Background:Nucleos(t)ide analog(NA)in combination with peginterferon(PegIFN)therapy in patients with hepatitis B e antigen(HBeAg)-positive chronic hepatitis B(CHB)shows better effectiveness than NA monotherapy in hepatitis B surface antigen loss,termed"functional cure,"based on previous published ***,it is not known which strategy is more cost-effective on functional *** aim of this study was to analyze the cost-effectiveness of first-line monotherapies and combination strategies in HBeAg-positive CHB patients in China from a social ***:A Markov model was developed with functional cure and other five states including CHB,compensated cirrhosis,decompensated cirrhosis,hepatocellular carcinoma,and death to assess the cost-effectiveness of seven representative treatment ***(ETV)monotherapy and tenofovir disoproxil fumarate(TDF)monotherapy served as comparators,***:In the two base-case analysis,compared with ETV,ETV generated the highest costs with$44,210 and the highest quality-adjusted life-years(QALYs)with 16.78 *** with TDF,treating CHB patients with ETV and NA-PegIFN strategies increased costs by$7639 and$6129,respectively,gaining incremental QALYs by 2.20 years and 1.66 years,*** incremental cost-effectiveness ratios were$3472/QALY and$3692/QALY,respectively,which were less than one-time gross domestic product per ***-way sensitivity analysis and probabilistic sensitivity analyses showed the robustness of the ***:Among seven treatment strategies,first-line NA monotherapy may be more cost-effective than combination strategies in HBeAg-positive CHB patients in China.

Management of chronic hepatitis B before and after liver transplantation

Liver transplantation remains the only curative option for eligible patients with complications of chronic hepatitis B(CHB) infection,including severe acute hepatitis flares,decompensated cirrhosis,and hepatocellular carcinoma. In general,all patients with CHB awaiting liver transplantation should be treated with oral nucleos(t)ide analogs(NAs) with high barriers to resistance to prevent potential flares of hepatitis and reduce disease progression. After liver transplantation,lifelong antiviral therapy is also required to prevent graft hepatitis,which may lead to subsequent graft loss. Although combination therapy using NA and hepatitis B immune globulin(HBIG) has been the regimen most widely adopted for over a decade,recent studies have demonstrated that newer NAs with low rates of resistance are effective in preventing graft hepatitis even without the use of HBIG,achieving excellent long term outcome. For patients without pre-existing resistant mutations,monotherapy with a single NA has been shown to be effective. For those with resistant strains,a combination of nucleoside analog and nucleotide analog should be used. To date,clinical trials using therapeutic vaccination have shown suboptimal response,as CHB patients likely have an immune deficit against HBV epitopes. Future strategies include targeting different sites of the hepatitis B replication cycle and restoring the host immunity response to facilitate complete viral eradication.

慢性创面的缺氧微环境导致血管化不足,进而使得伤口延迟愈合.内皮细胞作为组成新生血管的关键细胞,在血管化过程中发挥着核心作用.本研究构建了CX3CR1功能化凋亡小体纳米囊泡(nABs).“Find-eat”策略通过受体-配体组合,靶向低氧微环境中高表达CX3CR1的内皮细胞(ECs),从而放大“Find-eat”信号,促进血管生成.作者首先通过化学诱导脂肪干细胞(ADSCs)凋亡获得ABs,然后通过优化的低渗处理-温和超声-药物混合-挤压一系列处理步骤,获得载有去铁胺(DFO)的功能化凋亡小体纳米囊泡.体外实验显示ABs纳米囊泡生物相容性良好;可以通过CX3CL1/CX3CR1诱导缺氧微环境中的内皮细胞实现对其的“Find-eat”,进而促进内皮细胞的增殖、迁移、成管能力.体内实验中,ABs纳米囊泡可以促进伤口快速闭合,并可通过释放“Find-eat”信号靶向内皮细胞的同时实现促血管药物的缓释,促进糖尿病大鼠创面的新生血管形成.这种能够通过释放双重信号靶向内皮细胞并实现促血管药物缓释的ABs纳米囊泡为慢性糖尿病创面的愈合提供了一种新思路.

Administration of modified Gegen Qinlian decoction for hemorrhagic chronic radiation proctitis: A case report and review of literature

BACKGROUND Hemorrhagic chronic radiation proctitis(CRP) is a common late complication of irradiation of the pelvis and seriously impairs life quality. There is no standard treatment for hemorrhagic CRP. Medical treatment, interventional treatment, and surgery are available, but they are limited in their applications due to nondefinite efficacy or side effects. Chinese herbal medicine(CHM), as a complementary or alternative therapy, may provide another option for hemorrhagic CRP *** SUMMARY A 51-year-old woman with cervical cancer received intensity-modulated radiation therapy and brachytherapy with a total dose of 93 Gy fifteen days after hysterectomy and bilateral adnexectomy. She received six additional cycles of chemotherapy with carboplatin and paclitaxel. Nine months after radiotherapy treatment, she mainly complained of 5-6 times diarrhea daily and bloody purulent stools for over 10 d. After colonoscopy examinations, she was diagnosed with hemorrhagic CRP with a giant ulcer. After assessment, she received CHM treatment. The specific regimen was 150 mL of modified Gegen Qinlian decoction(GQD) used as a retention enema for 1 mo, followed by replacement with oral administration of 150 mL of modified GQD three times per day for 5 mo. After the whole treatment, her diarrhea reduced to 1-2 times a day. Her rectal tenesmus and mild pain in lower abdomen disappeared. Both colonoscopy and magnetic resonance imaging confirmed its significant improvement. During treatment,there were no side effects, such as liver and renal function *** Modified GQD may be another effective and safe option for hemorrhagic CRP patients with giant ulcers.

Objective chronic pain represents a frequent and poorly understood public health issue. Numerous studies have documented the key significance of plastic changes along the somatosensory pain pathways in chronic pain states. Our recent study demonstrated that the c GMP-dependent protein kinase I（PKG-I） specifically localized in nociceptors constitutes a key mediator of hyperexcitability of primary sensory neurons and spinal synaptic plasticity after inflammation. However, whether PKG-I in nociceptors further affects the cortical plasticity in the ascending pain pathways under pathological states has remained elusive. Methods Behavioral surveys and immunohistochemistry as well as biochemical assays were performed. Results The immediate early gene c-fos and phosphorylated ERK1/2（pERK1/2） are considered reliable indicators for the neuronal activation status and it permits a comprehensive and large-scale observation of nociceptive neuronal activity along the ascending pain pathways subjected to tissue injury. In the present study, we systemically demonstrated that peripheral injury in PKGIfl/fl mice produced a significant upregulation of c-Fos or pERK1/2 over from the periphery to the cortex along the pain pathways, including dorsal root ganglion（DRG）, spinal dorsal horn, ventral posterolateral thalamus（VPL）, primary somatosensory hindlimb cortex（S1 HL）, anterior cingulate cortex（ACC）, basolateral amygdale, periaqueductal grey（PAG） and parabrachial nuecleus（PBN）. In contrast, very few cells in the above regions showed c-Fos or pERK1/2 induction in nociceptor specific knockout mice lacking PKG-I（SNS-PKG-I-/-mice）. Conclusion Our results indicate that PKG-I expressed in nociceptors is not only a key determinant of DRG hyperexcitability and spinal synaptic plasticity, but also an important modulator of cortical neuronal activity in pathological pain states and represent what we believe to be novel targets in the periphery for pain therapeutics.

disease-specific mi R-34a as diagnostic marker of nonalcoholic steatohepatitis in a Chinese population

AIM To assess disease-specific circulating micro RNAs(mi RNAs) in non-alcoholic steatohepatitis(NASH) *** A total of 111 biopsy-proven non-alcoholic fatty liver disease(NAFLD) or chronic hepatitis B(CHB) patients and healthy controls from China's Mainland were enrolled to measure their serum levels of mi R-122,-125 b,-146 b,-16,-21,-192,-27 b and-34 a. The correlations between serum mi RNAs and histological features of NAFLD were determined. The diagnostic value of mi RNA in NASH and significant fibrosis was analyzed and compared with that of cytokeratin-18(CK-18), fibrosis-4(FIB-4), and aspartate aminotransferase to platelet ratio index(APRI), *** Circulating mi R-122,-16,-192 and-34 a showed differential expression levels between NAFLD and CHB patients, and mi R-34 a had an approximately 2-fold increase in NAFLD samples compared with that of CHB samples(P disease-specific noninvasive biomarker for the diagnosis of NASH.
