国际糖尿病足工作组(IWGDF)1999年发布了关于预防和管理糖尿病足病的循证指南。该指南旨在预防糖尿病患者的足部溃疡,并在2015年,IWGDF对预防指南进行了更新。我们遵循GRAde(grading of recommendations assessment development and evaluation)的评分方法,采用以患者为观察对象、有干预措施、与对照组进行比较、观察结局(patient-intervention-comparison-outcome,PICO)的方法来设计临床问题和关键成果,对医学科学文献进行系统评价,并撰写推荐意见及其理由。这些推荐基于系统评价中发现的证据质量、专家意见,以及益处及危害的权重、患者偏好、可行性和适用性以及与干预相关成本的权衡。我们建议对处于极低溃疡风险的患者每年做一次筛查,了解其是否有周围神经病变和外周血管病变,对高风险人群检查的频率要更高,以筛查其他危险因素。为了预防足溃疡,要对有危险因素的患者进行健康教育,教育患者适当地对足部进行自我护理以及对溃疡前症状进行有效治疗。指导中高危患者穿着合适的治疗鞋,并对其足部皮肤温度进行监测。处方治疗鞋在行走过程中具有明显的减轻足底压力的效果,能够防止足底溃疡复发。急性足溃疡或即将发生的足溃疡保守治疗失败,应考虑手术干预。我们不建议使用神经松解减压术。对高危患者给予综合足部医疗护理,以防溃疡复发。贯彻这些推荐将有助于医疗专业人员对有足溃疡风险的糖尿病患者提供更好的护理,降低糖尿病足溃疡的发生率,使足部健康维持更长的时间即增加无足病的天数,显著减少糖尿病患者和医疗机构的负担。

Optical parametric amplification(OPA)represents a powerful solution to achieve broadband amplification in wavelength ranges beyond the scope of conventional gain media,for generating high-power optical pulses,optical microcombs,entangled photon pairs and a wide range of other ***,we demonstrate optical parametric amplifiers based on silicon nitride(Si3N4)waveguides integrated with two-dimensional(2D)layered graphene oxide(GO)*** achieve precise control over the thickness,length,and position of the GO films using a transfer-free,layer-by-layer coating method combined with accurate window opening in the chip cladding using *** OPA measurements with a pulsed pump for the fabricated devices with different GO film thicknesses and lengths show a maximum parametric gain of~24.0 dB,representing a~12.2 dB improvement relative to the device without *** perform a theoretical analysis of the device performance,achieving good agreement with experiment and showing that there is substantial room for further *** work represents the first demonstration of integrating 2D materials on chips to enhance the OPA performance,providing a new way of achieving high performance photonic integrated OPA by incorporating 2D materials.

Background: The Center of Molecular Immunology (CIM) is a center in Cuba devoted to the research, development and manufacturing of biotechnological products. CIMAvax?EGF is a vaccine for the treatment of non-small cell lung cancer patients (NSCL). Purpose: The aim of this work is to evaluate the effects of some potential prognostic factors on the overall survival of patients treated with CIMAvax?EGF vaccine, based on data collected in a phase II and a phase III clinical trials. Methods: The stratified Cox regression model is used to evaluate the effects of these prognostic factors, based on separate analysis for each trial, and on the combined data from both trials. Results: Patients with Performance status 0 or 1, with IV stage of tumor and male under 60 years obtain more benefit in terms of overall survival if they receive CIMAvax?EGF. Conclusions: Vaccinated group has a better performance if patients have a performance status 0 or 1, stage IV and age under 60 years. These prognostic factors influence overall survival in a positive way for those patients that received CIMAvax?EGF.

The Pamir plateau may have been a westward continuation of Tibet ***,the Rushan-Pshart suture is correlative to the Bangong-Nujiang suture of Tibet,and the Central Pamir is the lateral equivalent of the Qiangtang *** present the first detailed la-ICPMS zircon U-Pb chronology,major and trace element,and Lu-Hf isotope geochemistry of Taxkorgan two-mica monzogranite to illuminate the Tethys evolution in central ***-ICPMS zircon U-Pb dating shows that two-mica monzogranite is emplaced in the Cretaceous(118 Ma).Its geochemical features are similar to S-type granite,with enrichment in LREEs and negative Ba,Sr,Zr and Ti *** the samples show negative zirconεHf(t)values ranging from 17.0 to 12.5(mean 14.5),corresponding to crustal Hf model(TDM2)ages of 1906 to 2169 *** is inferred that these granitoids are derived from partial melting of peliticmetasedimentary rocks analogous to the Paleoproterozoic Bulunkuole Group,predominantly with muscovite schists *** on the petrological and geochemical data presented above,together with the regional geology,this work provides new insights that Bangong Nujiang Ocean closed in Early Cretaceous(120114 Ma).

＜正＞In this paper,we propose an integrated growth model of software reliability based on the non-homogeneous Poisson *** model shows the relationship between several distinctively developed *** also demonstrates not only each of the major attributes of the previous models but also the combined hidden attributes of those models. In addition,we derived estimates of the parameters of the integrated growth model and subsequently,tested the model with real world *** found that our growth model is better than previous models in its flexibility to explain the reliability of software attributes and ability to apply to various situations in the real world.

Several consecutive extreme cold events impacted China during the first half of winter 2020/21,breaking the low-temperature records in many *** to make accurate climate predictions of extreme cold events is still an urgent *** synergistic effect of the warm Arctic and cold tropical Pacific has been demonstrated to intensify the intrusions of cold air from polar regions into middle-high latitudes,further influencing the cold conditions in ***,climate models failed to predict these two ocean environments at expected lead *** seasonal climate forecasts only predicted the 2020/21 la Niña after the signal had already become apparent and significantly underestimated the observed Arctic sea ice loss in autumn 2020 with a 1-2 month *** this work,the corresponding physical factors that may help improve the accuracy of seasonal climate predictions are further *** the 2020/21 la Niña prediction,through sensitivity experiments involving different atmospheric-oceanic initial conditions,the predominant southeasterly wind anomalies over the equatorial Pacific in spring of 2020 are diagnosed to play an irreplaceable role in triggering this cold event.A reasonable inclusion of atmospheric surface winds into the initialization will help the model predict la Niña development from the early spring of *** predicting the Arctic sea ice loss in autumn 2020,an anomalously cyclonic circulation from the central Arctic Ocean predicted by the model,which swept abnormally hot air over Siberia into the Arctic Ocean,is recognized as an important contributor to successfully predicting the minimum Arctic sea ice extent.

This study was to evaluate the effects of the supplementation of saturated fatty acids with different chain lengths on growth performance,intestinal morphology,epithelial cell proliferation,differentiation and apoptosis in weaned ***-two weaned piglets(Duroc×landrace×Yorkshire,BW=7.81±0.26 kg)were weaned at 21 d and randomly assigned to 1 of 4 experimental treatments:(1)a basal diet(control);(2)control+0.3% butyrate(BT);(3)control+0.3%lauric acid(la);(4)control+0.3% stearic acid(SA).All piglets were then slaughtered for tissue sampling after having been fed experimental diets for 28 d after *** of BT increased the gain-to-feed ratio(G:F)(Pdepth(CD)ratio(VH:CD ratio)of the ileum were higher in the BT and la diets than that of the control diet(Pla,or SA all tended to increase jejunal proliferation(Ki67/crypt positive cells)(P=0.190);diets supplemented with BT or SA significantly increased the number of ki67-positive cells in the ileal crypt(Pla,or SA diets compared to those on the control diet(Pded to increase the protein level of mammalian phosphorylation target of rapamycin(p-m TOR,Pla or SA diets significantly increased p-m TOR protein expression(Pla,or SA promotes jejunal cell renewal in weaned *** the same time,increased proliferation of ileal crypt cells by promoting p-m TOR expression has beneficial effects on ileal morphology in weaned piglets.

The FAIR principles have been widely cited,endorsed and adopted by a broad range of stakeholders since their publication in *** intention,the 15 FAIR guiding principles do not dictate specific technological implementations,but provide guidance for improving Findability,Accessibility,Interoperability and Reusability of digital *** has likely contributed to the broad adoption of the FAIR principles,because individual stakeholder communities can implement their own FAIR ***,it has also resulted in inconsistent interpretations that carry the risk of leading to incompatible ***,while the FAIR principles are formulated on a high level and may be interpreted and implemented in different ways,for true interoperability we need to support convergence in implementation choices that are widely accessible and(re)-*** introduce the concept of FAIR implementation considerations to assist accelerated global participation and convergence towards accessible,robust,widespread and consistent FAIR *** self-identified stakeholder community may either choose to reuse solutions from existing implementations,or when they spot a gap,accept the challenge to create the needed solution,which,ideally,can be used again by other communities in the ***,we provide interpretations and implementation considerations(choices and challenges)for each FAIR principle.

Diverse models have been proposed for the role of the Tarim Craton within the Paleoproterozoic Columbia supercontinent assembly. Here we report a suite of-1.71 Ga Nb-enriched meta-gabbro lenses in the eastern Quanji Massif, within the Tarim Craton in NW China. The meta-gabbroic rocks have Nb contents of 11.5-16.4 ppm with Nb/la ratios varying from 0.84 to 1.02((Nb/la)_N = 0.81-0.98) and Nb/U ratios from 38.0 to 47.2. They show low SiO_2(45.1-48.5 wt.%) and MgO(5.96-6.81 wt.%) and Mg#(Mg# = Mg/(Mg + Fe) = 43.5-47.7), high FeO^t(13.0-15.7 wt.%) and moderate Ti02(1.70-2.51 wt.%).with tholeiitic affinities. These rocks possess low fractionated REE patterns without obvious Eu anomalies(Eu/Eu~* = 0.87-1.02). Their primitive mantle-normalized elements patterns display significant Zr-Hf troughs, positive Nb anomalies, weak negative Ti and P anomalies, and high contents of Rb and Ba,resembling Nb-enriched basalts generated in arc-related tectonic settings. Their arc-like geochemical signatures together with whole rock εNd(t) values of 0.4-2.1 and corresponding old T_(DM)(2.22-2.37 Ga)as well as(^(143)Nd/^(144)Nd)_t and(^(87)Sr/^(86)Sr)t(t = 1712 Ma) values of 0.5104-0.5105 and 0.7030-0.7058,respectively, suggest that their precursor magma originated from mantle wedge peridotite metasomatised by subduction-derived melts. The results from our study reveal subduction along the eastern periphery of the Tarim Craton and marginal outgrowth continuing to ~1.7 Ga within the Columbia supercontinent.
