美国心脏超声协会（american society of echocardiography，ASE）始成立于1975年，长期以来一直鼓励对超声心动图检查质量的评估。为此，已发表了一些超声心动图检查的指南，并不断进行修订。1995年，ASE发表了一系列专门用于超声心动图持续质量改进（Continuous Quality Improvement，CQI）的建议。超声心动图检查的临床广泛应用以及超声技术、检查方法和图像判读的复杂性是进行CQI项目的原因之一。本文旨在阐述美国心脏超声协会与美国心血管麻醉医师协会针对围手术期超声心动图检查CQI项目所发表的建议和指南。在ASE已发表的有关CQI项目文章的基础上，本文将涉及如下内容：1）提出围手术期开展CQI的理由，2）确定围手术期超声心动图检查的项目内容，3）提出与围手术期超声心动图检查有关的CQI原则，4）评估CQI项目在围手术期的有效性。本文所提出的建议和操作指南适用于任何围手术期间的超声心动图检查，包括手术前、手术中和手术后。任何具有Ⅲ级水平超声心动图检查的单位可独立实施CQI项目。

The integrated clinical,laboratory and ultrasound approach is essential for the diagnosis,evaluation and monitoring of the patient's therapy in coronavirus disease 2019 *** ideal imaging approach in this context is not yet well *** X-ray is characterized by low sensitivity in identifying earlier lung ***"bedside"pulmonary ultrasound has an undeniable series of advantages in the patient at high infectious risk and can provide incremental data in the respiratory intensive care for the serial control of the individual patient as well as for the home delivery of the stabilized *** computed tomography shows high sensitivity but should not be routinely performed in all patients,because in the first 48 h it can be absolutely negative and in the late phase the imaging findings may not change the therapeutic *** should be limited to patients with hemodynamic instability to assess ventricular function and pulmonary pressures.

Transcatheter aortic valve replacement(TAVR) has been validated as a new therapy for patients affected by severe symptomatic aortic stenosis who are not eligible for surgical intervention because of major contraindication or high operative risk. Patient selection for TAVR should be based not only on accurate assessment of aortic stenosis morphology, but also on several clinical and functional data. Multi-Imaging modalities should be preferred for assessing the anatomy and the dimensions of the aortic valve and annulus before TAVR. Ultrasounds represent the first line tool in evaluation of this patients giving detailed anatomic description of aortic valve complex and allowing estimating with enough reliability the hemodynamic entity of valvular stenosis. Angiography should be used to assess coronary involvement and plan a revascularization strategy before the implant. Multislice computed tomography play a central role as it can give anatomical details in order to choice the best fitting prosthesis, evaluate the morphology of the access path and detect other relevant comorbidities. Cardiovascular magnetic resonance and positron emission tomography are emergent modality helpful in aortic stenosis evaluation. The aim of this review is to give an overview onTAVR clinical and technical aspects essential for adequate selection.

Sonographic markers for early diagnosis of fetal malformations

Fetal malformations are very frequent in industrialized *** advanced maternal age may affect pregnancy outcome adversely,80%-90%of fetal malformations occur in the absence of a specific risk factor for *** only effective approach for prenatal screening is currently represented by an ultrasound ***,ultrasound methods present two important limitations:the substantial absence of quantitative parameters and the dependence on the sonographer *** recent years,together with the improvement in transducer technology,quantitative and objective sonographic markers highly predictive of fetal malformations have been *** markers can be detected at early gestation(11-14 wk)and generally are not pathological in themselves but have an increased incidence in abnormal ***,prenatal ultrasonography during the second trimester of gestation provides a"genetic sonogram",including,for instance,nuchal translucency,short humeral length,echogenic bowel,echogenic intracardiac focus and choroid plexus cyst,that is used to identify morphological features of fetal Down’s syndrome with a potential sensitivity of more than 90%.Other specific and sensitive markers can be seen in the case of cardiac defects and skeletal *** the future,sonographic markers could limit even more the use of invasive and dangerous techniques of prenatal diagnosis(amniocentesis,etc.).
