Background: Control of oral anticoagulant treatment has been reported to be suboptimal, but previous studies suggest that patient self-management improves control. Objective: To compare the quality of control and the clinical outcomes of oral anticoagulant treatment in self-managed patients versus patients following conventional management. design: Randomized, controlled trial. Setting: University-affiliated hospital in Spain. Patients: 737 patients with indications for anticoagulant treatment. Intervention: The self-management group(n=368)received simple instructions for using a portable coagulometer weekly and self-adjusting treatment dose. The conventional management group(n=369)received usual care in an anticoagulation clinic(monthly measurement and control of international normalized ratio [INR], managed by hematologists). Measurements: Percentage of INR values within the target range and major related complications. Results: The median follow-up period was 11.8 months(range, 0.3 to 16.9 months). The unadjusted percentages of in-range INRs were 58.6%in the self-management group and 55.6%in the conventional management group(difference, 3.0 percentage points [95%CI, 0.4 to 5.4 percentage points]). Twenty-seven patients(7.3%)in the conventional management group and 8(2.2%)in the self-management group had major complications related to anticoagulant treatment. The unadjusted risk difference for major complications between groups was 5.1 percentage points(exact 95%CI, 1.7 to 8.5 percentage points). Fewer patients had minor hemorrhages in the self-management group(14.9%)than in the conventional management group(36.4%). Fifteen patients(4.1%)in the conventional management group and 6(1.6%)in the self-management group died(unadjusted risk difference, 2.5 percentage points [exact 95%CI, 0.0 to 5.1 percentage points]). Limitations: The trial was performed at only 1 center and was not blinded. The dropout rate in the intervention group was 21%. Conclusions: Compared with

目的系统梳理国际卫生系统绩效评价框架并构建关键路径模型，研究框架构建过程中的理论、方法及规律。方法通过描述性系统综述法系统检索相关文献，结合理想类型法构建关键路径模型，并在此基础上对框架进行研究。结果国际卫生系统绩效评价框架构建包括评价框架构建目的、卫生系统边界、卫生系统目标、卫生系统结构以及卫生系统框架维度确定5个关键步骤。结论目的决定评价框架的功能类型，同时与边界和目标共同决定评价框架的结构模型。广义卫生系统结构模型和狭义卫生保健系统结构模型作为目前主流模型，广泛应用于卫生系统绩效评价。然而，卫生系统目标和结构之间联系的建立，作为评价框架构建的关键步骤，仍然面临挑战。

Mixed-valance manganites with strong electron correlation exhibit strong potential for spintronics,where emergent magnetic behaviors,such as propagation of high-frequency spin waves and giant topological Hall Effects can be driven by their mesoscale spin ***,we create magnetic vortex clusters with flux closure spin configurations in single-crystal la0.67Sr0.33MnO3 wire.A distinctive transformation from out-of-plane domains to a vortex state is directly visualized using magnetic force microscopy at 4 K in wires when the width is below 1.0μ*** phase-field modeling indicates that the inhomogeneous strain,accompanying with shape anisotropy,plays a key role for stabilizing the flux-closure spin *** work offers a new perspective for understanding and manipulating the non-trivial spin textures in strongly correlated systems.

This review summarizes techniques used by operational centers to forecast tropical cyclone intensity change as presented to the international Workshop on Tropical Cyclones(IWTC-9)in Hawaii in *** advances and major changes over the past four years are presented,with a special focus on forecasting rapid intensity *** intensity change remains one of the most difficult aspects of tropical cyclone forecasting,objective guidance has shown some *** greatest improvements are realized when consensus methods are utilized,especially those that blend statistical-dynamical based guidance with dynamical ocean-coupled regional *** models become even more skillful when initialized with inner core observational *** improvement and availability of intensity guidance along with associated forecaster training are expected to deliver forecasting improvements in the future.

We present the results of the first commissioning phase of the short-focal-length area of the Apollon laser facility(located in Saclay,France),which was performed with the first available laser beam(F2),scaled to a nominal power of 1 *** the conditions that were tested,this beam delivered on-target pulses of 10 J average energy and 24 fs *** diagnostics were fielded to assess the performance of the *** on-target focal spot and its spatial stability,the temporal intensity profile prior to the main pulse,and the resulting density gradient formed at the irradiated side of solid targets have been thoroughly characterized,with the goal of helping users design future *** of energetic electrons,ions,and electromagnetic radiation were recorded,showing good laser-to-target coupling efficiency and an overall performance comparable to that of similar international *** will be followed in 2022 by a further commissioning stage at the multipetawatt level.

目的：对比在矫正低度近视,近视散光和高度近视时应用波前像差引导的激光上皮瓣下角膜磨镶术（laSEK）的安全性,有效性,可预测性,稳定性和并发症。方法：该回顾性分析共纳入416眼,分为3组,低度近视组159眼,等值球镜-3.68±1.33D;近视散光组161眼,等值球镜-5.99±2.24D,柱镜度2.41±1.07D;高度近视组96眼,等值球镜-7.41±0.80D.制瓣后,进行波前像差为基础的准分子激光削切术。在术后10d,2,6和12mo后评估其安全性,有效性,可预测性和稳定性。结果：术后12mo,低度近视组等值球镜-0.36±0.31D,近视散光组0.15±0.41D,高度近视组0.58±0.68D。低度近视组中,裸眼视力为20/20的患者占90.60%,近视散光组78.90%,高度近视组67%。疗效指标在三组中分别为0.98,1.04和0.92.安全性指标分别为1.00,1.07和1.05。低度近视组有5眼（3.1%）最佳矫正视力提升1行,近视散光组有44眼（27.3%）提升1~3行,高度近视组18眼（19.2%）提升1~2行。低度近视组只有2例产生角膜雾状混浊。在疗效和安全性方面三组比较均无统计学差异。结论：波前像差引导的激光上皮瓣下角膜磨镶术是治疗低度近视,近视散光和高度近视的一种有效安全的方法,而在治疗近视散光时其可预测性、有效性和安全性更佳。
