Congenital unilateral lower lip palsy (CULLP), or congenital hypoplasia of the depressor anguli oris muscle, also known as asymmetric crying facies, is a rare condition that results in asymmetry of the lower lip during smiling, laughing, and crying. Although the etiology is unknown, weakness of the depressor labii inferioris (DLI) muscle is implicated as a contributing factor. Currently, no well-established treatment options are available. This case report describes an 18-year-old male patient diagnosed with CULLP. Physical examination revealed a symmetric face at rest, but asymmetry when smiling and opening the mouth. Following the administration of lidocaine into the affected DLI muscle, the patient's smile and lower lip symmetry were immediately restored without any adverse effects. Subsequently, administration of botulinum toxin for neuromodulation of the DLI muscle led to a significant improvement in symmetry and oral function within 2 weeks, which was sustained at 1 month and 3 months post-treatment. No adverse effects were reported, and both patients and families expressed high satisfaction with the outcomes. This case highlights the potential use of neuromodulation as a minimally invasive and effective treatment for CULLP. © 2024 China Medical Cosmetology Press Co. Ltd.

Burninjury significantly impacts the victim's long-term quality of life, both physical y and psychosocial y. This prospective, observational study aimed to assess the physical and psychological health status in adult burn survivors in Sydney Australia using the burns Specific Health Scale-Brief Version (BSHS-B) questionnaire, together with analysis of the baseline demographic data col ected from medical records. A total of 24 adult acute burn victims admitted consecutively to the burns unit at concord Repatriation General hospital, Sydney, Australia between March 2007 and February 2009 fulfil ed the inclusion criteria and participated in the study. The BSHS-B questionnaire (which includes nine domains or subscales) was administered to al 24 participants in person at time of discharge and by mail 6, 12, and 24 months post discharge. By 12 months, 11 participants dropped out and the final analysis was performed on the remaining 13 participants. The analyzed results showed that: 1) Perceived return to work was the only variable that continued to change with time at 12 months after discharge (P < 0.01);2) At 12 months;return to work was significantly correlated with simple functional ability (P < 0.05), heat sensitivity (P < 0.01), and treatment regimes (P < 0.05), but no longer with affect and body image as demonstrated at 6 months. In summary, our findings have shown that the perception of returning to work changes significantly with time post discharge and this perception is affected by certain subscales of the BSHS-B. Given that return to work is one of the most important outcome concerns and issues of recovery for adult burn injury victims and families, it is essential that therapists be aware of the factors influencing return to work and address these factors through a comprehensive rehabilitation program.

Background : Condyloma acuminatum (CA), also known as a genital wart (GW), is a sexually transmitted disease caused by human papillomavirus (HPV). HPV lesions, recurrence tendency, and risk of malignant formation are primarily dependent on the person’s immunity level. GW recurrence is a major challenge in CA treatment. The aim of this study was to explore how lesional auto-transplantation (LT) can be used to treat CA and decrease its recurrence. Methods : We treated CA through the preparation and implantation of tissue from nine CA patients in our dermatology clinic. Transplantation of small pieces of HPV lesions to the subcutaneous fat of the inguinal region was carried out with the help of a simple surgical method under local anesthesia. Patients were followed up for six to eight months. We searched PubMed and the Web of Science for treatment options for CA to compare our treatment method recurrence rate with existing treatment options. Results : During three months of follow-up, seven patients experienced no recurrence of condyloma lesions, while two patients experienced recurrence of small lesions of condyloma acuminatum. The recurrence rate of CA once treated by auto-transplantation was relatively low compared to other CA treatment options. Conclusions : Transplantation of HPV lesions to the inguinal area reduces the recurrence rate in sexually active individuals. The 2/9 recurrence after implantation could be due to poor hygiene, sexual relationships or immune factors.

Background: Recently, microbotulinum, a new technique that involves injecting botulinum toxin type A (BoNTA) microdroplets into superficial cutaneous tissue, has gained popularity. The precise distribution of BoNTA in the targeted area profoundly affects outcomes. Many factors may influence the effective area of BoNTA in the dermis. This study aimed to determine the dermal distribution properties of BoNTA to guide microbotulinum injection. Methods: Ten healthy males aged 18–65 years without BoNTA treatment in the previous year were recruited to receive intradermal injections in the chest and back. Ultrasound was used to ensure the intradermal delivery of injections and measure the dermal thickness. The minor iodine starch test was performed at baseline and 3 days, 7 days, 21 days, 1 month, and 2 months after treatment. Results: All participants received intradermal injections. The dermis was thinner on the chest (thickness, 0.20 ± 0.03 cm) than on the back (thickness, 0.39 ± 0.07 cm) (P<0.05). An injection in the thicker dermis had a significantly smaller effective area at every follow-up visit. The drug concentration did not affect the effective area except at 3 days after treatment. Injection speed did not influence the effective area at any follow-up visits. Conclusion: An injection in a thicker dermis leads to a smaller effective area for intradermal injections. When the BoNTA dose is the same, the drug concentration and injection speed do not matter. Trial registration: ChiCTR, ChiCTR2100046507. Registered May 18, 2021. Missed Pancreaticobiliary Malignancy: The Flaw of the Expedited Cholecystectomy

Background: Early cholecystectomy has been recommended for patients with acute cholecystitis and gallstone pancreatitis. However, patients with pancreaticobiliary malignancy may present acutely with similar symptoms. We hypothesize that the diagnoses of these malignancies may potentially be delayed as an unintended consequence of expedited cholecystectomies. This study reviews a cohort of patients who underwent pancreaticoduodenectomy (PD) to identify those who underwent a separate cholecystectomy before their PD. Methods: We retrospectively reviewed 162 PDs performed between 2012 and 2022. Data collected included: demographics, disease etiology and the presence of cholelithiasis. We identified patients who had a previous cholecystectomy and the time elapsed before PD as well as procedures done during the interval. We reported detailed case summaries on those patients who had a cholecystectomy within 1 year of PD. Results: In the entire cohort, mean age was 65 years, 54% were males, and 83% had a malignant reason for PD. Thirty-one patients had cholelithiasis with 23 (14%) patients having had previous cholecystectomy. Six patients had cholecystectomy within 1 year of PD. They had the following malignancies: ampullary—3, pancreas—1, cholangiocarcinoma—1 and neuroendocrine—1. Four of these patients had expedited cholecystectomy on their index hospital admission and were later found to have a periampullary malignancy with further work up. Conclusions: Pancreaticobiliary malignancies can be difficult to diagnose, and surgeons should not overlook these potential diagnoses when considering expedited cholecystectomy. Future studies in large cohorts are needed to identify high risk candidates who should undergo more detailed testing to exclude malignancy before proceeding with cholecystectomy.
