ACE gene missense mutation in a case with early-onset, rapid progressing dementia

The population of early-onset Alzheimer's disease(EOAD)accounts for 1%-2%of the total population of Alzheimer's disease,and genetic mutations are more common in EOAD.The first symptom of the patient in the present case report was the decline in memories of recent events,and the disease progressed rapidly in the following 2 years.Genetic testing has revealed the presence of genetic mutations(c.A479G,p.N160S)of ACE,which causes the 160th codon of the ACE protein to change from aspartic acid to serine,and at the same time genotype of apolipoprotein E(APOE)is ε3/ε4.We think that this patient carries the mutation type of the sensitive gene ACE and the risk gene APOE of Alzheimer's disease,and this is the reason why the disease progressed rapidly.Moreover,we discussed ACE genetic mutation's meaning in EOAD progression.

Background Despite the availability of evidence-based treatments for posttraumatic stress disorder(PTSD),significant heterogeneity in the effectiveness of PTSD treatment persists,especially in community settings.Client demographics used to understand this variability in treatment outcome and dropout have yielded mixed results.Despite increasing evidence for the importance of attending to treatment engagement in community settings,few studies have explored client-level predictors.Aim The purpose of this study is to explore client-level predictors of treatment outcome and dropout beyond client demographics,and to identify client-level predictors of treatment engagement in community settings.Method Secondary data analysis was conducted with data collected as part of an implementation-effectiveness hybrid study of cognitive processing therapy(CPT)for PTSD in a diverse community healthcentre.Providers(n=19)treated(n=52)clients as part of their routine clinical care.Non-demographic client-level predictors included barriers to treatment,quality of life,session-level language and employment history assessed at baseline.Treatment engagement included number of weeks in the study,number of sessions with repeated CPT content,number of unique CPT sessions attended,frequency of session attendance and consistency of session attendance.Results Results showed language as a significant predictor of treatment engagement.There were significant differences between Spanish and English-speaking clients,with the former having a tendency to repeat more session content than the latter(β=1.4 sessions,p=0.003),and also less likely to attend treatment frequently(r=0.62,p=0.009)and consistently(r=0.57,p=0.027)if high logistical and financial barriers were endorsed.Irrespective of language,clients who reported high quality of life at baseline were less likely to repeat CPT session content(β=-0.3,p=0.04),and those with increased baseline barriers to treatment had deceleration in PTSD symptom improvement over time(β=-0.62,p<0.05).In terms of tre

AIM: To determine the effect of refractive error on temperament and character properties using Cloninger’s psychobiological model of personality.METHODS: Using the Temperament and Character Inventory(TCI), the temperament and character profiles of 41 participants with refractive errors(17 with myopia,12 with hyperopia, and 12 with myopic astigmatism) were compared to those of 30 healthy control participants.Here, temperament comprised the traits of novelty seeking, harm-avoidance, and reward dependence, while character comprised traits of self-directedness,cooperativeness, and self-transcendence.RESULTS: Participants with refractive error showed significantly lower scores on purposefulness,cooperativeness, empathy, helpfulness, and compassion(P <0.05, P <0.01, P <0.05, P <0.05, and P <0.01,respectively).CONCLUSION: Refractive error might have a negative influence on some character traits, and different types of refractive error might have different temperament and character properties. These personality traits may be implicated in the onset and/or perpetuation of refractive errors and may be a productive focus for psychotherapy.

Background Depression in adolescents is recognised as a global public health concern,but little is known about the trajectory of its clinical symptoms and pathogenesis.Understanding the nature of adolescents with depression and identifying earlybiomarkers can facilitatepersonalised intervention andreducediseaseburden.Aims To track multidimensional outcomes of adolescents with depression and develop objective biomarkers for diagnosis,as well as response to treatment,prognosis and guidance for early identification and intervention.Methods This is a multidimensional cohort study on the Symptomatic trajectory and Biomarkers of Early Adolescent Depression(sBEAD).We planned to recruit more than 1000 adolescents with depression and 300 healthy controls within 5 years.Multidimensional clinical presentations and objective indicators are collected at baseline,weeks 4,8,12 and 24,and years 1,2,3,4 and 5.Conclusions To the best of our knowledge,this is the first longitudinal cohort study that examines multidimensional clinical manifestations and multilevel objective markers in Chinese adolescents with depression.This study aims at providing early individualised interventions for young,depressed patients to reduce the burden of disease.

Background:Schizophrenia(SCZ)is a complex psychiatric disorder associated with widespread alterations in the subcortical brain structure.Hemispheric asymmetries are a fundamental organizational principle of the human brain and relate to human psychological and behavioral characteristics.We aimed to explore the state of thalamic lateralization of SCZ.Methods:We used voxel-based morphometry(VBM)analysis,whole-brain analysis of low-frequency fluctuations(ALFF),fractional amplitude of low-frequency fluctuations(fALFF),and resting-state seed-based functional connectivity(FC)analysis to investigate brain structural and functional deficits in SCZ.Also,we applied Pearson’’s correlation analysis to validate the correlation between Positive and Negative Symptom Scale(PANSS)scores and them.Results:Compared with healthy controls,SCZ showed increased gray matter volume(GMV)of the left thalamus(t=2.214,p=0.029),which positively correlated with general psychosis(r=0.423,p=0.010).SCZ also showed increased ALFF in the putamen,the caudate nucleus,the thalamus,fALFF in the nucleus accumbens(NAc),and the caudate nucleus,and decreased fALFF in the precuneus.The left thalamus showed significantly weaker resting-state FC with the amygdala and insula in SCZ.PANSS negative symptom scores were negatively correlated with the resting-state FC between the thalamus and the insula(r=-0.414,p=0.025).Conclusions:Collectively,these results suggest the possibility of aberrant laterality in the left thalamus and its FC with other related brain regions involved in the limbic system.
