Objective Manipulation of the aorta has been shown to be associated wit h posto perative neurologic events after surgical myocardial revascularization when the aorta is diseased. The Heartstring proximal anastomotic system (Guidant, Indiana polis, Ind) is a device designed to assist in the performance of proximal anasto moses with minimal aortic manipulation. We describe our initial experience with this product. Methods Twelve patients with a diseased aorta who underwent off-p ump myocardial revascularization and had their proximal anastomoses performed with the Heartstring device were stud ied for operative and postoperative outcomes and surgical technique. Results The mean age of the patients was 76 ±7 years. The estimated EuroSCORE perioperativ e mortality was 12%±8%. Themedian number of distal anastomoses was 3. In all patients, a saphenous vein graft was anastomosed to the aorta using the Heartstr ing device. In 5 patients, the radial artery was used as a conduit and connected proximally to a left internal thoracic artery as a Y graft, to a saphenous vein graft as a “horseshoe,”or on the hood of a saphenous vein graft. The operativ e and postoperative courses were uneventful. Three seals developed cracks and we re not used. One seal developed an unravel in its periphery but was used success fully. Conclusions The Heartstring proximal anastomotic system is a device that allows the surgeon to perform standard proximal clampless anastomoses. Elderly p atients with a diseased aorta may benefit from this device.

Paraspinal Intramuscular Schwannoma: Case Presentation and Literature Review of a Rare Pathology

Background: A pure intramuscular paraspinal schwannoma is extremely rare, with only few case reports in the literature. Due to low frequency, lack of specific clinical signs and symptoms and difficult radiological assessment, the pre-surgical workup leads to a broad differential diagnosis. The aim of the work carried out was therefore to search for evidence of common clinical and radiological signs of the intramuscular paraspinal schwannoma, useful in the diagnostic work-up, and to highlight possible standards of treatment. Methods: Starting from the analysis of our case, we have extended a search to similar cases on the databases PubMed and Google Scholar. The outcome of the analysis gathered, up to August 2021, six relevant papers. Result: We treated a 69-year-old female with a thoracic intramuscular paraspinal mass. The pre-operative MRI had identified two typical signs of schwannoma (“split-fat” sign and the “entry-exit” sign). Intraoperative nerve isolation was performed and complete surgical excision was achieved. At the pathological analysis, we could confirm a rare OMS grade I “ancient schwannoma”. The analysis of the literature data showed a homogeneous picture of radiological presentation of the pathology. Moreover, surgical treatment was always the treatment of choice. Conclusions: The proposed assessment and treatment were consistent with the standard collected in the literature. Typical radiological signs were also found in the other cases collected. Radical surgery is the preferred treatment.

目的分析肺动脉内膜剥脱术（PEA）治疗慢性栓塞性肺动脉高压的围手术期资料,探讨美国加州大学圣迭戈分校（UCSD）手术经验.方法回顾性研究UCSD 32例肺动脉血栓内膜剥脱手术资料,其中男17例,女15例;平均年龄（47.56±16.04）岁,平均病程（3.90±4.61）年;15例有深静脉血栓病史.采用全麻、胸骨正中切口、深低温、间断停循环双侧肺动脉内膜剥脱的手术方法.结果根据术中病理标本Jamieson分型,Ⅰ型占21.8%,Ⅱ型占28.1%,Ⅲ型占37.5%.平均转机（236.32±37.27）min,主动脉阻断（111.69±28.14）min,停循环（38.00±13.58）min.术后机械通气（66.23±99.24）h,住ICU（4.62±4.50）天,无死亡.病人肺动脉收缩压由术前（81.03±16.92）mm Hg（1 mm Hg=0.133 kPa）降至术后（51.20±12.16）mm Hg,肺血管阻力由术前（88.91±42.32）kPa·s·L-1降至术后的（34.38±15.68）kPa·s·L-1,心排量由术前（3.65±1.08）L/min增加到术后（5.85±1.21）L/min,中心静脉压由（13.07±2.11）cmH2O（1 cmH2O=0.098 kPa）降至（9.86±3.02）cmH2O.术后短期随访显示,病人心功能（NYHA）恢复到Ⅰ级19例、Ⅱ级13例,生活质量明显改善.结论 PEA是治疗慢性栓塞性肺动脉高压的重要手段,手术成功率逐年提高;深低温、间断停循环、双侧肺动脉内膜剥脱及内膜外翻技术为PEA标准术式.多中心资料证实该术式可以有效降低肺动脉压和肺血管阻力,明显改善血流动力学指标和心肺功能.多数国内医疗中心没有足够手术经验,应尽量避免选择肺动脉压收缩≥100mmHg,肺血管阻力≥100kPa·s·L^-1及Ⅲ型病变者行PEA手术.

A Pictorial Review on the Role of 64-Slice HD MDCT in Detecting Post CABG Cardiothoracic Complications

Aims: The aim of this study is to have a pictorial review on the role of 64-slice multi-detector computerized tomography (MDCT) in detecting post coronary artery graft (GABG) cardiothoracic complications. Materials & Methods: During the period from November 2012 to December 2013, a prospective evaluation of 40 patients with suspected post-operative cardiothoracic complications underwent MDCT coronary angiography in our clinical radiology department. Informed consent was obtained from all patients and the study had institutional review board approval. Descriptive statistics were used to analyze the data. Results: There were 35 males and 5 females, their mean age ± (SD) of 57 ± 3.9 years. A total of 60 grafts were available for evaluation. Two patients could not be evaluated due to clip-artifacts and advanced atherosclerotic disease. Eleven patients showed occluded graft (9) at proximal and distal (2) anastomosis. Seven patients showed pericardial effusion and pleural effusion (8). Two patients had chest wall infection with retrosternal extension and associated mediastinal involvement. One patient showed aneurysm at the re-implanted coronary artery after Bentall procedure, another patient had postoperative myocardial infarction, one patient showed dissection of the ascending aorta, one patient pseudoaneurysm of left ventricle, two patients localized pericardial hematoma, two patients pulmonary embolism, and two patients showed pneumonia. All these complications occurred from 1-60 days from surgery. Conclusion: HD MDCT is an easy non-invasive technique which showed to be effective in diagnosis of most postoperative CABG cardiothoracic complications and is gaining more ground in difficult clinical situations.

Spine Stereotactic Body Radiation Therapy Residual Setup Errors and Intra-Fraction Motion Using the Stereotactic X-Ray Image Guidance Verification System

Purpose: To determine the precision of our institution’s current immobilization devices for spine SBRT, ultimately leading to recommendations for appropriate planning margins. Methods: We identified 12 patients (25 treatments) with spinal metastasis treated with spine Stereotactic Body Radiation Therapy (SBRT). The Body-FIX system was used as immobilization device for thoracic (T) and lumbar (L) spine lesions. The head and shoulder mask system was used as immobilization device for cervical (C) spine lesions. Initial patient setup used the infrared positioning system with body markers. Stereotactic X-ray imaging was then performed and correction was made if the initial setup error exceeded predetermined institutional tolerances, 1.5 mm for translation and 2° for rotation. Three additional sets of verification X-rays were obtained pre-, mid-, and post-treatment for all treatments. Results: Intrafraction motion regardless of immobilization technique was found to be 1.28 ± 0.57 mm. The mean and standard deviation of the variances along each direction were as follows: Superior-inferior, 0.56 ± 0.39 mm and 0.77 ± 0.52 mm, (p = 0.25);Anterior-posterior, 0.57 ± 0.43 mm and 1.14 ± 0.61 mm, (p = 0.01);Left-right, 0.48 ± 0.34 mm and 0.74 ± 0.40 mm, (p = 0.09) respectively. There was a significantly greater difference in the average 3D variance of the BodyFIX as compared to the head and shoulder mask immobilization system, 1.04 ± 0.46 mm and 1.71 ± 0.52 mm;(p = 0.003) respectively. Conclusions: Overall, our institution’s image guidance system using stereotactic X-ray imaging verification provides acceptable localization accuracy as previously defined in the literature. We observed a greater intrafraction motion for the head and shoulder mask as compared with the BodyFIX immobilization system, which may be a result of greater C-spine mobility and/or the suboptimal mask immobilization. Chest X-ray is readily available and usually the first chest investigation. thoracic CT scan constitutes an alternative and complimentary chest investigation. It is currently the most efficient investigation for the chest and its contents. Objectives: To evaluate the relevance of chest CT Scan requests in two university teaching hospitals in Cameroon. Material and Methods: We conducted a cross-sectional, retrospective and descriptive study at the Radiology and imaging units of the Yaounde Central Hospital and the Yaounde University Teaching Hospital Centre. Included in our study were files of patients who had a chest CT scan investigation during three years. Results: We had a study population of 323 subjects. The age interval was 23 months to 91 years old. Pulmonologist were the most prescribers with 27.2%. We had 80% conformity of indications with French Society of Radiology (FSR) standards. 50 over of 323 indications were not recommended by the FSR. Conclusion: There is a conformity rate of 80% between indications and the FSR recommendations.
