global lessons on delivery of primary healthcare services for people with non-communicable diseases:convergent mixed methods

Objective To extract key lessons on primary healthcare(PHC)service delivery strategies for non-communicable diseases(NCD)from the work of researchers funded by the global Alliance for Chronic Diseases(GACD).Design A convergent mixed methods study that extracted data using a standardised template from research projects funded by the GACD that focused on *** strategies implemented in these studies were mapped onto the PHC Performance Initiative *** qualitative interviews were conducted with researchers from purposefully selected projects to understand the strategies and contextual factors in more *** PHC contexts from low or middle-income countries(LMIC)as well as vulnerable groups within high-income *** came from all regions of the world,particularly East Asia and Pacific,sub-Saharan Africa,South Asia,Latin America and *** The study extracted data on 84 research projects and interviewed researchers from 16 research *** Research projects came from all regions of the world,and mainly focused on diabetes(35.3%),hypertension(28.3%)and mental health(27.6%).Mapped onto the PHC Performance Initiative framework:49.4%focused on high-quality PHC(particularly the comprehensiveness of NCD care,41.2%);41.2%on the availability of PHC services(particularly the competence of healthcare workers,36.5%);35.3%on population health management(particularly community-based services,35.3%);34.1%on facility organisation and management(particularly team-based care,20.0%)and 31.8%on access(particularly digital technology,23.5%).Most common strategies were task shifting and training to improve the comprehensiveness of NCD care through community-based *** factors related to inputs:infrastructure,equipment and medication,workforce(particularly community health workers),finances,health information systems and digital *** Key strategies and contextual factors to improve PHC service del

1 Background Congenital heart disease(CHD)is the most common major congenital anomaly,affecting approximately one in every 100 live births[1].Among congenital anomalies,66%of preventable deaths are due to CHD,and 58%of the avertable morbidity and mortality due to congenital anomalies would result from scaling congenital heart surgery services[2].Every year,nearly 300,000 children and adults die from CHD,the majority of whom live in low-and middle-income countries(LMICs)[3].Approximately 49%of all individuals with CHD will require surgical or interventional care at some point in their lifetime[4];as a result of advances in access to and the delivery of such services,over 95%of children born with CHD in high-income countries now live into adulthood[3].Here,adults have surpassed children in the number of CHD cases at a ratio of 2:1[5].

Background Growing evidence attests to the efficacy of mindfulness-based interventions(MBIs),but their effectiveness for healthcare workers remains *** To evaluate the evidence for MBIs in improving healthcare workers’psychological *** A systematic literature search was conducted on Medline,Embase,Cumulative Index for Nursing and Allied Health Literature,PsycINFO and Cochrane Central Register of Controlled Trials up to 31 August 2022 using the keywords‘healthcare worker’,‘doctor’,‘nurse’,‘allied health’,‘mindfulness’,‘wellness’,‘workshop’and‘program’.Randomised controlled trials with a defined MBI focusing on healthcare workers and quantitative outcome measures related to subjective or psychological well-being were eligible for inclusion.R V.4.0.3 was used for data analysis,with the standardised mean difference as the primary outcome,employing DerSimonian and Laird’s random effects *** of Recommendations,Assessment,Development and Evaluation framework was used to evaluate the quality of ***’s Risk of Bias 2 tool was used to assess the risk of bias in the included *** A total of 27 studies with 2506 participants were included,mostly from the USA,involving various healthcare *** such as stress reduction programmes,apps,meditation and training showed small to large effects on anxiety,burnout,stress,depression,psychological distress and job strain outcomes of the *** effects were also seen in self-compassion,empathy,mindfulness and ***,long-term outcomes(1 month or longer postintervention)varied,and the effects were not consistently *** MBIs offer short-term benefits in reducing stress-related symptoms in healthcare *** review also highlights limitations such as intervention heterogeneity,reduced power in specific subgroup analyses and variable study quality.

In the era of rapid development of Internet of Things(IoT),numerous machine-to-machine technologies have been applied to the industrial *** to the divergence of IoT solutions,the industry is faced with a need to apply various technologies for automation and *** fact leads to a demand for an establishing interworking mechanism which would allow smooth interoperability between heterogeneous *** of the major protocols widely used today in industrial electronic devices is ***,data generated by Modbus devices cannot be understood by IoT applications using different protocols,so it should be applied in a couple with an IoT service layer ***2M,a global IoT standard,can play the role of interconnecting various protocols,as it provides flexible tools suitable for building an interworking framework for industrial ***,in this paper,we propose an interworking architecture between devices working on the Modbus protocol and an IoT platform implemented based on oneM2M *** the proposed architecture,we introduce the way to model Modbus data as oneM2M resources,rules to map them to each other,procedures required to establish interoperable communication,and optimization methods for this *** analyze our solution and provide an evaluation by implementing it based on a solar power management use *** results demonstrate that our model is feasible and can be applied to real case scenarios.

Reflections on Studying Huang Di Nei Jing in the West

Huang Di Nei Jing(《黄帝内经》The Yellow Emperor’s Inner Classic) has been the source text of Chinese medicine knowledge and innovation for over two thousand years. Despite this key relevance, many of its ideas and practices have proven difficult to understand and implement fully into clinical practice. Cultural and language differences can be compounded with these challenges but may also present new opportunities for advancement and insight when studied by researchers outside of the originating culture. This article introduces the method of Classical-Text Archaeology and delves into the author’s two-decade journey of researching this text, with a discussion on cultural differences and issues of medical scholarship.

Distributed user pairing and effective computation offloading in aerial edge networks

Future Sixth-Generation (6G) mobile communication networks extremely require the global connectivity and the ground Base Stations (BSs) are difficult to develop in some specific areas,such as mountainous ***,the aerial networks are the key framework for the future communications,in which the aerial vehicle could act as the access ***,Mobile Edge Computing (MEC) is also essential to enhance the data processing capabilities of aerial *** this paper,we study a comprehensive communication-computation resource management problem for jointly optimizing user pairing,power and time allocation in the MEC aided Cognitive Radio (CR) aerial networks,namely CR-MEC aerial ***,this multilevel optimization problem could be decomposed into the user pairing and power allocation as well as time allocation *** the conceived CR-MEC aerial networks,we propose a User Pairing and Computation Offloading Management Algorithm (UPCOMA) based on three-sided matching theory,aiming to minimize the system’s energy *** the first step of UPCOMA,multiple Primary Users (PUs) and Cognitive Users (CUs) on the ground negotiate to each other with the suitable power allocation coefficients and construct the stable user ***,the stable user pairs would match to a high altitude platform who act as the base station,which is for appropriately allocating Transmission Time Slots (TSs) at the second step of ***,a hybrid offloading mode is proposed in our conceived networks in order to support ground users to offload their tasks adaptively according to their individual ***,the simulation results reveal that the relative performance of UPCOMA is close to that of the Brute-Force Search Algorithm (BFSA) with low ***,the hybrid offloading mode with less energy consumption supports much more ground user pairs to offload tasks compared to the binary mode.

Enabling Space-Air integration: A Satellite-UAV networking authentication scheme

One of the goals of sixth-generation mobile networks(6G) is to achieve a larger network coverage area. Satellite networks enable global coverage and aerial nodes such as Unmanned Aerial Vehicle(UAV) can serve as a supplement to ground networks in remote environments. Therefore, 6G networks are gradually evolving towards Space-AirGround integrated networks. The combination of UAV networks and satellite networks is a research hotspot in the field of Space-Air integrated networks. However, the combination of UAV networks and satellite networks currently faces many challenges in terms of security. The characteristics of large propagation delay and unstable communication links in satellite networks make them vulnerable to various attacks, including eavesdropping, tampering, and impersonation. Meanwhile, existing research on UAV networks mainly focuses on UAV-Ground networking authentication mechanisms, which are not suitable for resourceconstrained nodes in the Space-Air integration scenario. Therefore, based on elliptic curve public key cryptography and Chebyshev polynomial, we propose a secure networking authentication scheme for satellite nodes and UAV nodes in the Space-Air integration *** security analysis indicates that our scheme possesses security attributes such as mutual authentication, key agreement, identity anonymity, unlinkability, perfect forward-backward security, and resistance against various protocol attacks, among other security *** analysis also indicates certain advantages of our scheme over existing schemes in terms of signaling, bandwidth, and computational overhead.
