One of the most vexing problems in AIHA is handling the acute situation where all blood is incompatible and the patient has severe,worsening anemia. Since the panagglutinin in the patient's serum typically reacts with all donor red cells,crossmatching donor blood is a difficult and time consuming process and probably of little benefit. The most pressing problem in a patient with previous pregnancies or transfusions is detecting alloantibody which may be hidden by the autoantibody. Sophisticated immunohematology laboratories can use a combination of procedures including differential adsorption and warm autoadsorption to identify underlying alloantibodies.

Objective:To study the therapy effect of ultraviolet blood irradiation and oxygenation （UBIO） on blood AChe activity and lung injury due to acute soman intoxication in rabbits. Methods:Forty rabbits were randomly divided into 4 groups: normal control group, intoxication group, routine therapy group and UBIO therapy group. blood AChe activity and artery blood gas were analyzed 2 h after intoxication. ACP and AKP activities in BALF were determined respectively. Results:blood AChe activity in intoxication group was lower than that in normal control group （Pblood AChe activities in UBIO therapy group increased and were higher than that in intoxication and routine therapy groups. Compared with intoxication group, BALF ACP and AKP activities were decreased （Pblood pH, PaO2 and SaO2 increased （Pblood AChe activity and alleviate lung injury induced by soman intoxication. So it may be a new way to treat acute soman intoxication.

AIM:To investigate the relationship between low-dose aspirin-induced small bowel mucosal damage and blood flow,and the effect of rebamipide. METHODS:Ten healthy volunteers were enrolled in this *** subjects were divided into two groups:a placebo group given low-dose aspirin plus placebo and a rebamipide group given low-dose aspirin plus rebamipide for a period of 14 *** endoscopy and contrast-enhanced ultrasonography were performed before and after administration of *** under the curves and peak value of time-intensity curve were calculated. RESULTS:Absolute differences in areas under the curves were-1102.5(95%CI:-1980.3 to-224.7,P=0.0194) in the placebo group and-152.7(95%CI:-1604.2 to 641.6,P=0.8172) in the rebamipide group. Peak values of time intensity curves were-148.0(95% CI:-269.4 to-26.2,P=0.0225) in the placebo group and 28.3(95%CI:-269.0 to 325.6,P=0.8343) in the rebamipide *** endoscopy showed mucosal breaks only in the placebo group. CONCLUSION:Short-term administration of low-dose aspirin is associated with small bowel injuries and blood flow.

Developing a practical means of reducing alloimmunization in chronically transfused patients would be of significant clinical benefit. Immunocamouflaging red blood ceils （RBCs） by membrane grafting of methoxypoly（ethylene glycol）（mPEG） may reduce the risk of allo-immunization. The results of this study showed that antibody recognition of non-ABO antigens was sig- nificantly reduced in an mPEG-dose- and polymer size-dependent manner, with higher molecular weight mPEGs providing better immunoprotection. Furthermore, in vivo immunogenicity was significantly reduced in mice serially transfused with mPEG-modified xenogeneic （sheep; sRBCs）, allogeneic （C57B1/6）, or syngeneic （Balb/c） RBCs. Following a primary transfu- sion of sRBCs, mice receiving mPEG-sRBCs showed a 〉90% reduction in anti-sRBC IgG antibody levels. After two transfusions, mice receiving mPEG-sRBCs showed reductions of 〉80% in anti-sRBC IgG levels. Importantly, mPEG-modified autologous cells did not induce neoantigens or an immune （IgG or IgM） response. These data suggest that the global immuno- camouflage of RBCs by polymer grafting may provide a safe and cost-effective means of reducing therisk of alloimmunization.

In recent years, there have been an increasing number of hematologic patients in Vietnam. Hematopoietic stem cell transplantation （HSCT） is fast becoming an effective method to cure many benign and malignant blood disorders. Nine transplant centers are operating throughout Vietnam. HCMC blood transfusion Hematology Hospital （BTH） is one of these centers where bone marrow, peripheral blood and cord blood stem cell transplantation were first conducted in our country. At BTH, half of the patients received allogeneic HSCT, mainly from matched sibling donors due to lack of a national bone marrow donor registry. While acute myeloid leukemia was a common indication of allogeneic HSCT, most of the patients undergoing autologous HSCT had multiple myeloma. One major issue we encountered concerned bacterial and fungal infections during neutropenia period. Because of an endemic area, CMV reactivation frequently occurred. Five-year overall survival （OS） and event-free survival （EFS） were 47% and 46%, respectively. To create more opportunities for patients, our future directions will focus on some new transplant procedures, such as haploidentical, unrelated and cord blood transplantation.

BACKGROUND Acquired hemophilia A (AHA) is a rare and potentially severe bleeding disordercaused by circulating autoantibodies against factor Ⅷ (FⅧ). In approximately50% of the patients, the condition is associated with autoimmune diseases,cancers, medication use, pregnancy, and the post-partum period. Bullous pemphigoid(BP) is a chronic autoimmune subepidermal blistering disease associatedwith tissue-bound and circulating autoantibodies against BP antigens 180 (BP180)and 230 (BP230). AHA-associated BP has a high mortality rate;hence, the understandingof this disease must *** SUMMARY A 69-year-old man presented with erythema, blisters, blood blisters, and crustsaccompanied by severe pruritus for more than 20 days, and ecchymosis andswelling on his left upper arm for 3 days. Pathological examination revealed asubepidermal blister that contained eosinophils. Laboratory tests showed that theBP180 autoantibody levels had increased, isolated activated partial thromboplastintime was notably prolonged (115.6 s), and coagulation FⅧ activity wasextremely low (< 1.0%). Furthermore, the FⅧ inhibitor titer had greatlyincreased (59.2 Bethesda units). Therefore, the patient was diagnosed as having BP associated with AHA, prescribed 0.05% topical halometasone cream, and transferred to a higher-level hospitalfor effective treatment;however, he died after 2 *** AHA associated BP is rare, dangerous, and has a high mortality rate. Therefore, its timely diagnosis and effectivetreatment are necessary.

Inflammatory bowel diseases (IBDs) are a group of chronic inflammatory conditions mainly of the colon and small intestine. Crohn’s disease (CD) and ulcerative colitis (UC) are the most frequent types of IBD. IBD is a complex disease which arises as a result of the interaction of environmental, genetic and immunological factors. It is increasingly thought that alterations of immunological reactions of the patients to their own enterable bacteria (microfilm) may contribute to inflammation. It is characterized by mucosal and sub mucosal inflammation, perpetuated by infiltration of activated leukocytes. CD may affect the whole gastrointestinal tract while UC only attacks the large intestine. The therapeutic goal is to achieve a steroid-free long lasting remission in both entities. UC has the possibility to be cured by a total colectomy, while CD never can be cured by any operation. A lifelong intake of drugs is mostly necessary and essential. Medical treatment of IBD has to be individualized to each patient and usually starts with anti-inflammatory drugs. The choice what kind of drugs and what route administered (oral, rectal, intravenous) depends on factors including the type, the localization, and severity of the patient’s disease. IBD may require immune-suppression to control symptoms such as prednisolone, thiopurines, calcineurin or sometimes folic acid inhibitors or biologics like TNF-α inhibitors or anti-integrin antibodies. For both types of disease (CD, UC) the same drugs are available but they differ in their preference in efficacy between CD and UC as 5-aminosalicylic acid for UC or budesonide for ileocecal CD. As therapeutic alternative the main mediators of the disease, namely the activated pro-inflammatory cytokine producing leukocytes can be selectively removed via two apheresis systems (Adacolumn and Cellsorba) in steroid-refractory or dependent cases.
