Importance:Preserving skin health is crucial for atopic dermatitis control as well as for the thriving of children.However,a well-developed and validated tool that measures the knowledge,attitude,and practice of skin care is lacking.Objective:To develop and validate the atopic dermatitis and infant skincare knowledge,attitude,and practice(ADISKAP 1.0)scale that measures parental health literacy on atopic dermatitis and skin care.Methods:We conducted a review of the literature,a focus group(two dermatologists and 12 parents),and a panel discussion in order to generate the ADISKAP prototype.Two samples of parents with knowingly superior(dermatologists,n=59)and inferior(general population,n=395)knowledge traits participated in the validation of ADISKAP.Cronbach’s alpha was reported as a measure of internal consistency,and the intraclass correlation coefficient(ICC)was calculated to assess the test-retest validity.The known-groups technique was used to evaluate construct validity.Results:The ADISKAP scale contained 17 items after content and face validity validation.After removing items that displayed poor test-retest reliability(n=4)and construct validity(n=3),12 items were retained in the ADISKAP 1.0.Interpretation:ADISKAP 1.0 is a reliable and valid tool for assessing parental knowledge,attitude,and practice on infantile atopic dermatitis and skin care.

Tuberous sclerosis complex combined with primary lymphedema:A case report

BACKGROUND Tuberous sclerosis complex(TSC)and primary lymphedema(PLE)are both rare diseases,and it is even rarer for both to occur in the same patient.In this work,we have provided a detailed description of a patient's clinical presentation,imaging findings,and treatment.And a retrospective analysis was conducted on 14 published relevant case reports.CASE SUMMARY A 16-year-old male came to our hospital for treatment due to right lower limb swelling.This swelling is already present from birth.The patient’s memory had been progressively declining.Seizures had occurred 1 year prior at an unknown frequency.The patient was diagnosed with TSC combined with PLE through multimodal imaging examination:Computed tomography,magnetic resonance imaging,and lymphoscintigraphy.The patient underwent liposuction.The swelling of the patient's right lower limb significantly improved after surgery.Epilepsy did not occur.after taking antiepileptic drugs and sirolimus.CONCLUSION TSC with PLE is a rare and systemic disease.Imaging can detect lesions of this disease,which are important for diagnosis and treatment.

Efficacy of Moxifloxacin against Mycobacterium abscessus in Zebrafish Model in vivo

Objective Moxifloxacin(MFX)shows good in vitro activity against Mycobacterium abscessus and can be a possible antibiotic therapy to treat M.abscessus infection;however,other studies have shown a lower or no activity.We aimed to evaluate MFX activity against M.abscessus using zebrafish(ZF)model in vivo.Methods A formulation of M.abscessus labeled with CM-Dil was micro-injected into ZF.Survival curves were determined by recording dead ZF every day.ZF were lysed,and colony-forming units(CFUs)were enumerated.Bacteria dissemination and fluorescence intensity in ZF were analyzed.Inhibition rates of MFX and azithromycin(AZM,positive control)were determined and compared.Results Significantly increased survival rate was observed with different AZM concentrations.However,increasing MFX concentration did not result in a significant decrease in ZF survival curve.No significant differences in bacterial burdens by CFU loads were observed between AZM and MFX groups at various concentrations.Bacterial fluorescence intensity in ZF was significantly correlated with AZM concentration.However,with increasing MFX concentration,fluorescence intensity decreased slightly when observed under fluorescence microscope.Transferring rates at various concentrations were comparable between the MFX and AZM groups,with no significant difference.Conclusion MFX showed limited efficacy against M.abscessus in vivo using ZF model.Its activity in vivo needs to be confirmed.

Treatment of severely calcified coronary artery disease by intravascular lithotripsy primary outcomes and 180-day followup from the Chinese SOLSTICE Trial

OBJECTIVE To assess the safety and effectiveness of intravascular lithotripsy(IVL)treatment for de novo coronary lesion involving severely calcified vessels in a Chinese population.METHODS The Clinical Trial of the ShOckwave Coronary IVL System Used to Treat CalcIfied Coronary ArtEries(SOLSTICE)was a prospective,single-arm,multicentre trial.According to the inclusion criteria,patients with severely calcified lesions were enrolled in the study.IVL was used to perform calcium modification prior to stent implantation.The primary safety endpoint was freedom from major adverse cardiac events(MACEs)at 30 days.The primary effectiveness endpoint was procedural success,defined as successful stent delivery with residual stenosishospital MACEs.The morphological changes of calcium modification were assessed by optical coherence tomography(OCT)before and after IVL treatment.RESULTS Patients(n=20)were enrolled at three sites in China.Severe calcification by core lab assessment was present in all lesions,with a mean calcium angle and thickness of 300±51°and 0.99±0.12 mm(by OCT),respectively.The 30-day MACE rate was 5%.Both primary safety and effectiveness endpoints were achieved in 95% of patients.The final in-stent diameter stenosis was 13.1%±5.7% with no patient had a residual stenosis<50%after stenting.No serious angiographic complications(severe dissection grade D or worse,perforation,abrupt closure,slow flow/no-reflow)observed at any time during the procedure.OCT imaging demonstrated visible multiplane calcium fracture in 80% of lesions with a mean stent expansion of 95.62%±13.33% at the site of maximum calcification and minimum stent area(MSA)of 5.34±1.64 mm^(2).CONCLUSIONS The initial coronary IVL experience for Chinese operators resulted in high procedural success and low angiographic complications consistent with prior IVL studies,reflecting the relative ease of use of IVL technology.

Bioinformatics analysis and experimental validation of cystathionine-gamma-lyase as a potential prognosis biomarker in hepatocellular carcinoma

Background:Hepatocellular carcinoma(HCC)is a common malignant tumor with poor prognosis and high mortality worldwide.Although cystathionine-gamma-lyase(CSE)plays an important role in the development of multiple tumors,the clinical implication and potential mechanisms of CSE in HCC development remain elusive.Methods:In our study,the CSE expression in HCC was analyzed in Gene Expression Omnibus(GEO)and The Cancer Genome Atlas(TCGA)datasets and further conﬁrmed by RT-qPCR and immunohistochemistry assays in HCC samples.Furthermore,the associations between CSE expression and HCC malignancy as well as survival were analyzed in GSE14520 and validated in HCC patients.Finally,the biological functions of CSE in HCC cells was assessed by CCK-8,ﬂow cytometry and Western blotting.Results:Lower transcriptional and proteomic CSE expressions were found in HCC tissues in contrast to adjacent normal tissues.Decreased CSE mRNA expression was signiﬁcantly associated with advanced clinicopathological features and poor outcomes in HCC patients from public database and our cohort.Following univariate and multivariate analyses of GSE14520 data showed that CSE expression was an independent prognostic indicator for the overall survival(OS)and recurrence-free survival(RFS)of HCC patients.In vitro experiments further explained that CSE might trigger HCC cell apoptosis by H2S.Conclusion:In summary,the present study identiﬁed the relationship between CSE expression and HCC malignancy as well as OS and RFS,indicating that CSE might be a potential prognostic biomarker and a novel therapeutic target for HCC.

Effect of Maximal Oxygen Pulse on Patients with Chronic Obstructive Pulmonary Disease

Objective This study evaluated the effect of maximal oxygen pulse(O_(2)P_(max))on patients with chronic obstructive pulmonary disease(COPD)and confirmed the predictive effect on acute exacerbations of COPD(AECOPD).Methods This retrospective study included 91 participants who underwent cardiopulmonary exercise testing(CPET),lung function testing,a dyspnea scale assessment,and a 3-year follow-up.The participants were divided into two groups according to the O_(2)P_(max)value.Exercise capacity,ventilatory conditions,gas exchange efficiency,and dyspnea symptoms were compared,and the correlations between O_(2)P_(max)and these indices were evaluated.The ability of O_(2)P_(max)to predict AECOPD was examined.Results Exercise capacity,ventilatory conditions,and gas exchange efficiency were lower,and dyspnea symptom scores were higher in the impaired O_(2)P_(max)group(P<0.05).O_(2)P_(max)was positively correlated with forced vital capacity(FVC)%,forced expiratory volume in 1 sec(FEV_(1))%,FEV_(1)/FVC%,anaerobic threshold(AT),work rate(WR)%,aximal oxygen uptake(VO_(2))%,VO_(2)/kg_(max),VO_(2)/kg_(max)%,WR_(AT),WR_(max),VO_(2AT),VO_(2max),and V,and was negatively correlated with EqCO_(2AT),and EqCO_(2max)(P<0.05).Most importantly,O_(2)P_(max)could be used to predict AECOPD,and the best cut-off value was 89.5%(area under the curve,0.739;95%CI,0.609–0.869).Conclusion O_(2)P_(max)reflected exercise capacity,ventilation capacity,gas exchange capacity,and dyspnea symptoms in patients with COPD and may be an independent predictor of AECOPD.

Childhood strokes in China describing clinical characteristics, risk factors and performance indicators: a case-series study

Aim To investigate clinical characteristics,risk factors(RFs),neurologic deficits and medical care provided in children who had a stroke in China.Methods We conducted a retrospective case-series study using the medical records of children aged 1 month to 18 years with arterial ischaemic stroke(AIS)or haemorrhagic stroke(HS)(with the entry codes I60,I61,I62,I63(ICD-10)),who were admitted to different hospitals in Beijing,between January 2018 and December 2018.We obtained the following information from the charts:demographic characteristics,clinical presentations,RFs for paediatric stroke,laboratory examination,neuroimaging records and neurologic sequelae.Results We identified 312 first admissions for stroke(172 AIS and 140 HS).The mean age at onset was 8.6±3.9 years for patients who had an AIS and 8(5–13)years for patients who had an HS.There were more males than females in both groups(AIS:59.88%vs 40.12%;HS:52.14%vs 47.86%).A known aetiology was identified in 92.44%and 86.43%of patients who had an AIS and HS,respectively.The leading cause of AIS was cerebrovascular diseases including moyamoya(68.6%),while that for HS was arteriovenous malformation(51.43%).The most common initial clinical presentation was hemiplegia(86.05%)in patients who had an AIS and headache(67.86%)in patients who had an HS.The use of healthcare,including acute treatment(antithrombotic in 17.44%,anticoagulant in 5.23%)and secondary prevention(antithrombotic in 6.39%,anticoagulant in 1.16%),varied and was significantly lower among patients who had an AIS.The most common complications were epilepsy(22.09%)and pneumonia(4.65%)in patients who had an AIS and epilepsy(17.14%)and hydrocephalus(12.14%)in patients who had an HS.Neurological deficits occurred in 62.8%of patients who had an AIS and 72.86%of patients who had an HS.Conclusion Cerebral arteriopathy was a major RF for both AIS and HS in children living in China.Large epidemiological studies are required to identify RFs to prevent stroke as well as appropriate interventions.

目的探讨由多学科合作运动管理在经皮冠状动脉介入治疗(Percutaneous Coronary Intervention,PCI)术后患者中应用的效果。方法2020年1—10月,54例PCI术后患者被随机分配到试验组(n=27)和对照组(n=27)。试验组接受多学科合作运动管理,对照组接受常规护理。PCI术后患者在其术后1个月时开始进行干预,干预时间2个月。干预前后分别采用6 min步行距离评估患者运动耐量,根据移动应用程序记录的患者运动情况评估患者运动依从性,采用冠心病介入治疗相关知识认知调查问卷、慢性病自我效能量表及领悟社会支持量表分别评估患者疾病相关认知、自我效能感及领悟社会支持情况。结果51例PCI术后患者完成此项研究(试验组25例,对照组26例)。干预2个月后,试验组患者运动依从性优于对照组,6 min步行距离(469.36±57.48比432.81±67.09)、冠心病PCI治疗相关知识(52.64±9.82比42.42±8.54)、慢性病自我效能感(42.40±8.04比36.88±7.73)、领悟社会支持(74.04±5.73比66.69±6.86)得分均高于对照组,差异均有统计学意义(P<0.05)。结论基于移动应用程序的多学科运动管理可有效提高PCI术后患者运动耐量、运动依从性、疾病相关认知、自我效能感及领悟社会支持。

Risk Factors for Intraoperative Pressure Injury in Aortic Surgery:A Nested Case-Control Study

Aims and Objectives:The aim of this study was to identify risk factors associated with an increased risk of intraoperative pressure injury in patients undergoing aortic surgery.Background:Intraoperative pressure injuries are some of the most signifi cant health problems in clinical practice.According to previous studies,patients undergoing aortic surgery are at high risk of developing an intraoperative pressure injury,with an incidence much higher than that associated with other types of cardiac surgery.Design:This was a nested case-control study.Methods:Following the STROBE checklist,a nested case-control approach was adopted in this study.A patient cohort was selected on the basis of inclusion and exclusion criteria from patients undergoing aortic surgery.Data were collected from these patients by means of a tailored questionnaire designed in-house.Patients with intraoperative pressure injury at the end of surgery were identifi ed as the case group,while the control group consisted of patients without intraoperative pressure injury.Patients in the groups underwent 1:1 matching based on age and sex.Initially,a single-factor analysis was conducted between the two groups.Subsequently,risk factors for intraoperative pressure injury were identifi ed through conditional logistic regression analysis with use of the variables that exhibited statistically signifi cant differences in the single-factor analysis.Results:A total of 400 patients were selected.Among these,167 patients experienced intraoperative pressure injury at an incidence rate of 41.8%.Strict preoperative bed confi nement,deep hypothermic circulatory arrest during surgery,application of norepinephrine or dopamine during surgery,and intraoperative skin wetting were associated with the occurrence of intraoperative pressure injury in patients undergoing aortic surgery.Conclusions:Nurses should thoroughly assess the risk of intraoperative pressure injury and implement appropriate preventative interventions,particularly in high-risk patients undergoing aortic

There is a strong association between abnormalities in central nervous system structure and function and increased risk for the development of hypertension.Comprehensive clinical studies and research using animal models,including brain imaging studies,have revealed effects of insula metabolites on hypertension associated with OSA.Alterations in the limbic system of the central nervous system are closely linked both to hypertension status and to myocardial changes that result from pressure overload.While therapy,including continuous positive airway pressure,have shown excellent results,new studies delineating mechanisms of action are needed.This review article summarizes current concepts regarding central nervous system connections to hypertension and cardiac pathology associated with obstructive sleep apnea（OSA）.The limbic system provides one such novel mechanism for resistant hypertension and related cardiac diseases that may be amenable to treatment in the setting of OSA.
