目的：报告在西班牙Castilla-Leo地区患有渗出性老年性黄斑变性（AMD）的样本人群的流行病学特点，并与一组来自同一地区和相同年龄人群进行比较。方法：在本次观察性前瞻性调查中，我们访问了在一家地区性诊所接受光动力学疗法的AMD患者。患者报告了高血压、高血脂、吸烟的病史，检查患者虹膜颜色并分为浅色（绿色、蓝色、灰色）和深色（淡褐色、褐色、黑色）。结果：共有343名患者接受了访问，平均年龄为74．9岁（55～93岁），其中64．5％为女性，35．5％为男性，有45．1％的患者虹膜颜色被划为浅色。这些患者中，有50％有动脉性高血压，15．3％接受高胆固醇血症治疗，30．2％吸烟或者已经戒烟（女性中占80．9％）。结论：在渗出性AMD患者中，虹膜颜色为浅色的频率较高。在我们的调查中，其他渗出性AMD其他的风险因素有男性的吸烟习惯、未接受高胆固醇血症治疗、患者为女性身份等。

目的:报告2例以视力下降为首发症状的白血病患者,采用光学相干断层扫描(OCT)识别和定位视网膜病变。方法:回顾性干涉个案报告。结果:伴随视力下降,确诊急性淋巴细胞白血病和慢性淋巴细胞白血病患者各1例。OCT发现1例为黄斑部浆液性脱离。另1例为视网膜低荧光浸润,OCT表现为高反射性损伤。在随访时间内,肿瘤完全缓解后视网膜改变消失,视力明显恢复。结论:OCT是一种检测、定位和跟踪的眼部肿瘤有价值的,非侵入性的诊断工具。

脑血流临时阻断中脑电图连续监测的临床意义及局限性

目的探讨在脑前交通动脉瘤患者手术中,临时阻断脑血流,脑电图监测结果的临床意义及局限性。方法直接夹闭动脉瘤16例,血管搭桥术后孤立动脉瘤30例。脑电图监测采用皮肤针状电极国际标准10-20系统;手术后12 h内行CT扫描检查。结果 6例术前血管造影显示侧支循环不良的患者中,2例在试验性颈内动脉阻断时,出现脑电图的异常改变;其他4例大脑中动脉临时阻断时,无脑电图改变;1例术前造影显示侧支循环良好的患者,颈内动脉试验性阻断时出现脑电图异常改变,改行大脑中动脉阻断,未出现脑电图异常。不论术前血管造影如何,大脑中动脉阻断过程中均未出现脑电图异常改变。手术后CT检查发现基底核区梗死3例,手术造成的皮质损伤3例,基底核区出血1例。在这7例患者中,术中均无脑电图的异常改变。颈内动脉临时阻断时间为5-25min,大脑中动脉为25-50min。结论在脑血流临时阻断过程中,进行脑电图监测,可以指导脑保护性药物(如巴比妥等)的用量;可提示可能出现的重度缺血性脑损害;但受开颅范围影响,脑电图对手术暴露区域的脑组织,尤其皮质下及脑深部组织不敏感。

Background: Both competitive and recreational sports are considered a positive practice that is good for human health and quality of life, so it is difficult to understand how young, energetic and apparently healthy people can die while playing their usual sports. When these events occur in elite athletes during monitored events, they normally receive considerable media attention. Objective: To describe the case of sudden death in a young professional athlete who was successfully aided and resuscitated outside our hospital, and to review the literature about sudden death in sports. Materials and Method: We searched for Sudden death (according to the international Classification of Diseases (ICD-9 and ICD-10)) in our hospital but found no other relevant cases. We performed a literature search on the state of the art in sudden death (Google Scholar, PubMed, LIlaCS, Scielo and the Revista Española de Cardiología). Results: Although we observed that the prevalence of sudden death is not very high, it still represents a potentially avoidable event. An adequate medical examination prior to regular sporting practice can decrease the incidence of sudden death in athletes under the age of 35. Data available in the literature support medical check-ups that include history-taking, physical examination and electrocardiogram. The findings of this evaluation may also prompt further assessment (electrocardiograph, ergometry). Conclusions: We could not identify standard, obligatory guidelines for screening possible heart diseases causing cardiorespiratory failure and sudden death. As a measure for primary prevention, we propose a protocol for reducing the risk of sudden death in athletes.

We study the global well-posedness of large-data solutions to the Cauchy problem of the energy critical Cahn-Hilliard-Brinkman equations in R^(4).By developing delicate energy estimates,we show that for any given initial datum in H^(5)(R^(4)),there exists a unique global-in-time classical solution to the Cauchy *** a special consequence of the result,the global well-posedness of large-data solutions to the energy critical Cahn-Hilliard equation in R^(4) follows,which has not been established since the model was first developed over 60 years *** proof is constructed based on extensive applications of Gagliardo-Nirenberg type interpolation inequalities,which provides a unified approach for establishing the global well-posedness of large-data solutions to the energy critical Cahn-Hilliard and Cahn-Hilliard-Brinkman equations for spatial dimension up to four.
