BACKGROUND Malignancy prediction remains important to preoperative diagnosis and postoperative follow-up in laryngeal *** To evaluate the circulating immune population and develop a nomogram for prediction of malignancy in patients with laryngeal *** A primary cohort of 156 patients was divided into laryngeal benign lesion,premalignant lesion and malignant lesion *** blood from patients was measured by blood routine test and flow cytometry.A nomogram was developed and applied to a validation cohort containing 55 consecutive *** Age,gender and seven circulating immune parameters exhibited significant differences between laryngeal benign lesion and premalignant *** nomogram incorporated predictors,including gender,age,smoke index,proportions of monocytes,CD8+T cells,CD4+T cells,B cells and CD4/CD8+T cell *** showed good discrimination between laryngeal premalignant lesion and malignant lesion,with a C-index of 0.844 for the primary *** of this nomogram in the validation cohort(C-index,0.804)still had good discrimination and good *** curve analysis revealed that the nomogram was clinically *** This novel nomogram,incorporating both clinical risk factors and circulating immune parameters,could be appropriately applied in preoperative individualized prediction of malignancy in patients with laryngeal neoplasm.

Objectives To establish a method for high yield mesenchymal stem cells collection, as well as a culture method for iden- tifying mesenchymal stem cells from the swine adipose-derived mesenchymal stem cell （ADMSC）. Methods Swine AD- MSCs were isolated from fat tissue with collagenase, followed by induction of differentiation to osteogenic, adipogenic and chondrogrnic cells. The survival curve of the ADMSC at the 37℃ and 38℃ were measured using WST-1 Cell Proliferation Assay Reagent. Result ADMSCs isolated with collagenase from swine neck fat tissue generated a stable uniform appearance af- ter the second generation. The passage period was five days. ADMSC could differentiate into osteogenic, adipogenic or chon- drogrnic cells under different culture conditions. The highest growth rate was achieved at 38℃in this study. Conclusion Swine ADMSCs have the potential to differentiate into osteogenic, adipogenic or chondrogrnic cells, and they may be appropriate for transplantation for both research and clinical purpose.

Postaurical injection of therapeutics was recently applied in clinical practice to treat inner ear diseases based on supposed existence of a direct channel from the postaurical area to the inner ear. Doubting on the associated reports and aiming to provide evidence on the inner ear uptake mechanism, the present study tracked the dynamic distribution of gadolinium-tetra-azacyclo-dodecane-tetra-acetic acid (Gd-DOTA) in rat inner ears after postaurical injection using MRI. A targeted tympanic medial wall delivery was utilized as control. The results showed that, at the early time points after postaurical injection, Gd-DOTA distributed mainly in tissues surrounding the bulla, temporal bone and skull and neck space. In the inner ear, there was gradual uptake of Gd-DOTA on both the ipsilateral and contralateral sides with equal signal intensities. There was no sign of direct channel carrying the agent from the postaurical area to the inner ear. Targeted tympanic medial wall delivery induced significantly greater uptake of Gd-DOTA in the inner ear than did postaurical injection. At 30 min post-administration, targeted tympanic medial wall delivery yielded 4.6-folds higher signal intensity than did postaurical injection. The total dose of Gd-DOTA delivered by the targeted tympanic medial wall approach was only 0.1% of that delivered by postaurical injection. In conclusion, postaurical injection is a systemic administration, which is similar to hypodermic injection, rather than a focal delivery method. By contraries, targeted tympanic medial wall delivery induces fast and abundant uptake of Gd-DOTA in the ipsilateral inner ear without significant distribution in unwanted areas.

Age-related hearing loss（AHL） is one of the most common sensory disorders among elderly persons. The inwardly rectifying potassium channel 5.1（Kir5.1） plays a vital role in regulating cochlear K~＋ circulation which is necessary for normal hearing. The distribution of Kir5.1 in C57BL/6J mice cochleae, and the relationship between the expression of Kir5.1 and the etiology of AHL were investigated. Forty C57BL/6J mice were randomly divided into four groups at 4, 12, 24 and 52 weeks of age respectively. The location of Kir5.1 was detected by immunofluorescence technique. The m RNA and protein expression of Kir5.1 was evaluated in mice cochleae using real-time polymerase-chain reactions（RT-PCR） and Western blotting respectively. Kir5.1 was detected in the type Ⅱ and Ⅳfibrocytes of the spiral ligament in the cochlear lateral wall of C57BL/6J mice. The expression levels of Kir5.1 m RNA and protein in the cochleae of aging C57BL/6J mice were down-regulated. It was suggested that the age-related decreased expression of Kir5.1 in the lateral wall of C57BL/6J mice was associated with hearing loss. Our results indicated that Kir5.1 may play an important role in the pathogenesis of AHL.

BACKGROUND Papillary cystadenoma is a rare benign epithelial tumor of the salivary gland,which is characterized by papillary structures and oncocytic cells with rich eosinophilic cytoplasm. We found only one case of papillary cystadenoma in nearly 700 cases of salivary gland tumors. Our case was initially mistaken for a tumor of the right temporomandibular joint(TMJ) capsule rather than of parotid gland origin. Preoperative magnetic resonance imaging(MRI) and computed tomography(CT) should be carefully studied, which allows for appropriate preoperative counseling and operative *** SUMMARY Here, we report an unusual case of a 54-year-old woman with a parotid gland papillary cystadenoma(PGPC) that was misdiagnosed as a tumor of the right TMJ capsule. She was initially admitted to our hospital due to a mass anterior to her right ear inadvertently found 5 d ago. Preoperative CT and MRI revealed a well circumscribed tumor that was attached to the right TMJ capsule. The patient underwent a resection through an incision for TMJ, but evaluation of an intraoperative frozen section revealed a benign tumor of the parotid gland. Then we removed part of the parotid gland above the temporal facial trunk. The facial nerve was preserved. Postoperative histopathological findings revealed that the tumor was PGPC. No additional treatment was performed. There was no recurrence during a 20-mo follow-up *** The integrity of the interstitial space around the condyle in MRI or CT should be carefully evaluated for parotid gland or TMJ tumors.

The study reports a case of a 5-year-old Chinese girl diagnosed with Kabuki make-up Syndrome(KMS).The patient showed classic KMS appearance:widely separated eyes,ectropion of lateral one-third lower eyelids,flat nasal tip,and prominent *** features in this individual included bilateral severe sensorineural hearing loss and lack of 40 Hz AERP responses identified at I year of *** individual received cochlear implant(CI) in the left ear when 5 years *** rehabilitation after CI treatment were 3 in speech intelligibility and 5 in auditory ***,our findings suggest that cochlear implant may be helpful to restore hearing for individuals with Kabuki syndrome.

Objective To report experiences with use of otoendoscopy in cerebellopontine angle(CPA) *** Twenty five cases of CPA surgeries performed between November 2002 and December 2008 in which microscope enabled otoendoscopy was used were *** 25 cases included 19 cases of acoustic neuroma,3 cases of CPA facial nerve tumors,1 case of trigeminal neurinoma,a case of glossopharyngeal neuralgia and 1 case of hemifacial *** was used in all cases together with monitoring of brainstem auditory responses and facial *** hearing and facial nerve function were evaluated and compared to pre-operative *** Endoscopy provided improved visualization of local anatomy,revealed hidden lesions and reduced unnecessary anatomical *** resection was achieved in 18 of the 19 acoustic neuroma cases,Facial nerve anatomical integrity was preserved in all 19 *** week postoperative House-Brackmann grading was I in 3 cases,Ⅱ in 10 cases and Ⅲ in 6 *** nerve function continued to improve in some cases at 3 *** tumor resection was achieved in all 3 patients with facial *** facial nerve was sacrificed in 2 of the 3 cases with primary faciohypoglossal nerve *** nerve function was Grade Ⅱ and Grade III one year after surgery,*** the case with anatomically preserved facial nerve,postoperative facial nerve function was initially Grade Ⅲ and improved to Ⅱ at 3 *** tumor was completely resected in the trigeminal neurinoma patient with a Grade Ⅲ postoperative facial nerve function which improved Grade II three months *** of the 19 patients with acoustic neuroma retained hearing postoperatively,of these 12 maintained preoperative levels of *** hearing capacity was preserved in 2 of the 3 patients with facial nerve tumors,but lost in patients with other tumor *** neurotomy(n=1) and microvascular decompression(n=1) re
