New serological markers in pediatric patients with inflammatory bowel disease

The spectrum of serological markers associated with inflammatory bowel disease (IBD) is rapidly growing. Due to frequently delayed or missed diagnoses, the application of non-invasive diagnostic tests for IBD, as well as differentiation between ulcerative colitis (UC) and Crohn’s disease (CD), would be useful in the pediatric population. In addition, the combination of pancreatic autoantibodies and antibodies against Saccharomyces cerevisiae antibodies/perinuclear cytoplasmic antibody (pANCA) improved the sensitivity of serological markers in pediatric patients with CD and UC. Some studies suggested that age-associated differences in the patterns of antibodies may be present, particularly in the youngest children. In CD, most patients develop stricturing or perforating complications, and a significant number of patients undergo surgery during the disease course. Based on recent knowledge, serum antibodies are qualitatively and quantitatively associated with complicated CD behavior and CD-related surgery. Pediatric UC is characterized by extensive colitis and a high rate of colectomy. In patients with UC, high levels of anti-CBir1 and pANCA are associated with the development of pouchitis after ileal pouch-anal anastomosis. Thus, serologic markers for IBD can be applied to stratify IBD patients into more homogeneous subgroups with respect to disease progression. In conclusion, identification of patients at an increased risk of rapid disease progression is of great interest, as the application of early and more aggressive pharmaceutical intervention could have the potential to alter the natural history of IBD, and reduce complications and hospitalizations.

Objective:Rheumatoid nephropathy is one of the most severe extra-articular manifestations of rheumatoid arthritis(RA)associated with a very unfavorable *** study aimed to identify changes in renal function and morphological variations of kidney diseases in RA ***:The study enrolled patients(126 patients)between 18 and 55 years of age with a confirmed active RA of more than 12 *** patient underwent the following range of laboratory and instrumental research methods:general clinical analysis of blood and urine,performing urinalysis according to Nechiporenko method;determining daily proteinuria;determining the blood content of glucose,urea,creatinine,uric acid,total bilirubin,liver transaminase level,ionogram,lipidogram,and coagulogram;determining the blood content of rheumatoid factor,anti-streptolysin O,and C-reactive protein;and X-ray of the joints of hands and *** function was examined by estimating glomerular filtration rate,tubular reabsorption index,and renal functional *** studying the morphological changes in the kidneys under ultrasound examination,renal biopsy was performed in 31 patients with RA with urinary syndrome(proteinuria more than 0.3 g per day and hematuria).Results:Nephropathy in RA is characterized by impaired renal function and manifested by an increased blood creatinine and a decrease in glomerular filtration rate and renal functional *** morphological variations of nephropathy at RA,mesangial proliferative glomerulonephritis prevails,accounting for 48.4%of *** disorders include the secondary amyloidosis(29.0%of patients),tubulointerstitial nephritis(16.1%),membranous glomerulonephritis(3.2%),and focal-segmental glomerulosclerosis(3.2%).Conclusion:Kidney damage is a common systemic manifestation of RA with a long and active course,a major nephropathy trigger.

Risk factors for fever and sepsis after percutaneous nephrolithotomy

Sleep disorders in Alzheimer’s disease:the predictive roles and potential mechanisms

Sleep disorders are common in patients with Alzheimer’s disease,and can even occur in patients with amnestic mild cognitive impairment,which appears before Alzheimer’s *** disorders further impair cognitive function and accelerate the accumulation of amyloid-βand tau in patients with Alzheimer’s *** present,sleep disorders are considered as a risk factor for,and may be a predictor of,Alzheimer’s disease *** that sleep disorders are encountered in other types of dementia and psychiatric conditions,sleep-related biomarkers to predict Alzheimer’s disease need to have high specificity and ***,we summarize the major Alzheimer’s disease-specific sleep changes,including abnormal non-rapid eye movement sleep,sleep fragmentation,and sleep-disordered breathing,and describe their ability to predict the onset of Alzheimer’s disease at its earliest *** the mechanisms underlying these sleep changes is also crucial if we are to clarify the role of sleep in Alzheimer’s *** paper therefore explores some potential mechanisms that may contribute to sleep disorders,including dysregulation of the orexinergic,glutamatergic,andγ-aminobutyric acid systems and the circadian rhythm,together with amyloid-β*** review could provide a theoretical basis for the development of drugs to treat Alzheimer’s disease based on sleep disorders in future work.

Incidental adenocarcinoma in patients undergoing surgery for stricturing Crohn's disease

AIM To evaluate frequency and clinical course of incidental adenocarcinoma in patients with stricturing Crohn's disease(CD).METHODS In this study, consecutive patients, who were operated on for stricturing CD between 1997-2012, were included at an academic tertiary referral center. Demographic data and clinical course were obtained by an institutional database and individual chart review. Besides baseline characteristics, intraoperative findings and CD related history were also recorded. Colorectal cancer was classified and staged according to the Union for International Cancer Control(UICC).RESULTS During the study period 484 patients underwent resections due to stricturing CD. Incidental adenocarcinoma was histologically confirmed in 6(1.2%) patients(4 males, 2 females). Patients diagnosed with colorectal cancer had a median age of 43(27-66)years and a median history of CD of 16(7-36) years. Malignant lesions were found in the rectum(n = 4, 66.7%), descending colon(n = 1, 16.7%) and ileocolon(n = 1, 16.7%). According to the UICC classification two patients were stages as?Ⅰ(33.3%), whereas the other patients were classified as stage ⅡA(16.7%), stage ⅢB(16.7%), stage ⅢC(16.7%) and stage Ⅳ(16.7%), respectively. After a median follow-up of 2(0.03-8) years only 1 patient is still *** The frequency of incidental colorectal cancer in patients, who undergo surgery for stenotic CD, is low but associated with poor prognosis. However, surgeons need to be aware about the possibility of malignancy in stricturing CD, especially if localized in the rectum.

A study of female genital swabs in a Nigerian Tertiary Hospital

Objective:To detect some common microbial agents of vaginal discharge and improve the current syndromic management of abnormal vaginal ***:A prospective study of female genital swabs collected from obstetrics and gynecology units of Aminu Kano teaching Hospital,Kano,Nigeria and an analysed of microscopy,culture and sensitivity in the medical microbiology and parasitology laboratory of this hospital were conducted from December 2007 to December *** on epidemiologic indices were collected from the patients,using structured interviewer-administered ***:Eight hundred and forty Candida species were detected,constituting 60%(n=840) of 1 400 female genital discharge samples of microbial etiology in a total of 2 000 female genital samples *** distribution of vaginal candidiasis was tbe highest in young adults aged 21 to 30 years with 43%(n=360) of the total 840 cases. Pregnant women that presented with vaginal candidiasis constituted 40%(n=360) of the total 840 *** risk groups included the immuno-suppression with 24%(n=202),group on hormonal therapy with 15%(n=126) and broad spectrum antibiotics users with 16%(n=134).Conclusions: The results show that Candida is the most common cause of vaginitis and vulvo-vaginal candidiasis followed by *** in the young adults aged 21 to 30 years,pregnant mothers, immuno-suppression,contraceptive and broad spectrum antibiotic *** management of vaginal candidiasis and vulvo-vaginal candidiasis is recommended especially among the risk groups in order to avoid complications and reduce HIV transmission.

Background: Infertility is a complex disorder with significant psycho-social and economic consequences. It globally affects 10% - 15% of couples. In Cameroon, little is known about what women do to overcome the psychosocial aspects of the disease. Objectives: This study aimed to identify the support systems and coping strategies of infertile women attending the outpatient consultation unit of the Gynaecological Endoscopic Surgery and Reproductive teaching Hospital (CHRACERH), Yaoundé, Cameroon. Methods: A hospital-based cross-sectional study was conducted from the 14th of March to the 6th of April 2023 at CHRACERH Yaoundé. A total of 190 participants were recruited using a convenience sampling method. Data regarding socio-demographic characteristics, support systems and coping strategies were collected using a pretested questionnaire. Descriptive and analytic statistics were conducted using SPSS version 25. Results: The mean age of participants was 39.52 ± 7.64 years. The majority 78.9% of participants were workers (public, private sector, or traders) and were Christians 95.8%. The most common source of psychological support was from family 76.8 and husbands 72.63%. Most of the participants 89.5% resorted to prayer and getting busy 48.4% as a coping strategy. There was no statistically significant relationship between coping strategies and psychological disorders p > 0.05. Conclusion: The main support system of participants was family, husband, and friends. Prayer, getting busy and adoption were the most common coping strategies. There is a need for the Ministry of Public Health and other stakeholders to put in place other support systems and coping strategies (FELICIA) used elsewhere and provide adequate health education and infection control to prevent infertility in Cameroon.

Human Interaction Recognition (HIR) was one of the challenging issues in computer vision research due to the involvement of multiple individuals and their mutual interactions within video frames generated from their movements. HIR requires more sophisticated analysis than Human Action Recognition (HAR) since HAR focuses solely on individual activities like walking or running, while HIR involves the interactions between people. This research aims to develop a robust system for recognizing five common human interactions, such as hugging, kicking, pushing, pointing, and no interaction, fromvideo sequences usingmultiple cameras. In this study, a hybrid Deep Learning (DL) and Machine Learning (ML) model was employed to improve classification accuracy and generalizability. The dataset was collected in an indoor environment with four-channel cameras capturing the five types of interactions among 13 participants. The data was processed using a DL model with a fine-tuned ResNet (Residual Networks) architecture based on 2D Convolutional Neural Network (CNN) layers for feature extraction. Subsequently,machine learning models were trained and utilized for interaction classification using six commonly used ML algorithms, including SVM, KNN, RF, DT, NB, and XGBoost. The results demonstrate a high accuracy of 95.45% in classifying human interactions. The hybrid approach enabled effective learning, resulting in highly accurate performance across different interaction types. Futureworkwill exploremore complex scenarios involving multiple individuals based on the application of this architecture. © 2024 The Authors.

Pulmonary arteriovenous malformations （AVM） lung is defined by an abnormal communication between pulmonary artery（ies） and vein（s） responsible for a right-left shunt. Congenital forms are most common and usually associated with Rendu-Osler disease （ROD）. Inversely, 15-45% of patients with ROD present lung AVM Nowadays, embolization is preferred to surgical resection in the majority of cases. Except for certain cases where surgery is indicated. This paper shows the role of surgery in AVMs.
