Introduction: Paraquat (1,1’-dimethyl-4,4’-bipyridine-dichloride) is a herbicide used to combat weeds and improve crop productivity. Its acute toxicity can cause fatal poisoning in humans and animals and several studies have shown a strong correlation between paraquat and Parkinson’s disease. Objective: To determine the concentration of paraquat in different types of rice in Brazil. Method: Paraquat quantification is based on a complexation reaction with sodium dithionite, which generates a blueish color compound, whose absorbance was read in a spectrophotometer at the 600 nm wavelength. Results: Five samples were found to be above the maximum allowed limit, while one sample had a concentration in the upper allowed limit. The method employed did not allow quantification of black rice. Conclusion: There is a need for identification of the type of pesticide used in each examined crop as well as their respective dates of remission in order to improve the safety of agricultural practices.

The flavonoid content in orange peels of different Brazilian citrus varieties such as bahia, lima, lima-of-persian, morcote, pera, ponkan, seleta, cravo, kinkan and pomelo was assessed. Industry processing juice wastes such as bagasse, bagasse residues, animal feeding bagasse, pulp WEUE and CORE-wash were also analyzed. The HPLC analysis indicates that the most abundant flavonoids found in these Brazilian citrus peels are hesperidin and naringin. The solvents used are selective for flavonoid extraction, and depending on their polarity, glycoside or aglycone flavonoids are extracted. The use of multivariate analysis shows that DMSO is the best solvent to extract glycosides flavanones while hexane displays high selectivity in the extraction of polymethoxylated flavones. The flavonoids present in the orange wastes, obtained at different stages of the industrial processing, are qualitative and quantitatively different. The identification and quantification of the flavonoid composition in each Brazilian citrus variety were evaluated and allowed the selection of the best solvent for the extraction of each specific class of flavonoids. These compounds were found to be more abundant in the fruit peels than in their juices, revealing their great industrial potential. The residual portion of the processing juices is also rich in flavonoids, depending on the processing step.

Objective:To assess the role of polymerase chain reaction(PCR)in serum sauples,in the diagnosis of osteoarticular tuberculosis(OTB)in a setting where only clinical and imaging diagnoses determine the ***:A total of 44 consecutive serum specimens were collected from clinically suspected OTB patients,based on clinical and radiological[X-ray or magnetic resonance imagng/computecl tomography]*** were scrcened by in-house nested *** addition,a few specimens were examined by Gram stain,acid-fast bacilli stain,histand routine bacterial culture.A total of 39 specimens were collected from patients suffering from other bone diseases of nontuberculous origin and included as negative ***:of the 44 clinically suspected OTB patients,in-house nested PCR was positive in 40(91%)cases;PCR was negative in 38(97%)negative *** and specificity of our in—house nested PCR was 90.3%and 97.4%,*** PCR report was available within 48 *** was possible to standardize serum PCR technique and in positive cases,a good n was observed in terms of an adequate treatment ***:Nested PCR in serum samples is a rapid,highly sensitive and specific modality for OTB detection,PCR should be performed in addition to clinical evaluation,imaging studies,acidfast bacilli staining,culture and histopathology diagnosis,if possible.

Background: Adolescence includes a period (10 to 19 years) of profound biopsychosocial changes, constituting potentially difficult challenges, which may become more pronounced in the presence of a chronic condition and its limitations. Responses are not homogeneous and can be quite variable, depending on various specific individual factors. Research comparing adolescents with or without chronic illness, or comparing across different conditions, has been contradictory, not confirming a direct relationship between the degree of suffering and the chronic condition. Objective: To characterize and assess the impact of 1) having a chronic condition (CC) and 2) how CC affects school participation;and its association with life satisfaction and perception of wellness, controlling for demographic factors: age, gender and family socioeconomic status (SES). Methods: 5050 Portuguese adolescents with an average age of 14 years participated in the Health Behaviour in School-aged Children/WHO (HBSC). Results: The majority of the adolescents with CC reported that their conditions did not affect school participation. Adolescents with CC who indicated that CC affected school participation felt more frequently unwell and presented lower life satisfaction. Being a boy, younger and having high family socio economic status (SES) were identified as predictors of higher life satisfaction;on the other hand, being a girl, older, having lower SES, living with CC and feeling that CC affects school participation are predictors for feeling more frequently unwell. Conclusions: These adolescents showed an increased vulnerability, presenting internalized symptoms and lower life satisfaction. Furthermore, when there was simultaneous occurrence of living with CC and that CC affected school participation, the impact was even higher. Thus, it is crucial that future interventions should include the identified predictors, combined with “listen to the voice” of adolescents, throughout the adaptation proces

Background:Acre has reported the highest incidence of American cutaneous leishmaniasis(ACL)in Brazil in recent *** present study seeks to identify high and low risk agglomerations of ACL in space and space-time during the period from 2007 to 2013 in Acre,and also to characterize the occurrence of the disease in time and according to sociodemographic ***:This is an ecological study,the study population of which consisted of autochthonous ACL cases notified in the municipalities of Acre by an epidemiological surveillance *** statistics of SaTScan^(TM)software were used to identify spatial and space-time *** addition,the cases were characterized by sex,age,home situation(in a rural or urban area),and temporal ***:Acre reported an incidence rate of 12.4 cases per 10000 inhabitant-years in the study period,with the rates varied greatly(standard deviation of 21.8)among their 22 *** agglomeration of high risk and three of low risk were detected in space and *** of the five micro-regions of Acre presented a stationary temporal *** profile of transmission varied according to the *** speaking,the disease occurred more often among young people,those of male gender,and those living in rural ***:Acre has stood out within the Brazilian national context due to its high rates of ACL incidence in the central region of the Acre *** high rates in the micro-region of Brasileia are related to the disease’s intra/peridomiciliary occurrence,and it would seem that the municipality of Sena Madureira is approaching a transmission pattern similar to that of *** other micro-regions,the profile of the disease’s transmission is mainly related to the forest/sylvatic cycle of ACL.

Punica granatum *** known antimicrobial and anti-inflammatory *** of different parts of this plant have been commonly used in the folk medicine to prevent and treat infections;and studies have supported the use of *** extracts for this ***,little is known of the effects of ***-derived fractions in severe infectious diseases such as ***,we investigated the actions of the hexane fraction(HF),rich in lipophilic compounds,obtained from the leaves of this plant,in a murine model of polymicrobial *** pre-treatment with HF increased mortality in septic *** same animals presented with lower levels of interleukin-6,nitric oxide and hydrogen peroxide in their peritoneal lavage samples than vehicle *** also increased polymorphonuclear cell accumulation in to the peritoneum of mice with ***-derived nitric oxide upon lipopolysaccharide stimuli was markedly reduced by incubation with ***,triterpenes,phytosterols,vitamin E and ascorbyl palmitate were detected in the *** results indicate that *** leaf lipophilic fractions may worsen sepsis *** effect may be associated with its bioactive compounds which act synergistically or not,increasing sepsis mortality in vivo.

国际糖尿病足工作组(IWGDF)自1999年以来一直以循证为基础发布糖尿病足病的预防和管理指南。本文中针对日常临床工作中糖尿病足溃疡的分类提出了新指南,并对既有的分类进行了综述。新指南只对现患糖尿病足溃疡进行分类,不包括用于定义未来溃疡风险的分类系统。本指南的制定是基于对现有文献的综述以及专家对最大程度影响临床结局的8个关键因素的鉴定。分类级别是根据关键因素的数量、内部和外部有效性验证以及使用意向而制定的。判定分类评分的关键因素有3种:患者相关的因素(终末期肾衰竭)、肢体相关的因素(周围动脉病变和保护性感觉丧失)和溃疡相关的因素(面积、深度、部位、单发或多发以及感染)。特殊分类系统要考虑以下5种临床情况:①卫生专业人员之间交流;②预测单个溃疡预后;③对个案临床决策的辅助;④伤口评估,有感染或无感染及周围动脉病变(评估灌注及血管重建的潜在益处);⑤用于地方、区域或国家人群结局的稽查。指南推荐:①医务人员之间交流使用SINBAD系统。②无适用于预测个体溃疡预后的分类。③感染的评估采用美国传染病学会/国际糖尿病足工作组(IDSA/IWGDF)分类系统。④评估灌注及血管重建的潜在获益使用创面、缺血、足感染(WIfI)系统。⑤人群结局的稽查使用SINBAD系统。

国际糖尿病足工作组(IWGDF)1999年发布了关于预防和管理糖尿病足病的循证指南。该指南旨在预防糖尿病患者的足部溃疡,并在2015年,IWGDF对预防指南进行了更新。我们遵循GRAde(grading of recommendations assessment development and evaluation)的评分方法,采用以患者为观察对象、有干预措施、与对照组进行比较、观察结局(patient-intervention-comparison-outcome,PICO)的方法来设计临床问题和关键成果,对医学科学文献进行系统评价,并撰写推荐意见及其理由。这些推荐基于系统评价中发现的证据质量、专家意见,以及益处及危害的权重、患者偏好、可行性和适用性以及与干预相关成本的权衡。我们建议对处于极低溃疡风险的患者每年做一次筛查,了解其是否有周围神经病变和外周血管病变,对高风险人群检查的频率要更高,以筛查其他危险因素。为了预防足溃疡,要对有危险因素的患者进行健康教育,教育患者适当地对足部进行自我护理以及对溃疡前症状进行有效治疗。指导中高危患者穿着合适的治疗鞋,并对其足部皮肤温度进行监测。处方治疗鞋在行走过程中具有明显的减轻足底压力的效果,能够防止足底溃疡复发。急性足溃疡或即将发生的足溃疡保守治疗失败,应考虑手术干预。我们不建议使用神经松解减压术。对高危患者给予综合足部医疗护理,以防溃疡复发。贯彻这些推荐将有助于医疗专业人员对有足溃疡风险的糖尿病患者提供更好的护理,降低糖尿病足溃疡的发生率,使足部健康维持更长的时间即增加无足病的天数,显著减少糖尿病患者和医疗机构的负担。
