Primary low-grade B-cell lymphomas of the skin are separated into marginal zone and follicle center lymphomas according to the recent World health Organization-European Organization for Research and Treatment of Cancer classification, with distinct histologic and immunohistochemical profiles. Some cases remain difficult to distinguish. The degree of relationship with their extracutaneouscounterparts is currently being investigated on clinical, histologic and molecular *** analysis using fluorescence in situ hybridization was performed on 12 frozen samples of infiltrated skin that had been classified as marginal zone lymphoma (MZL). Chromosomal changes known to be recurrently observed in systemic MZL of the mucosa-associated lymphoid tissue type, and in follicular center lymphoma were analyzed. These included trisomy for chromosomes 3, 7, 12, and 18 as well as t(14;18) IGH/BCL2, t(14;18) IGH/MLT1, and t(11;18) API2/MLT1 translocations. complementary molecular search of IGH/BCL2 rearrangement using a polymerase chain reaction technique and of API2/MLT1 mRNA expression by reverse transcriptase-polymerase chain reaction were performed. Two cases showed evidence of trisomy 3 at levels varying from 14%to 20%of the analyzed cells. No other chromosomal abnormalities were found with those techniques in the remaining cases. These results demonstrate that known recurrent chromosomal abnormalities rarely occur in primary cutaneous MZLs and suggest the possibility of a variety of initial oncogenic events leading to a common downstream pathway. These data also underline that fluorescence in situ analysis on routine skin punch biopsies represents a reliable tool for the detection of chromosomal changes, but requires consistent dermal infiltration.

Effects of Aromatherapy of Himekuromoji (<i>Lindera lancea</i>), Essential Oil on Vital and Stress, Emotion and Cognitive Function of the Elderly in Need of Care

Although there are limited means of delaying the cognitive decline in patients with dementia, aromatherapy contributes to alleviating the pain manifested through peripheral symptoms and psychological expression of dementia as part of dementia control. However, there are no studies that comprehensively verify the stress reduction effect, cognitive function, vital signs, etc. in the elderly requiring long-term care. In this study, we focused on Himekuromoji (Lindera lancea ), which grows naturally in Japan and has no verification cases and decided to verify the possibility of aromatherapy using that essential oil. As a result, olfactory stimulation by Lindera lancea aroma stabilizes the mood and emotions of the client and suppressed the rise in blood pressure. In sentiment analysis, four emotions were quantified using a wearable device. Although the effects of worker emotions on labor productivity have already been elucidated using the same device as in this study, emotional quantification by aromatherapy intervention is unprecedented. There were no significant differences in the four emotions depending on the intervention period and test section, but we were able to capture changes in emotions due to differences in recreational activities. In addition, it is considered that the client was able to spend about 30% of the activity time in a relaxed manner. Background: Patients with syncope or near syncope of unknown etiology represen t a great challenge to cardiologists. An initial symptomatic episode triggers a series of diagnostic analysis which remain unsatisfactory when negative. More in vasive tools such as electrophysiologic testing yield only partial answers to ri sk stratification while the complementary implantable holter diagnostics are not usually considered until a recurrent episode is documented. Objective: This stu dy targets predictors of significant cardiac rhythmic events in patients with a reported episode of syncope or near syncope presenting with negative diagnostics and electrophysiologic study results(EPS). A significant cardiac rhythmic event was defined as a combined end-point of(1) symptomatic AV block;(2) symptomatic conduction abnormalities requiring pacemaker therapy;(3) symptomatic sustained ventricular arrhythmia; and(4) sudden ***: All patients undergoing EPS after a first episode of syncope or presyncope between January 1997 and Decembe r 2001 were included for analysis. The study population consisted of 329 pts(42. 6%women), 21 to 96 years old(mean 70±15 years) referred for an EP study for sy ncope or near syncope. Results: Of the 329 patients who underwent EPS, 305(92.7 %) had follow-up data. The population, mean age 70(±15 years) and composed of 42%women, presented with hypertension(51.5%), diabetes mellitus(14.4%), hype rcholesterolemia(30%), tobacco use(35%), a familial history of coronary heart disease(22%), history of stroke(4%), history of MI(12%), history of atrial fi brillation(10%), structural heart disease(17.4%), left ventricular ejection fr action 61(±11%) and ECG abnormalities(37%). Observations from the Working Group Peritoneal Carcinosis

The aim of this report was to describe the feasibility, overall survival and quality of life of combining multimodal therapy with a complementary therapy concept called LOTUS Care Cure program. The peritoneal carcinomatosis (PC) working group described their observations on the combination of multimodal therapy with a complementary therapy concept based on 132 patients with different cancer entities with suspected PC. PC was not confirmed by laparoscopy in 32.5% of the patients included in the working group of patients with suspected PC. Patient compliance and the feasibility were high. For Ki67, there is a cut-off at 45% with a slower progression at 45%. The higher the Karnofsky index, the more improved the therapy and tolerability, with a cut-off of 80%. Overall, 72.0% of patients died. The median survival time in the overall population was 3.74 years (95% CI, 2.57 to 4.91) with a sharp decline in the first 16 weeks. The quality of life of patients can be improved with the implementation of the complementary LOTUS Care Cure Project. Overall, the therapy of PC requires a multi-professional team of therapists and a multimodal therapy concept. The multimodal concept together with the Lotus Care Cure project shows very good feasibility with high compliance and ultimately leads to better and low-risk patient care.

背景:在2004年,全球估计有1710万人死于心血管疾病。根据美国心脏病协会的数据,心脏病与中风是2型糖尿病患者的首要致死及致残因素。寻找更多的安全有效的心血管疾病治疗方案对于该疾病的预防和治疗十分必要,营养补充品是可行方案之一。目的:探讨菠萝蛋白酶(一种食品补充品)对心血管疾病的预防和治疗作用。检索策略:2009年9月至2010年3月进行电子与手工检索。检索的数据库包括Ovid MEDLINE;All EBMReviews-Cochrane Database of Systematic Reviews(Cochrane DSR)、American College of Physicians(ACP)Journal Club、Database of Abstracts of Reviews of Effects(DARE)、Cochrane Central Register ofControlled Trials(CCTR)、Cochrane Methodology Register(CMR)、health Technology Assessment(HTA)和National health service Economic Evaluation Database(NHSEED);Allied and complementaryMedicine(AMED)、British Nursing Index and Archive、EMBASE、health Management InformationConsortium(HMIC)、ScienceDirect和Electronic Thesis Online services(ETHOS)。检索语言限定为英文。纳入标准:有关菠萝蛋白酶用于预防或治疗心脑血管疾病的随机对照试验、人体研究、动物研究与实验性研究。资料提取与分析:3位作者分别对检索到的文章进行文献质量评估并提取文献资料。所收集的资料包括实验种类、用药剂量、治疗周期、测量指标、研究结果、菠萝蛋白酶的特征、结果的意义与研究结论。结果:在搜索到的223篇文章中,只有6篇文章符合纳入标准。包括3项动物实验和3项人体试验。菠萝蛋白酶可用来治疗急性血栓性静脉炎,因为它可减少血液中的纤维蛋白原含量,降低血小板聚集度,有保护心脏的作用,并能够改善动脉壁的结构,避免由凝血酶所诱发的血小板聚集,减少血栓的形成。结论:迄今为止,没有菠萝蛋白酶用于心血管疾病治疗的高质量的人体研究。在1948年至2010年间所发表的有关菠萝蛋白酶与心血管疾病的研究的数量极为有限。在未来的心血管疾病研究中这可能是一个值得探索的领域。

BREV的有效性:173例学习障碍儿童BREV与对照系列的比较

Introduction. -The BREV battery (Battery for rapid evaluation of cognitive fu nctions) is a tool which can be used for the rapid neuropsychological evaluation of children aged between 4 and 9 years. Objectives. -After standardization (70 0 unaffected children) and validation by comparison with a reference battery (20 2 children with epilepsy), the aim of this study was further validation in 173 children with learning disorders. Population and methods. -The study protocol in cluded administration of the BREV, precise neuropsychological examination and ev aluation of oral and written language. Statistical analysis was used to compare the findings of the BREV with those of the reference method, and the recommendat ions indicated by the BREV with the final diagnoses, and to define the sensitivi ty and the specificity of the BREV battery. Results. -All the correlations betw een BREV tests and reference tests were significant. Recommendations after the B REV were in agreement with the conclusions of the reference evaluation in 168/17 2 children for language, 145/173 for the psychometric evaluation. For only 4 chi dren, the results of the BREV were false negative. Diagnoses corresponded in 168 /173 children for oral language, in 102/110 for written language, 166/173 for pr axis disorders and 157/173 for intellectual deficit. The most predictive subtest s of the BREV and sensitivity and specificity of verbal and nonverbal scores wer e calculated. Conclusion. -The BREV is a reliable examination, in learning diso rders, to determine the most complementary investigations both in terms of langu age disorders and for non-verbal or global learning disabilities.
