BACKGROUND The development of regenerative therapy for human spinal cord injury(SCI)is dramatically restricted by two main challenges:the need for a safe source of functionally active and reproducible neural stem cells and the need of adequate animal models for preclinical *** reprogramming of somatic cells into neuronal and glial precursors might be a promising solution to the first *** use of non-human primates for preclinical studies exploring new treatment paradigms in SCI results in data with more translational relevance to human *** To investigate the safety and efficacy of intraspinal transplantation of directly reprogrammed neural precursor cells(drNPCs).METHODS Seven non-human primates with verified complete thoracic SCI were divided into two groups:drNPC group(n=4)was subjected to intraspinal transplantation of 5 million drNPCs rostral and caudal to the lesion site 2 wk post injury,and lesion control(n=3)was injected identically with the equivalent volume of *** Follow-up for 12 wk revealed that animals in the drNPC group demonstrated a significant recovery of the paralyzed hindlimb as well as recovery of somatosensory evoked potential and motor evoked potential of injured *** resonance diffusion tensor imaging data confirmed the intraspinal transplantation of drNPCs did not adversely affect the morphology of the central nervous system or cerebrospinal fluid *** immunohistochemical analysis showed that drNPCs maintained SOX2 expression characteristic of multipotency in the transplanted spinal cord for at least 12 wk,migrating to areas of axon growth *** Our data demonstrated that drNPC transplantation was safe and contributed to improvement of spinal cord function after acute SCI,based on neurological status assessment and neurophysiological recovery within 12 wk after *** functional improvement described was not associated with neuronal differentiation of th

The authors describe a newborn patient with mesenchymal hamartoma of the chest wall associated with pulmonary hypoplasia. A massive thoracic tumor was diagnosed by prenatal ultrasonography and magnetic resonance imaging at the 28th week of gestation. She was delivered through cesarean delivery at the 36th gestational week. Respiratory distress because of pulmonary hypoplasia necessitated neonatal intensive care. The tumor extensively involved the left hemithorax including all 12 ribs and the first 10 thoracic vertebrae, resulting inmarked deformity of the thorax. At 5 days of age, she underwent the incisional biopsy through a left thoracotomy. Histopathology of biopsy specimens showed multiple components of mesenchymal origin including premature cartilage, bone, and cystic lesions resembling aneurysmal bone cyst. The tumor then showed a rapid overgrowth, but subsequently exhibited a selflimited growth for months, in which her respiratory condition gradually improved to spontaneous breathing without oxygenation support. The present case advocates perinatal preparations for associated pulmonary hypoplasia and conservative management for the neoplasmin fetuses prenatally diagnosed as having this unique pathological entity.

Purpose The aim of this study was to investigate the prenatal courses and mana gement of abdominal surgical diseases. Methods Of the 327 patients registered wi th our fetal treatment board since March 2002, 83 fetuses referred to the surgic al department were enrolled for the current study. The prenatal diagnosis, seque ntial fetal images, and perinatal courses of these cases were reviewed retrospec tively. Results Of the 83 cases, abdominal diseases were suspected in 34, lung a nd thoracic diseases in 25, genitourinary diseases in 12, and other anomalies in *** peritonitis (MP), intestinal obstruction, and abdominal wall defec ts accounted for approximately 65%of the abdominal diseases. Five patients with prenatally diagnosed lung diseases underwent fetal surgical intervention, and 1 7 of the 22 liveborn patients survived. In contrast, none of the patients with p renatally diagnosed abdominal anomalies underwent fetal surgical intervention, y et, 23 of the 24 liveborn patients survived. However, preterm labor and hydrops were seen frequently in the patients with giant cystic MP, suggesting a fetal cr itical condition. Conclusions Although the clinical outcome of abdominal disease s seemed favorable with postnatal treatment, the current results suggested the o ccurrence of hidden mortality in utero and the potential need for fetal interven tion for some abdominal conditions, such as MP.

Analysis of factors related to postoperative pulmonary infection in lung cancer patients after recovery from COVID-19

Objective:To explore factors related to postoperative pulmonary infection in lung cancer patients after recovery from COVID-19 and to provide methods for preventing and reducing the incidence of postoperative lung infection in patients with lung ***:A total of 92 patients who underwent lung cancer surgery in the Department of thoracic and Cardiac surgery of Yichang Central People’s Hospital from January 28,2023,to March 3,2023,were *** were divided into a pulmonary infection group(47 cases)and a nonpulmonary infection group(45 cases)according to whether pulmonary infection *** clinical data of patients were collected and collated to analyse the related influencing factors of pulmonary infection in lung cancer patients after recovery from ***:Univariate analysis showed that patient age(≥60 years),fever after COVID-19 infection,oral and laryngeal symptoms,digestive tract symptoms,neurological symptoms,long-term smoking history,hypertension history,and operation time(≥3 h)were correlated with pulmonary infection(all P0.05).Logistic multivariate analysis showed that age(≥60 years old),long operation time(≥3 h)and long-term smoking history were independent influencing factors for postoperative pulmonary infection in patients with radical resection of lung cancer(P>0.05).Conclusion:In this study,older age(≥60 years old),long-term smoking history,and long operation time(≥3 h)were risk factors for pulmonary infection after lung cancer *** the future,active treatment measures can be taken to address these risk factors during the perioperative period to reduce the incidence of postoperative pulmonary infection.

Objectives: We sought to describe the midterm results of extensive primary rep air of the thoracic aorta by means of the modified elephant trunk technique with a stent graft for acute type A aortic dissection, particularly the changes of t he false lumen shown by enhanced computed tomographic scanning. Methods: The sub jects were 35 consecutive patients who received arch replacement with open stent grafting for type A acute aortic dissection between December 1997 and April 200 2. The mean follow-up period was 55 months (range, 30-83 months). Computed tom ographic scanning was performed at 1, 3, 12, and 36 months postoperatively to de tect thrombosis and obliteration of the false lumen after its exclusion by the s tent graft. The diameter of the aorta was measured at 3 levels: the distal edge of the stent graft, the diaphragm, and the origin of the superior mesenteric art ery. Results: Two patients died in the initial operation, but no patients requir ed additional surgical treatment of the thoracic aorta. The mean diameter of the stent grafts was 26.2 mm, and the mean length was 8.9 cm. Thrombus formation in the false lumen was recognized at the distal edge of the graft in all patients, at the diaphragmatic level in 26 patients, and at the superior mesenteric arter y level in 15 patients. Obliteration of the false lumen was recognized at the di stal edge of the graft in all patients, at the diaphragmatic level in 20 patient s, and at the superior mesenteric artery level in 15 patients. The aorta distal to the stent graft showed minimal changes. Conclusions: In patients with acute t ype A aortic dissections, it is possible to perform extensive primary repair of the thoracic aorta with relative safety by using a synthetic graft with a self- expanding stent, and this method might reduce the necessity of further operation s not only for the distal descending aorta but also for the thoracoabdominal aor ta.

Objective:To determine post-discharge mortality and associated factors of the first-wave multicenter Turkish thoracic Society(TTD)-TURCOVID ***:In this retrospective cohort study,we analyzed the data of 18 of 26 centers included in the first TTD-TURCOVID study,and 1112 cases diagnosed with COVID-19 between 11 March and 31 July 2020 participated in the *** causes of death after COVID-19 discharge were ***:The mean age of the patients was(51.07±16.93)years,with 57.6%male *** the cohort group,89.1%of COVID-19 treatment locations were hospital wards,3.6%were intensive care units(ICUs),and 7.2%were community *** the longterm follow-up,the in-hospital mortality rate was 3.6%(95%CI 2.6-4.8),the post-discharge mortality rate was 2.8%(95%CI 1.9-3.9),and the total mortality was 6.3%(95%CI 5.0-7.8).After discharge,63.3%of mortality overall occurred during the first six *** rates in post-discharge follow-ups were 12.7%(95%CI 8.0-30.6)in cancer patients,10.8%(95%CI 6.3-22.9)in chronic obstructive pulmonary disease patients,11.1%(95%CI 4.4-22.7)in heart failure patients,7.8(95%CI 3.8-14.3)in atherosclerotic heart disease patients,and 2.3%(95%CI 0.8-5.6)in diabetes mellitus *** smokers/ex-smokers,the all-mortality rates were higher than in ***:This multicenter study showed that patients over 65 years of age,males,former/active smoker,ICU stay,lung,heart disease,and malignancy should be followed up for at least the first six months after discharge due to COVID-19.
