Validation of proposed diagnostic criteria for allergic bronchopulmonary aspergillosis

To the Editor:Allergic bronchopulmonary aspergillosis (ABPA) is an allergic lung disease caused by hypersensitivity to antigens from Aspergillus,and it most commonly occurs in patients with asthma or cystic fibrosis.The main clinical characteristics of the disease are those related to elevated serum immunoglubulin (Ig) E level,positive Aspergillus fumigatus (A.fumigatus) specific IgE/IgG,repeated pulmonary infiltration,and bronchiectasis.However,due to the lack of unified diagnostic criteria and screening methods,ABPA is always underdiagnosed.In 2021,new diagnostic criteria were proposed for allergic bronchopulmonary mycosis (ABPM) in the Japanese population.The criteria were verified by Asano et al for the first time (so-called Asano criteria),and showed that compared with the existing diagnostic criteria (Rosenberg- Patterson criteria,International Society for Human and Animal Mycology [ISHAM] criteria),the Asano criteria showed better sensitivity and specificity for the diagnosis of ABPM.In this retrospective study,we modified the Asano criteria to verify its effectiveness for diagnosing ABPA.In recent years,latent class analysis (LCA) has been used to evaluate the performance of diagnostic criteria for diseases without diagnostic gold standards.[3] LCA is a form of finite mixture modeling which can integrate multiple variables to define a subset when the true outcome is unknown.In diagnostic tests,LCA can be used to define diagnostic and non-diagnostic groups.Previous studies also used "multidisciplinary team (MDT) diagnosis" or "physician diagnosis"as the reference standard to evaluate the diagnostic efficiency of the criteria.We compared the performance of the modified Asano criteria [Supplementary Table 1,http://***/CM9/B940] with existing criteria (Rosenberg-Patterson criteria [Supplementary Table 2,http://***/CM9/B940],ISHAM criteria [Supplementary Table 3,http://***/CM9/B940],and modified ISHAM criteria [Supplementary Table 4,http://links.***.com/CM9/B940] using LCA and MDT diagnosis of ABPA as the refe

Flexible bronchoscopy(FB)has become a standard of care for the triad of inspection,sampling,and treatment in critical care patients.It is an invaluable tool for diagnostic and therapeutic purposes in critically ill patients in intensive care unit(ICU).Less is known about its role outside the ICU,particularly in the intermediate care unit(IMCU),a specialized environment,where an intermediate grade of intensive care and monitoring between standard care unit and ICU is provided.In the IMCU,the leading indications for a diagnostic work-up are:To visualize airway system/obstructions,perform investigations to detect respiratory infections,and identify potential sources of hemoptysis.The main procedures for therapeutic purposes are secretion aspiration,mucus plug removal to solve atelectasis(total or lobar),and blood aspiration during hemoptysis.The decision to perform FB might depend on the balance between potential benefits and risks due to frailty of critically ill patients.Serious adverse events related to FB are relatively uncommon,but they may be due to lack of expertise or appropriate precautions.Finally,nowadays,during dramatic recent coronavirus disease 2019(COVID-19)pandemic,the exact role of FB in COVID-19 patients admitted to IMCU has yet to be clearly defined.Hence,we provide a concise review on the role of FB in an IMCU setting,focusing on its indications,technical aspects and complications.

Timely removal of oxidatively damaged proteins is crit- ical for cells exposed to oxidative stresses; however, cellular mechanism for clearing oxidized proteins is not clear. Our study reveals a novel type of protein modifi- cation that may play a role in targeting oxidized proteins and remove them. In this process, DSS1 （deleted in split hand/split foot 1）, an evolutionally conserved small protein, is conjugated to proteins induced by oxidative stresses in vitro and in vivo, implying oxidized proteins are DSS1 clients. A subsequent ubiquitination targeting DSSl-protein adducts has been observed, suggesting the client proteins are degraded through the ubiquitin- proteasome pathway. The DSS1 attachment to its clients is evidenced to be an enzymatic process modulated by an unidentified ATPase. We name this novel protein modification as DSSylation, in which DSS1 plays as amodifier, whose attachment may render target proteins a signature leading to their subsequent ubiquitination, thereby recruits proteasome to degrade them.

Lymph node regression grading of locally advanced rectal cancer treated with neoadjuvant chemoradiotherapy

Neoadjuvant chemoradiotherapy(nCRT)and total rectal mesenteric excision are the main standards of treatment for locally advanced rectal cancer(LARC).Lymph node regression grade(LRG)is an indicator of prognosis and response to preoperative nCRT based on postsurgical metastatic lymph node pathology.Common histopathological findings in metastatic lymph nodes after nCRT include necrosis,hemorrhage,nodular fibrosis,foamy histiocytes,cystic cell reactions,areas of hyalinosis,residual cancer cells,and pools of mucin.A number of LRG systems designed to classify the amount of lymph node regression after nCRT is mainly concerned with the relationship between residual cancer cells and regressive fibrosis and with estimating the number of lymph nodes existing with residual cancer cells.LRG offers significant prognostic information,and in most cases,LRG after nCRT correlates with patient outcomes.In this review,we describe the systematic classification of LRG after nCRT,patient prognosis,the correlation with tumor regression grade,and the typical histopathological findings of lymph nodes.This work may serve as a reference to help predict the clinical complete response and determine lymph node regression in patients based on preservation strategies,allowing for the formulation of more accurate treatment strategies for LARC patients,which has important clinical significance and scientific value.

Two synchronous somatostatinomas of the duodenum and pancreatic head in one patient

Somatostatinomas are extremely rare neuroendocrine tumors of the gastrointestinal tract,f irst described in the pancreas in 1977 and in the duodenum in 1979.They may be functional and cause somatostatinoma or inhibi-tory syndrome,but more frequently are non-functioning pancreatic endocrine tumors that produce somatostatin alone.They are usually single,malignant,large lesions,frequently associated with metastases,and generally with poor prognosis.We present the unique case of a 57-year-old woman with two synchronous non-function-ing somatostatinomas,one solid duodenal lesion and one cystic lesion within the head of the pancreas,that were successfully resected with a pylorus-preserving Whipple's procedure.No secondaries were found in the liver,or in any of the removed regional lymph nodes.The patient had an uneventful recovery,and remains well and symptom-free at 18 mo postoperatively.This is an extremely rare case of a patient with two synchro-nous somatostatinomas of the duodenum and the pancreas.The condition is discussed with reference to the literature.

The value of nasal nitric oxide measurement in the diagnosis of primary ciliary dyskinesia

Importance:Nasal nitric oxide(nNO)testing is a method used in the diagnosis of primary ciliary dyskinesia(PCD).It has not been evaluated in Chinese population.Objective:To establish a reference nNO value to assist in the diagnosis of PCD in Chinese children.Methods:nNO values were measured in children with PCD(n=36),cystic fibrosis(CF)(n=20),asthma(n=45),post-infectious bronchiolitis obliterans(BO)(n=41)and non-PCD/non-CF bronchiectasis(n=32).The receiver operating characteristic nNO value for the diagnosis of PCD was plotted and the area under the curve was calculated.Results:nNO values were significantly lower in children with PCD(median 25.66 nL/min)than in children with asthma(186.26±58.95 nL/min),BO(143.47±49.71 nL/min)and non-PCD/non-CF bronchiectasis(173.13±63.80 nL/min),but not in children with CF(90.90±43.20 nL/min).Notably however,no CF patient had an nNO valuereference value for PCD screening in Chinese children.

Over the years,accumulating evidence has confirmed the crucial role of cystic fibrosis transmembrane conductance regulator(CFTR)mutations in male infertility caused by congenital bilateral absence of the vas deferens(CBAVD),the most common cause of obstructive azoospermia(OA).1,2 The distribution and frequency of CFTR mutations in CBAVD patients vary considerably across countries and ethnic groups,and this suggests the need to develop regional variant panels to significantly improve diagnosis and management of infertile males.

fibrosis is characterized by the excessive extracellular matrix deposition due to dysregulated wound and connective tissue repair response.Multiple organs can develop fibrosis,including the liver,kidney,heart,and lung.fibrosis such as liver cirrhosis,idiopathic pulmonary fibrosis,and cystic fibrosis caused substantial disease burden.Persistent abnormal activation of myofibroblasts mediated by various signals,such as transforming growth factor,platelet-derived growth factor,and fibroblast growh factor,has been recongized as a major event in the occurrence and progression of fibrosis.Although the mechanisms driving organ-specific fibrosis have not been fully elucidated,drugs targeting these identified aberrant signals have achieved potent anti-fibrotic efficacy in clinical trials.In this review,we briefly introduce the aetiology and epidemiology of several fibrosis diseases,including liver fibrosis,kidney fibrosis,cardiac fibrosis,and pulmonary fibrosis.Then,we summarise the abnormal cells(epithelial cells,endothelial cells,immune cells,and fibroblasts)and their interactions in fibrosis.In addition,we also focus on the aberrant signaling pathways and therapeutic targets that regulate myofibroblast activation,extracellular matrix cross-linking,metabolism,and inflammation in fibrosis.Finally,we discuss the anti-fibrotic drugs based on their targets and clinical trials.This review provides reference for further research on fibrosis mechanism,drug development,and clinical trials.
