Objective To evaluate the efficacy and safety of administration of the formulated Persian herbal syrup on improving the symptoms of patients with constipation-predominant irritable bowel syndrome(IBS-C).Methods This study was conducted in 70 patients with IBS-C,who were recruited from 3 medical centers in Mashhad,Iran,from November 2017 to August *** patients were randomly assigned to 2 groups including treatment and placebo groups by block randomization,35 cases in each *** in the treatment group received 15 mL of anti-IBS syrup,thrice daily for 6 weeks and followed up for 4 *** syrup was also prepared through similar instruction,BP syrup without plant extract was *** outcome induding IBS Symptom Severity Scale(IBS-SSS)questionnaire and secondary outcomes in terms of Hospital Anxiety and Depression(HADS)questionnaires,the Bristol Stool Form Scale(BSFS)were completed and evaluated at weeks 6 and 10,*** indices were collected at the end of the treatment and Common Terminology Criteria for Adverse Events v4.0(CTCAE)was used to evaluate the adverse *** The response to treatment was 84.4%(27/32)in the treatment group and 46.4%(13/28)in the placebo group,respectively(P=0.002).Compared with pre-treatment,a significant decrease was found on the IBS-SSS and BSFS scores after 6-week intervention in both groups(P0.05).Side effects reported in the treatment group included 2 cases of headache during the first week of the onset of the treatment,1 case of drowsiness,1 case of increase in menstrual bleeding,which did not result in discontinuation of the *** the placebo group,1 case of exacerbation of the disease was *** Anti-IBS syrup significantly reduced th

Objective:To investigate the effect of Anethum graveolens(AG) extracts on the mounting frequency,histology of testis and epididymis,and sperm ***:Male rats induced by cold immobilization before treating with vehicle or AG extracts [50,150,and 450 mg/kg body weight(BW)] via gastric tube for consecutive 1,7,and 14 d were examined for mounting frequency,testicular phosphorylation level by immunoblotting,sperm concentration,sperm acrosome reaction,and histological structures of testis and epididymis,***:AG(50 mg/kg BW) significantly increased the mounting frequency on Days 1 and 7 compared to the control ***,rat testis treated with 50 mg/kg BW AG showed high levels of phosphorylated proteins as compared with the control *** histological analyses,AG extract did not affect the sperm concentration,acrosome reaction,and histological structures of testis and ***:AG extract enhances the aphrodisiac activity and is not harmful to sperm and male reproductive organs.

Persian medicine non-pharmacological therapies for headache：phlebotomy and wet cupping

OBJECTIVE: To describe the common causes of headache as well as the different methods of Phlebotomy(Fasd) and wet Cupping(Hijamat) based on Persian ***: In this study, we searched Google Scholar, Science Direct, Pub Med and Scopus to get any in vitro or clinical evidence of the efficacy and possible mechanisms of phlebotomy or wet cupping on different types of headaches. Also different types of headaches, according to PM, under the title of "Sodaa"; extracted from famous Persian medical text books with key words including "Sodaa","Bayze" and "Shaqiqe".RESULTS: According to PM, diseases(dystemperament) are created by abnormal alteration in temperament. Temperament is a monolithic quality yielded by interaction between opposing qualities of four elements that are coldness, hotness, dryness and wetness. Headaches are caused by dystemperament(sue-Mizaj) in brain or in total body that are divided into simple and corporal. Common forms of the latter type are Vaporal and Migrant headache(Bokhari and Rihi). Each of these headaches is treated either through phlebotomy or wet ***: In Persian medicine, Fasd and Hijamat are easy and economical methods for treatment of headache with reasonable results. By describing the precise areas and indications of phlebotomy and wet cupping, this study provides the first step for future clinical trials in this field.

Background： Gastroesophageal reflux disease （GERD） is one of the most common gastrointestinal com- plaints. GERD, caused by the reflux of stomach contents into the esophagus, leads to troublesome symptoms such as heartburn and regurgitation. It is classified into two types： erosive esophagitis, characterized by visible esophageal mucosa erosion in endoscopy, and non-erosive reflux disease （NERD）. GERD is a chronic and recurrent disease that impairs the quality of life and imposes socioeco- nomic and therapeutic burdens to both patients and society. Objective： Due to the failure of the conventional treatments for GERD and to the traditional use of Amla （Phyllanthus emblica L.）, in addition to beneficial effects shown in recent studies, we evaluated the safety and efficacy of Amla tablet for improvement of symptoms of patients with NERD. Design, setting, participants and interventions： We designed a double-arm, randomized, double-blind, placebo-controlled clinical trial. Sixty-eight patients who had classic symptoms of GERD （heartburn, regurgitation and epigastralgia） for at least three months before the start of the trial were randomized in two parallel groups. Patients in the Amla group received two 500 mg Amla tablets twice a day, after meals, for 4 weeks. In the control group, patients received placebo tablets similar to the Amla prescription. Main outcome measures： The patients were visited at baseline, and at the end of the 2nd and 4th weeks of intervention; their symptoms were measured on a frequency and severity scale for the symptoms of NERD, according to the quality of life in reflux-associated disease questionnaire. Results： Frequencies of heartburn and regurgitation in both groups of the study were significantly reduced after intervention （P 〈 0.001）. Do patients with gastroesophageal reflux disease and somatoform tendencies benefit from antireflux surgery?

BACKGROUND The clinical presentation of gastroesophageal reflux disease(GERD) shows a large symptom variation also in different intensities among patients. As several studies have shown, there is a large overlap in the symptomatic spectrum between proven GERD and other disorders such as dyspepsia, functional heartburn and/or somatoform *** To prospectively evaluate the GERD patients with and without somatoform disorders before and after laparoscopic antireflux *** In a tertiary referral center for foregut surgery over a period of 3 years patients with GERD, qualifying for the indication of laparoscopic antireflux surgery, were investigated prospectively regarding their symptomatic spectrum in order to identify GERD and associated somatoform disorders. Assessment of symptoms was performed by an instrument for the evaluation of somatoform disorders[Somatoform Symptom Index(SSI) > 17]. Quality of life was evaluated by Gastrointestinal Quality of Life Index(GIQLI).RESULTS In 123 patients an indication for laparoscopic antireflux surgery was established and in 43 patients further medical therapy was suggested. The portion of somatoform tendencies in the total patient population was 20.48%(34 patients).Patients with a positive SSI had a preoperative GIQLI of 77(32-111). Patients with a normal SSI had a GIQLI of 105(29-140)(P < 0.0001). In patients with GERD the quality of life could be normalized from preoperative reduced values of GIQLI102(47-140) to postoperative values of 117(44-144). In patients with GERD and somatoform disorders, the GIQLI was improved from preoperative GIQLI 75(47-111) to postoperative 95(44-122)(P < 0.0043).CONCLUSION Patients with GERD and associated somatoform disorders have significantly worse levels of quality of life. The latter patients can also benefit from laparoscopic fundoplication, however they will not reach a normal level.

Diabetic neuropathy(DN)is the most common form of neuropathy worldwide,with its prevalence rising alongside diabetes,and being characterized by sensory,motor or autonomic *** is considered to be an incurable complication of diabetes,the management of which mainly consists of improving glycemic control,managing pain relief and ensuring continuous foot *** gabapentin,duloxetine and tricyclic antidepressants are commonly used to reduce patient symptoms,they do not affect the pathophysiology and progression of ***,these drugs can have various side effects including insomnia,decreased appetite,arrhythmia,heart failure,and suicidal *** to traditional Persian medicine,DN is recognized as a type of“Khadar”or“Esterkha”(a sensory or motor disorder,respectively)that occurs due to the accumulation of sugars in the peripheral *** spinosa L.,commonly known as the caper plant,has been recommended in authentic sources of traditional Persian medicine to treat such *** this study,we reviewed the pharmacological properties of *** using the Web of Science,PubMed,Scopus and Google Scholar databases,and found that Capparis spinosa *** affect several pathways involved in DN pathogenesis,including aldose reductase activity,the secretion of inflammatory mediators(IL-17,TNF-α,IL-1β,IL-6),oxidative stress,hyperlipidemia,hyperglycemia and advanced glycation end product *** on these findings,we hypothesize that Capparis spinosa L.,may prevent the progression and reduce the symptoms of diabetic neuropathy,and so can be considered as a complementary treatment in this *** hypothesis should be evaluated in well-designed in vitro and in vivo studies,and through clinical trials.

Anti-inflammatory and anti-oxidant effects of aloe vera in rats with non-alcoholic steatohepatitis

BACKGROUND Aloe vera exerts several biological activities,such as,anti-inflammatory,antioxidant,and antimicrobial *** was recently shown to reduce insulin resistance and triglyceride *** hypothesized that aloe vera would have beneficial effects in alleviating non-alcoholic steatohepatitis(NASH)in *** To examine the therapeutic effects of aloe vera in NASH *** All rats were randomly divided into 3 groups(n=6 in each group).Rats in the control group were fed ad libitum with a standard diet for 8 *** in the NASH group were fed ad libitum with a high-fat high-fructose diet(HFHFD)for 8 *** in the aloe vera group were fed ad libitum with a HFHFD and aloe vera in dimethylsulfoxide(50 mg/kg)by gavage daily for 8 *** samples were collected at the end of the treatment *** Hepatic malondialdehyde(MDA)levels increased significantly in the NASH group as compared with the control group(377±77 nmol/mg vs 129±51 nmol/mg protein,respectively,P<0.001).Glutathione(GSH)levels were significantly lower in the NASH group than the control group(9±2 nmol/mg vs 24±8 nmol/mg protein,respectively,P=0.001).The expression of interleukin-18 (IL-18), nuclear factor-kappa β, and caspase-3 increased, while peroxisomeproliferator-activated receptor gamma decreased in the NASH group comparedwith the controls. Following aloe vera administration, MDA levels decreased (199± 35 nmol/mg protein) and GSH increased (18 ± 4 nmol/mg protein) ***, hepatocyte ballooning, lobular inflammation and increased hepatocyteapoptosis were observed in the NASH group. Aloe vera treatment attenuatedthese changes in liver *** vera attenuated oxidative stress, hepatic inflammation and hepatocyteapoptosis, thus improving liver pathology in rats with NASH.

Traditional Chinese medicine （TCM） is a unique health resource in China and one of the main representative traditional medicines globally. TCM has formed a new way of looking at medical practices, health care, scientific research, education, industry and culture. It focuses on promoting and safeguarding the health of people, with an increasing contribution to economic and social development. Establishing a comprehensive evaluation system in accordance with the characteristics of TCM services could promote the scientific merit and the standardization of services management. This would improve health service quality and the social and economic benefits of TCM. It would broaden the field of TCM services research. It would also provide the basis for the formulation of relevant government policies. This study estimates the prospect of establishing a comprehensive evaluation system of TCM services.
