Identifying timing and risk factors for early recurrence of resectable rectal cancer: A single center retrospective study

BACKGROUND Colorectal cancer is a common malignancy and various methods have been introduced to decrease the possibility of *** recurrence(ER)is related to worse *** date,few observational studies have reported on the analysis of rectal ***,we reported on the timing and risk factors for the ER of resectable rectal cancer at our *** To analyze a cohort of patients with local and/or distant recurrence following the radical resection of the primary *** Data were retrospectively collected from the institutional database from March 2011 to January *** data at diagnosis,perioperative and postoperative data,and first recurrence were collected and *** was defined via receiver operating characteristic *** factors were evaluated using the Kaplan–Meier method and Cox proportional hazards *** We included 131 *** optimal cut off value of recurrence-free survival(RFS)to differentiate between ER(n=55,41.9%)and late recurrence(LR)(n=76,58.1%)was 8 *** median post-recurrence survival(PRS)of ER and LR was 1.4 mo and 2.9 mo,respectively(P=0.008)but PRS was not strongly associated with RFS(R^(2)=0.04).Risk factors included age≥70 years[hazard ratio(HR)=1.752,P=0.047],preoperative concurrent chemoradiotherapy(HR=3.683,P9 d(HR=0.441,P=0.006).CONCLUSION RFS of 8 mo was the optimal cut-off *** ER was not associated with PRS,it was still related to prognosis;thus,intense surveillance is recommended.

8-hydroxy-2-(di-n-propylamino)tetralin intervenes with neural cell apoptosis following diffuse axonal injury

Previous studies have reported a neuroprotective effect of 8-hydroxy-2-（di-n-propylamino）tetralin （8-OH-DPAT） against traumatic brain injury. In accordance with the Marmarou method, rat models of diffuse axonal injury were established. 8-OH-DPAT was intraperitoneally injected into model rats. 8-OH-DPAT treated rats maintained at constant temperature served as normal temperature controls TUNEL results revealed that neural cell swelling, brain tissue necrosis and cell apoptosis occurred around the injured tissue. Moreover, the number of Bax-, Bcl-2- and caspase-3-positive cells increased at 6 hours after diffuse axonal injury, and peaked at 24 hours. However, brain injury was attenuated, the number of apoptotic cells reduced, Bax and caspase-3 expression decreased, and Bcl-2 expression increased at 6, 12, 24, 72 and 168 hours after diffuse axonal injury in normal temperature control and in 8-OH-DPAT-intervention rats. The difference was most significant at 24 hours. All indices in 8-OH-DPAT-intervention rats were better than those in the constant temperature group. These results suggest that 8-OH-DPAT inhibits Bax and caspase-3 expression, increases Bcl-2 expression, and reduces neural cell apoptosis, resulting in neuroprotection against diffuse axonal injury. This effect is associated with a decrease in brain temperature.

A case of rapid growing colonic NK/T cell lymphoma complicated by Crohn's disease

A 37-year-old man developed abdominal pain and bloody diarrhea 11 months before admission. The colonoscopy revealed multifocal ulcers in the colon. Histology showed active chronic inflammation. Although anti-tuberculosis medication was effective, his symptoms repeated 2 months later. The subsequent colonoscopy revealed more extensive irregular ulcers than before, and he was clinically suspected with intestinal malignant lymphoma. He underwent subtotal colectomy and was histologically suggested Crohn＇s disease, then 5-aminosalicylic and a combination of prednisone and azathioprine were administered in succession postoperatively, but they achieved minimal relief of symptoms for a period of 7 months. The third colonoscopy showed a large irregular ulcer in the ileocolon stomas, and primary colonic NK/T cell lymphoma was diagnosed through histological and immunophenotypic studies. Malignant lymphoma should be taken into consideration when clinically diagnosed Crohn＇s disease was refractory to medication or when its clinical course became aggressive.

A Review of the Research on the Indicator Evaluation of the New Development Philosophy

The philosophy of innovative, coordinated, green, open and inclusive development, collectively referred to as the new development philosophy, is an important component of Xi Jinping Thought on Socialist Economy with Chinese Characteristics for a New Era. It plays a vital role in China’s efforts to resolve problems in economic development and cultivate new advantages. Based on a keen understanding of the new development philosophy, we review the literature on the evaluation of a single indicator of the five dimensions of philosophy and that on a comprehensive evaluation of all the indicators of philosophy. The results show varied depths of research in single-indicator evaluation, and a lack of systematic study on comprehensive evaluation. We propose a multi-tier(macro–micro)-indicator evaluation system of new development philosophy, and recommend to establish a smart evaluation platform by integrating big data with traditional data, which will serve as a foundation and provide reference for the development of a scientific, well-conceived indicator evaluation system of new development philosophy that can guide the practice.

High power all-fiberized and narrow-bandwidth MOPA system by tandem pumping strategy for thermally induced mode instability suppression

An all-fiberized and narrow-bandwidth master oscillator power amplification(MOPA) system with record output power of 4 kW level and slope efficiency of 78% is demonstrated. Tandem pumping strategy is tentatively introduced into the narrow-bandwidth MOPA system for thermally induced mode instability(TMI) suppression. The stimulated Brillouin scattering(SBS) effect is balanced by simply using one-stage phase modulation technique. With different phase modulation signals, SBS limited output powers of 336 W, 1.2 kW and 3.94 kW are respectively achieved with spectral bandwidths accounting for 90% power of ～0.025, 0.17 and ～0.89 nm. Compared with our previous 976 nm pumping system, TMI threshold is overall boosted to be >5 times in which tandem pumping increases the TMI threshold of >3times. The beam quality(M～2　factor) of the output laser is well within 1.5 below the TMI threshold while it is ultimately saturated to be 1.86 with the influence of TMI at maximal output power. Except for SBS and TMI, stimulated Raman scattering(SRS) effect will be another challenge for further power scaling. In such a high power MOPA system, multidetrimental effects(SBS, SRS and TMI) will coexist and may be mutual-coupled, which could provide a well platform for further comprehensively investigating and optimizing the high power, narrow-bandwidth fiber amplifiers.

Effect of Sm^+ Rare Earth Ion on the Structural,Thermal,Mechanical and Optical Properties of Potassium Hydrogen Phthalate Single Crystals

Rare earth Sm＋ ion doped potassium hydrogen phthalate （KHP） single crystal was grown by slow evaporation technique. Single crystal and powder X-ray diffraction analyses confirm the crystalline perfection of Sm＋ ion doped KHP crystal. The functional groups of pure and Sm＋ ion doped KHP crystals were identified by Fourier transform infrared spectroscopy （FTIR） spectral studies. Thermogravimetric and differential thermal analyses were carried out to study the thermal behavior of the grown crystals. UV-Vis studies explored the optical transmittance of the grown crystals in the entire visible region. The mechanical strength and etching studies were performed to assess the perfection of the pure and Sm＋ ion doped KHP crystals. The refractive index and birefringence properties of the grown crystal were analyzed. The second harmonic generation efficiency of Sm＋ ion doped KHP crystals was observed by Kurtz-Perry powder test.

Optical emission spectrometric diagnosis of laser-induced plasma and shock front produced at moderate pressure

We present a non-contact optical investigation of laser-induced plasma at moderate Ar pressure ranging from 1 to 100 *** significant shock front and spatial fractionation among the different charged ions are demonstrated at the pressure of 20 *** collisions between Si IV ions and ambient Ar atoms generate distinct and excited ArⅡions,fresh SiⅢions,and electrons at the dense *** electron density peaks at the position of the shock front,indicating that the collision that yields electrons is dominant over the recombination process in the region of the shock layer and its immediate vicinity.

Effect of single-legged coil on 3D plasma boundary corrugation in EAST

A single-legged coil behind the lower divertor and covering a 120° toroidal angle is utilized in a recent EAST discharge, for the purpose of increasing the wetted area of the divertor surface by locally modifying the magnetic field near the X-point. The plasma response, in particular, the plasma boundary surface corrugation due to the single-legged coil current, is modeled by the updated MARS-F code, by computing the plasma displacement for all important toroidal harmonics(n?=?1, 2, 4 and 5) associated with the partial toroidal coverage by the coil. The plasma response produced by the single-legged coil is found to be non-local and is of the kinkpeeling type. For a reference EAST plasma with a lower single-null magnetic configuration, the plasma boundary corrugation near the X-point, produced by the upper single-legged coil, is about twice as large as that produced by the lower single-legged coil, despite the proximity of the latter to the X-point.
