Total pancreatectomy with islet cell transplantation vs intrathecal narcotic pump infusion for pain control in chronic pancreatitis

AIM: To evaluate pain control in chronic pancreatitis patients who underwent total pancreatectomy with islet cell transplantation or intrathecal narcotic pump ***: We recognized 13 patients who underwent intrathecal narcotic pump(ITNP) infusion and 57 patients who underwent total pancreatectomy with autologous islet cell transplantation(TP + ICT) for chronic pancreatitis(CP) pain control between 1998 and 2008 at Indiana University Hospital. All patients had already failed multiple other modalities for pain control and the decision to proceed with either intervention was made at the discretion of the patients and their treating physicians. All patients were evaluated retrospectively using a questionnaire inquiring about their pain control(using a 0-10 pain scale), daily narcotic dose usage, and hospital admission days for pain control before each intervention and during their last follow-up. RESULTS: All 13 ITNP patients and 30 available TP + ICT patients were evaluated. The mean age was approximately 40 years in both groups. The median duration of pain before intervention was 6 years and 7 years in the ITNP and TP + ICT groups, respectively. The median pain score dropped from 8 to 2.5(on a scale of 0-10) in both groups on their last follow up. The median daily dose of narcotics also decreased from 393 mg equivalent of morphine sulfate to 8 mg in the ITNP group and from 300 mg to 40 mg in the TP + ICT group. No patient had diabetes mellitus(DM) before either procedure whereas 85% of those who underwent pancreatectomy were insulin dependent on their last evaluation despite ICT. CONCLUSION: ITNP and TP + ICT are comparable for pain control in patients with CP however with high incidence of DM among those who underwent TP + ICT. Prospective comparative studies and longer follow up are needed to better define treatment outcomes.

HiAtGang: How to Mine the Gangs Hidden Behind DDoS Attacks

Identifying and determining behaviors of attack gangs is not only an advanced stage of the network security event tracing and analysis, but also a core step of large-scale combat and punishment of cyber attacks. Most of the work in the field of distributed denial of service(DDoS) attack analysis has focused on DDoS attack detection, and a part of the work involves the research of DDoS attack sourcing. We find that very little work has been done on the mining and analysis of DDoS attack gangs. DDoS attack gangs naturally have the attributes of human community relations. We propose a framework named HiAtGang, in which we define the concept of the gang detection in DDoS attacks and introduce the community analysis technology into DDoS attack gang analysis. Different attacker clustering algorithms are compared and analyzed. Based on analysis results of massive DDoS attack events that recorded by CNCERT/CC(The National Computer network Emergency Response Technical Team/Coordination Center of China), the effective gang mining and attribute calibration have been achieved. More than 250 DDoS attack gangs have been successfully tracked. Our research fills the gaps in the field of the DDoS attack gang detection and has supported CNCERT/CC in publishing “Analysis Report on DDoS Attack Resources” for three consecutive years and achieved a good practical effect on combating DDo S attack crimes.

Varicella Outbreak at a Boarding School, Chikomba District, Mashonaland East Province, Zimbabwe, 2019

Background: Varicella is a contagious illness, caused by varicella zoster virus (VZV). It is transmitted via the respiratory route and through contact with the infected person. Fourteen cases of varicella from a boarding school in Chikomba District, Zimbabwe were reported on 14 February 2019. We investigated the outbreak to determine its scope, identify risk factors for transmission and recommend evidence-based control measures. Methods: A 1:2 unmatched case-control study was conducted. A case was a form one student at a boarding school in Chikomba District who developed acute onset of diffuse maculopapulovesicular rash without other apparent cause from 22 January 2019 to 24 February 2019. A control was a form one student at the same boarding school who did not develop the signs and symptoms of varicella during the same period. The diagnosis was based on clinical signs and symptoms. An interviewer-administered questionnaire was used to collect data. Epi info 7TM was used to calculate frequencies, odds ratios and perform logistic regression to control for confounding variables. Results: We recruited 31 cases and 62 controls. Independent risk factors for contracting varicella were classmate contact [AOR 24;(95% CI 4.4 - 83), p Conclusion: A perfect setting for the spread of infection was available in the form of students in a naive community living in close proximity both at class and at dormitories, which lead to the rapid transmission of the virus. Screening and isolation of the infected students controlled the outbreak.

One-hundred years after the 1918-19 H1N1 flu pandemic and 10 years after the 2009 H1N1 flu pandemic, another respiratory virus has now inserted itself into the human population. Severe acute respiratory syndrome coronavirus has become a critical challenge to global health with immense economic and social disruption. In this article we review salient aspects of the coronavirus disease2019(COVID-19) outbreak that are relevant to surgical practice. The emphasis is on considerations during the pre-operative and post-operative periods as well as the utility and limitations of COVID-19 testing. The focus of the media during this pandemic is centered on predictive epidemiologic curves and models. While epidemiologists and infectious disease physicians are at the forefront in the fight against COVID-19, this pandemic is also a "stress test" to evaluate the capacity and resilience of our surgical community in dealing with the challenges imposed to our health system and society. As recently pointed out by Dr. Anthony Fauci,the virus decides the timelines in the models. However, the models can also change based on our decisions and behavior. It is our role as surgeons, to make every effort to bend the curves against the virus’ will.

Exploring the Relationship between Obstetrical Nurses’ Work and Pregnancy Outcomes

The Registered Nurses Association of Ontario healthy Work Environments Best Practice Guideline recommends that employers promote safe, healthy workplaces. healthy workplaces include addressing the unique needs of nurses who work while pregnant. The purpose of this descriptive study, summarizing information from 120 pregnancies reported by 95 nurses, was to determine if the workload of obstetrical nurses was associated with negative pregnancy outcomes, including preterm delivery and birth weight. Full-time obstetrical nursing work is a predictor of reduced birth weight, but not of preterm birth when compared to outcomes of obstetrical nurses working part time. One third of nurses reported pregnancy complications and most nurses experienced work-related and personal stress. Further research evaluating work modifications during pregnancy is indicated to improve birth outcomes.

Neuroimaging has emerged over the last few decades as a crucial tool in diagnosing Alzheimer’s disease(AD).Mild cognitive impairment(MCI)is a condition that falls between the spectrum of normal cognitive function and ***,previous studies have mainly used handcrafted features to classify MCI,AD,and normal control(NC)*** paper focuses on using gray matter(GM)scans obtained through magnetic resonance imaging(MRI)for the diagnosis of individuals with MCI,AD,and *** improve classification performance,we developed two transfer learning strategies with data augmentation(i.e.,shear range,rotation,zoom range,channel shift).The first approach is a deep Siamese network(DSN),and the second approach involves using a cross-domain strategy with customized *** performed experiments on the Alzheimer’s Disease Neuroimaging Initiative(ADNI)dataset to evaluate the performance of our proposed *** experimental results demonstrate superior performance in classifying the three binary classification tasks:NC ***,NC ***,and MCI ***,we achieved a classification accuracy of 97.68%,94.25%,and 92.18%for the three cases,*** study proposes two transfer learning strategies with data augmentation to accurately diagnose MCI,AD,and normal control individuals using GM *** findings provide promising results for future research and clinical applications in the early detection and diagnosis of AD.

A User Participation Behavior Prediction Model of Social Hotspots Based on Influence and Markov Random Field

Hotspot topic trends can be captured by analyzing user attributes and historical behavior in social network. In this paper, we propose a user participation behavior prediction model for social hotspots, based on user behavior and relationship data, to predict user participation behavior and topic development trends. Firstly, for the complex factors of user behavior, three dynamic influence factor functions are defined, including individual, peer and community influence. These functions take timeliness into account using a time discretization method. Secondly, to determine laws of individual behavior and group behavior within a social topic, a hotspot user participation behavior prediction model is proposed and associated with the basic concepts of randora field and Markov property in information diffusion. The experimental results show that the model can not only dynamically predict the individual behavior, but also grasp the development trends of topics.

Multiple studies have reported decreased emergency department(ED)patient volumes during the coronavirus disease(COVID-19)pandemic,[1-6]including areas most affected by the virus.[7]Most existing studies have investigated general trends in ED presentations and have not examined the impact of COVID-19 on different types of EDs,specific ED patient groups,or illness presentations.
