Differentiation of periapical granulomas and cysts by using dental MRI: a pilot study

The purpose of this pilot study was to evaluate whether periapical granulomas can be differentiated from periapical cysts in vivo by using dental magnetic resonance imaging(MRI). Prior to apicoectomy, 11 patients with radiographically confirmed periapical lesions underwent dental MRI, including fat-saturated T2-weighted(T2 wFS) images, non-contrast-enhanced T1-weighted images with and without fat saturation(T1 w/T1 wFS), and contrast-enhanced fat-saturated T1-weighted(T1 wFS+C) images. Two independent observers performed structured image analysis of MRI datasets twice. A total of 15 diagnostic MRI criteria were evaluated, and histopathological results(6 granulomas and 5 cysts) were compared with MRI characteristics. Statistical analysis was performed using intraclass correlation coefficient(ICC), Cohen’s kappa(κ), Mann–Whitney U-test and Fisher’s exact test. Lesion identification and consecutive structured image analysis was possible on T2 wFS and T1 wFS+C MRI images. A high reproducibility was shown for MRI measurements of the maximum lesion diameter(intraobserver ICC = 0.996/0.998; interobserver ICC = 0.997), for the "peripheral rim" thickness(intraobserver ICC = 0.988/0.984; interobserver ICC = 0.970), and for all non-quantitative MRI criteria(intraobserver-κ = 0.990/0.995; interobserver-κ = 0.988). In accordance with histopathological results, six MRI criteria allowed for a clear differentiation between cysts and granulomas:(1) outer margin of lesion,(2) texture of "peripheral rim" in T1 wFS+C,(3)texture of "lesion center" in T2 wFS,(4) surrounding tissue involvement in T2 wFS,(5) surrounding tissue involvement in T1 wFS+C and(6) maximum "peripheral rim" thickness(all: P dental MRI enables a reliable differentiation between periapical cysts and granulomas in vivo. Thus, MRI may substantially improve treatment strategies and help to avoid unnecessary surgery in apical periodontitis.

Sealing ability of AH26 and soft core gutta-percha using different instrumentations techniques and irrigations protocol: A SEM evaluation

Aim: The aim of this ex vivo study, with SEM, was to determine the effects of the smear layer on the root canal systems after different instrumentation technique and irrigation protocol, obturated with carrier based gutta-percha (Soft Core) and AH26 sealer. Materials & Methods: Forty freshly extracted human teeth were used for this study. The teeth were divided into two major groups according to instrumentation technique (Manual and ProFile) and then each major group into four subgroups according to irrigation protocol (Saline solution, 17% EDTA, 5.25% NaOCl). All of the samples were obturated with AH26 and Soft Core Heating Gutta-percha. The evaluation of the objectives was performed using a Scanning Electron Microscope. Result: ProFile instrumentation followed with irrigation of 17% EDTA and 5.25% NaOCl, selaed with AH26 Soft Core Heating Guttapercha provided maximal contact at the S/D interface. The S/D interface had a serrated appearance due to effective penetration of the sealer into the patent dentin tubules. Conclusions: The adhezion of the respective sealer is more successful in cases when smear layer was removed completely.

Oral mucosal and salivary lipids exhibit potent antimicrobial activity for a variety of Gram-positive and Gram-negative bacteria;however,little is known about their spectrum of antimicrobial activity or mechanisms of action against oral *** this study,we examine the activity of two fatty acids and three sphingoid bases against Porphyromonas gingivalis,an important colonizer of the oral cavity implicated in *** inhibitory concentrations,minimal bactericidal concentrations,and kill kinetics revealed variable,but potent,activity of oral mucosal and salivary lipids against ***,indicating that lipid structure may be an important determinant in lipid mechanisms of activity against bacteria,although specific components of bacterial membranes are also likely *** micrographs showed ultrastructural damage induced by sapienic acid and phytosphingosine and confirmed disruption of the bacterial plasma *** information,coupled with the association of treatment lipids with *** lipids revealed via thin layer chromatography,suggests that the plasma membrane is a likely target of lipid antibacterial *** a combination of two-dimensional in-gel electrophoresis and Western blot followed by mass spectroscopy and N-terminus degradation sequencing we also show that treatment with sapienic acid induces upregulation of a set of proteins comprising a unique *** stress response,including proteins important in fatty acid biosynthesis,metabolism and energy production,protein processing,cell adhesion and *** or therapeutic lipid treatments may be beneficial for intervention of infection by supplementing the natural immune function of endogenous lipids on mucosal surfaces.

Occlusal tooth wear in human sculls of antique period from Vendenis and Municipium Dardanorum DD, Kosovo

Aim: To describe the presence, frequency of attrition in permanent dentition of human skulls from two ancient archeological locations. Aims: To determine the rate of attrition using a specific quantitative and qualitative method. Methods: Twenty one human skulls from two ancient archeological necropolises (Vendenis and Municipium Dardanorum DD) were included in the analysis of samples for this study. A total of 498 teeth were present in all human skulls. The skulls (with their accompanying teeth) were then divided into groups according to locality of origin, age and sex. Macroscopic (observational) analysis of the teeth relied on inspection and exploration of the teeth in their totality. The classification system that was used to rate tooth wear was Tooth Wear Index (TWI). Statistical analysis used: Chitest for small samples was used to evaluate the observations obtained from the study. Multiple Logistic Regression Analysis was not performed due to the small number of teeth and the content of zero values in respective age groups. Results: The results show that out of total 498 teeth, 89.35% had attrition and 10.65% had no attrition or Grade 0 TWI. 21.48% were Grade 1, 28.71% Grade 2, 26.90% Grade 3, 7.42% Grade 4 and 4.81% Grade 5. Conclusions: Grade 2 and higher rate of attrition, was predominantly present in first permanent molars.

The cleaning efficiency of the root canal after different instrumentation technique and irrigation protocol:A SEM analysis

Aim: The objective of this study was to evaluate the effectiveness of manual and ultrasonic instrumentation with varying irrigation protocols on removal of the smear layer from root canal walls. Methods: Forty extracted single rooted human teeth stored in 0.5% saline were used. Periodontal soft tissues were removed followed by crown separation at the CEJ. All the teeth were randomly divided into two groups. Manually Instrumented Group 1 was irrigated with 5.25% NaOCl and 17% EDTA alternately, same as Ultrasonically Instrumented Group 2. The controls for both groups were irrigated with saline solution. Results: Ultrasonic instrumentation and the combined use of two different solutions (5.25% NaOCl and 17% EDTA) yielded better results on smear removal. Conclusions: Irrigation with 17% EDTA followed by 5.25% NaOCl was successful in complete removal of smear layer on cervical and middle thirds of the root canals. Ultrasonic instrumentation was slightly more successful on the apical one third of the root canals.

In addition to its contributing role in the development of chronic liver diseases, chronic hepatitis C virus (HCV) infection is associated with extrahepatic manifestations, particularly, cutaneous-based disorders including those with pruritus as a symptom. Pruritus is frequently associated with the development of chronic liver diseases such as cholestasis and chronic viral infection, and the accumulation of bile acids in patients’ sera and tissues as a consequence of liver damage is considered the main cause of pruritus. In addition to their role in dietary lipid absorption, bile acids can trigger the activation of specific receptors, such as the G protein-coupled bile acid receptor (GPBA/ TGR5). These types of receptors are known to play a crucial role in the modulation of the systemic actions of bile acids. TGR5 expression in primary sensory neurons triggers the activation of the transient receptor potential vanilloid 1 (TRPV1) leading to the induction of pruritus by an unknown mechanism. Although the pathologic phenomenon of pruritus is common, there is no uniformly effective therapy available. Understanding the mechanisms regulating the occurrence of pruritus together with the conduction of large-scale clinical and evidence-based studies, may help to create a standard treatment protocol. This review focuses on the etiopathogenesis and treatment strategies of pruritus associated with chronic HCV infection.

Stress is an unavoidable consequence of life. It is described as adverse emotions or reactions to unpleasant experiences. Thus, stress can be viewed as a process with both psychological and physiological components. The possible mechanisms by which psychosocial factors act on periodontal tissues are oral hygiene negligence, changes in dietary intake, smoking, bruxism, gingival circulation, alteration in salivary component and flow, hormonal changes and lowered host resistance. During stress hypothalamic-pituitary-adrenal (HPA) axis and the sympathetic nervous system interact and release glucocorticoid which has myriad of effects that disrupt homeostasis and lead to increased susceptibility to periodontal diseases. The dental practioner, thus, may always decide to refer patients to appropriate professional for assistance and counselling. Therefore, reducing stress and improving coping strategies may improve the periodontal prognosis and treatment outcomes. The aim of this review is to provide an insight into the relationship between psychological stress and periodontal diseases.

Bond Strength of Brackets Bonded with Chemically and Light Activated Materials after Thermal Cycling

The aim of this study was to evaluate the bond strength of orthodontic brackets bonded to tooth enamel with different polymer composites after simulated clinical aging, and analyze the enamel/ bond system fracture interface. 80 human premolars, were randomly divided into five groups (n = 16). G1: CO (Concise Ortodontica—3M ESPE), G2: SB (SuperBond—Ortho Source), G3: FMO (Fill Magic Ortodontico—Vigodent), G4: TXT (Transbond XT—3M ESPE) and G5: TP (Transbond Plus Self Etching Primer + Transbond Plus Color Change—3M ESPE). In all groups, the metal orthodontic bracket was bonded to the tooth enamel surface. The specimens were submitted to 3000 thermal cycling baths for 30 s, at temperatures of 5°C, 37°C and 55°C (±1°C). Afterwards they were subjected to shear testing and the results were analyzed by the Student’s-t test (p G5 > G2 > G3. In all groups the majority of the fractures at the interface of the specimens were cohesive, except in G3, which presented the largest number of adhesive fractures. After clinical aging, the chemically activated material (CO) showed higher bond strength than the light activated types (FMO, TXT and TP). The self-etching adhesive system (TP) showed similar bond strength to that of conventional systems (FMO and TXT).

The inflammatory response of the pulp after direct capping with platelet-rich plasma and enamel matrix derivative:A controlled animal study

Aims: To evaluate the inflammatory response of the exposed pulp of incisor teeth in rats after direct pulp capping, using platelet rich plasma (PRP), enamel matrix derivate (EMD), mineral trioxide aggregate (MTA) and calcium hydroxide (Ca(OH)2). Methods: The study was conducted on 36 Wistar albino rats with a total of 144 incisor teeth. The pulps of 96 teeth of the rats were perforated and capped with different agents. Serving as the positive control group, the pulps of 24 teeth were only perforated and capped without capping agents, whereas the pulps of 24 teeth were used as the negative control group without being perforated (without any process). The research was ended with the extracting of the teeth on the 7th day-28th day. The teeth were taken to the routine and histological follows;cross sections were prepared and painted with hematoxylen & eosin. All of the sections were evaluated in terms of inflammatory reaction by histologic analysis taken by light microscope. Statistical analysis was used. The normal distribution of all data was tested with the Mann Whitney U and the differences between the groups were analyze dusing Kruskal Wallis test at 0.05 level. Results: There are no statistically significant differences in terms of inflammation type and necrosis among the treatment groups on 7 days’ post capping. However, improved inflammatory cell accumulation, hyperemia and lowest necrosis were observed from the samples treated with PRP (p < 0.05). Conversely, the EMD group indicated that the criteria of inflammation scores and hyper- emia were higher in the 28th day (p < 0.05). Conclusions: Most of cells accumulating in the PRP group and most necrosis were seen in the EMD group. These new PRP materials might serve as pulp capping biomaterials to induce initial healing response in the future.

Regulation of FN1 degradation by the p62/SQSTM1-dependent autophagy-lysosome pathway in HNSCC

Epithelial–mesenchymal transition(EMT)is involved in both physiological and pathological *** plays an essential role in the invasion,migration and metastasis of *** has been shown to regulate EMT in a variety of cancers but not in head and neck squamous cell carcinoma(HNSCC).Herein,we investigated whether autophagy also regulates EMT in *** of clinical data from three public databases revealed that higher expression of fibronectin-1(FN1)correlated with poorer prognosis and higher tumour pathological grade in *** from SCC-25 cells demonstrated that rapamycin and Earle’s balanced salt solution(EBSS)promoted autophagy,leading to increased FN1 degradation,while 3-methyladenine(3-MA),bafilomycin A1(Baf A1)and chloroquine(CQ)inhibited autophagy,leading to decreased FN1 *** the other hand,autophagic flux was blocked in BECN1 mutant HNSCC Cal-27 cells,and rapamycin did not promote autophagy in Cal-27 cells;also in addition,FN1 degradation was ***,we identified FN1 degradation through the lysosome-dependent degradation pathway using the proteasome inhibitor *** from immunoprecipitation assays also showed that p62/SQSTM1 participated as an autophagy adapter in the autophagy–lysosome pathway of FN1 ***,data from immunoprecipitation assays demonstrated that the interaction between p62 and FN1 was abolished in p62 mutant MCF-7 and A2780 cell *** results indicate that autophagy significantly promotes the degradation of ***,our findings clearly suggest that FN1,as a marker of EMT,has adverse effects on HNSCC and elucidate the autophagy–lysosome degradation mechanism of FN1.
