% for CIN2＋ versus 80.9% with cytology triage. Self-careHPV test- positive women with VIA triage was 26.5% versus 66.2 % with cytology triage. The sensitivity of HC2 test-positive women with VIA triage was 38.2 % versus 92.6% with cytology triage. The sensitivity of physician-careHPV testing for CIN2＋ decreased from 83.8% at _〉1.0 RLU/CO to 72.1% at _〉10.00 RLU/CO, while the sensitivity of self- careHPV testing decreased from 72.1% at _〉1.0 RLU/CO to 32.4% at _〉10.00 RLU/CO; similar trends were seen with age-stratification. Conclusions： VIA and cytology triage improved specificity for CIN2/3 than no triage. Sensitivity with VIA triage was unsuitable for a mass-screening program. Aptamerized silica/gold nanocapsules for stimulated release of doxorubicin through remote two-photon excitation

Precision-based drug delivery via remote triggering is fast becoming an attractive therapeutic design and is highly useful in complicated clinical situations that may require accurate site-delivery of drug while reducing the risk of collateral damage to surrounding healthy *** the many strategies available to achieve these desirable effects,silica/gold nano-assemblies offers a practical means to achieving these ***,as a proof-of-concept,a silica nanocapsule passivated with a gold outer nanoshell had been fabricated to deliver Doxorubicin,and this nano-assembly can be remotely triggered via two-photon excitation(TPE),even under in vivo setting.A polyethylene glycol(PEG)layer as well as AS1411 DNA aptamer had also been grafted to the surface to improve homing specificity toward MDA-MB-231 breast cancer *** assembly of silica/gold nanocapsules was characterized via TEM,FTIR,and UVVis to validate the the *** TPE irradiation,a higher expression level of Annexin V and Caspase-3 was observed in both in vitro and in vivo animal models.A significant reduction in tumor size on mice model was noticed after 21 days,and these results had suggested a viable nano-sized design serving as remotely triggered drug release platform based on current well-established silica nanoparticulate methodologies.

Achieving timely percutaneous reperfusion for rural ST-elevation myocardial infarction patients by direct transport to an urban PCI-hospital

Backgrounds ST-elevation myocardial infarction （STEMI） guidelines recommend reperfusion by primary percutaneous coronary in- tervention （PCI） ≤ 90 min from time of first medical contact （FMC）. This strategy is challenging in rural areas lacking a nearby PCI-capable hospital. Recommended reperfusion times can be achieved for STEMI patients presenting in rural areas without a nearby PCI-capable hospital by ground transportation to a central PCI-capable hospital by use of protocol-driven emergency medical service （EMS） STEMI field triage protocol. Methods Sixty STEMI patients directly transported by EMS from three rural counties （Nassau, Camden and Charlton Counties） within a 50-mile radius of University of Florida Health-Jacksonville （UFHJ） from 01/01/2009 to 12/31/2013 were identified from its PCI registry. The STEMI field triage protocol incorporated three elements：（1） a cooperative agreement between each of the rural emergency medical service （EMS） agency and UFHJ; （2） performance of a pre-hospital ECG to facilitate STEMI identification and laboratory activation; and （3） direct transfer by ground transportation to the UFHJ cardiac catheterization laboratory. FMC-to-device （FMC2D）, door-to-device （D2D）, and transit times, the day of week, time of day, and EMS shift times were recorded, and odds ratio （OR） of achieving FMC2D times was calculated. Results FMC2D times were shorter for in-state STEMIs （81 ± 17 vs. 87± 19 min）, but D2D times were similar （37 ± 18 vs. 39 ± 21 min）. FMC2D 〈 90 min were achieved in 82.7% in-state STEMIs compared to 52.2% for out-of-state STEMIs （OR = 4.4, 95% CI： 1.24-15.57; P = 0.018）. FMC2D times were homogenous after adjusting for weekday vs. weekend, EMS shift times. Nine patients did not meet FMC2D ≤ 90 min. Six were within 10 min of target; all patient achieved FMC2D 〈 120 min. Conclusions Guideline-compliant FMC2D ≤90 min is achievable for rural STEMI patients within a 50 mile radius of a PCI-capable hospital by use of protocol-driv

A Study of the Cardiovascular Risk Factor Profile in Patients with Acute Coronary Syndrome with Particular Reference to Metabolic Syndrome

Objectives: Cardiovascular risk factors for acute coronary syndrome (ACS) are on rise in people of Indian origin. Moreover, the prevalence of metabolic syndrome (MS) is higher in populations with ACS than in the general population. Thus, the aim of the study was to evaluate the cardiovascular risk factor profile of patients presenting with acute coronary syndrome and to study the prevalence and impact of metabolic syndrome in patients with acute coronary syndrome by using South Asian modified National cholesterol Education Program Adult Treatment Panel III definition. Methods: This study was conducted between October, 2010 and March, 2011 at Government General Hospital, Kurnool, India. A total of 100 patients with ST or non-ST segment elevation ACS and who had clinical, electrocardiographic and biochemical diagnosis within the first 24 hrs of clinical onset were enrolled in the study. The enrolled subjects were divided into two groups: those with MS and those without MS. Cardiovascular risk factors defined by components of MS were evaluated. Analysis was performed using SPSS software. Results: Sixty patients had MS along with ACS and 40 patients had ACS without MS. The most prevalent component of MS was increased waist circumference (83.3%), followed by increased triglycerides (78.3%) and increased fasting blood sugar (76.7%). Majority of patients (58.3%) had presence of three components of MS. Conclusion: The MS is a highly prevalent condition among the patients with ACS and is associated with severe coronary artery disease. The identification of MS imparts a high risk status to the patient in developing cardiovascular disease but at the same time provides abundant opportunities for intervention. Hence it is of paramount importance to aim for stricter goals and lower cut offs for intervention at all levels of prevention: primordial, primary and secondary.

Design and analysis of 20 Gb/s inductorless limiting amplifier in 65 nm CMOS technology

A high speed inductorless limiting amplifier （LA） in an optical communication receiver with the work- ing speed up to 20 Gb/s is presented. The LA includes an input matching network, a four-stage 3rd order amplifier core, an output buffer for the test and a DC offset cancellation （DCOC）. It uses the active interleaving feedback technique both to broaden the bandwidth and achieve the flatness response. Based on our careful analysis of the DCOC and stability, an error amplifier is added to the DCOC loop in order to keep the offset voltage reasonable. Fabricated in the 65 nm CMOS technology, the LA only occupies an area of 0.45 × 0.25 mm2 （without PAD）. The measurement results show that the LA achieves a differential voltage gain of 37 dB, and a 3-dB bandwidth of 16.5 GHz. Up to 26.5 GHz, the Sddlm and Sdd22 are less than -16 dB and -9 dB. The chip excluding buffer is supplied by 1.2 V VDD and draws a current of 50 mA.
