Introduction Clostridioides difficile infection(CDI)is a common gastrointestinal illness worldwide,with increased prevalence and worse outcomes in patients with inflammatory bowel disease(IBD)[1].Fecal microbiota transplant(FMT)is an effective treatment for recurrent CDIs(RCDIs)with minimal serious adverse events(SAEs)reported[2].While immunosuppressive medications increase susceptibility to infections,studies have shown that FMT does not increase SAE risk in immunocompromised patients(including IBD patients on immunosuppressive therapy)[3].

Hepatic adenomas are a rare byproduct of oral contraceptive pill (OCP) use. Laboratory workup is often inconclusive. Diagnosis can be made via MRI;however, select cases may require further testing such as fine needle aspiration or core needle biopsy. We report a case of a 36-year-old female who was referred to gastroenterology for further evaluation of several liver lesions found incidentally on imaging. Due to risk factors, such as age, long-term oral contraceptive use and obesity, adenoma was high on the differential. Although infrequent, this case emphasizes the importance of a detailed history, including medication review, and physical examination.

Dear Editor:I read with interest the excellent report by Zhang et al.1 of a 55-year-old woman with Norwegian scabies(which is also referred to as crusted scabies)that masqueraded as scalp seborrheic dermatitis for 5 months until the correct diagnosis was *** authors attributed her immunosuppressed state-which accounted not only for the crusted scabies but also its location on her scalp in addition to other body areas-to a chronic hepatitis C *** mite infestation also had a component of scabies incognito from the application of high-potency topical corticosteroids(0.05%fluticasone propionate cream and 0.1%halcinonide solution)for 5 months.

Checkpoint inhibitors, a subset of immunotherapies, are effective treatment modalities for cancers such as melanoma. However, they are not without possible adverse effects. Drug-induced reactions may present with similar symptoms of sarcoidosis. We present the case of a 73-year-old female with a recent diagnosis of anorectal melanoma treated with checkpoint inhibitors who developed subsequent episodes of hypercalcemia. Her clinical symptoms along with her surgical cytology and radiologic results are characteristic of a drug-induced sarcoidosis-like reaction. With the increasing usage of immunotherapies in cancer treatment, it is important for clinicians to be aware of which patient population they are ideal for and possible adverse effects.

Objective: Metals including arsenic, lead, mercury, and cadmium are toxic and can increase cardiovascular disease risk. High-sensitivity C-reactive protein (hs-CRP) is a biomarker for inflammation and cardiovascular risk. This study will evaluate the association between urinary speciated arsenic, cadmium, lead, and mercury and blood values of hs-CRP in the United States adult population. Methods: A cross-sectional study using the 2015-2016 NHANES dataset, conducting multivariate linear regressions to analyze selected urinary metals and links with serum hs-CRP. Results: The sample consisted of 780 adults. In adjusted models, arsenocholine was found to be protective of inflammation, whereas aresnous acid, which is an inorganic, toxic type of arsenic acid, was positively associated with hs-CRP (b = 2.53). Conclusion: Urinary arsenous acid is a significant predictor of hs-CRP which is a biomarker for CVD.

Eruptive Keratoacanthomas within a Red Ink Tattoo: A Case Report

Background: There has been a longstanding debate regarding whether keratoacanthomas (KAs) are neoplastic or reactive/inflammatory lesions. Aim: The aim of this case report is to, within the aforementioned debate, offer support in favor of the potential reactive nature of keratoacanthomas. Case Presentation: A 64-year-old male presented with an eruption of 25 keratoacanthomas within a red ink tattoo. Eruptions were partially resolved over a matter of months, and completely resolved with treatment using oral Acitretin therapy. We believe this to be the third such reported case of eruptive KAs within only the red ink portions of a tattoo. Prior cases involved 2 and 8 KAs each. Discussion: Multiple studies suggest that KAs are neoplastic in nature and very distinct from cutaneous squamous cell carcinoma. Especially interesting is the finding that KAs have increased expression of apoptotic genes;this is particularly notable because of the tendency for these lesions to spontaneously involute. Skin tumorigenesis can occur in predisposed individuals after carcinogen exposure, thus red ink possesses theoretical potential as a carcinogen. KAs could be seen in this case as in fact a reactive neoplasm.

Background:Athletes have been shown to exhibit better balance compared to non-athletes(NON).However,few studies have investigated how the surface on which athletes train affects the strategies adopted to maintain *** distinct athlete groups who experience different types of sport-specific balance training are stable surface athletes(SSA) such as basketball players and those who train on unstable surfaces(USA) such as *** purpose of this study was to investigate the effects of training surface on dynamic balance in athletes compared to ***:Eight NON,eight SSA,and eight USA performed five 20-s trials in each of five experimental conditions including a static condition and four dynamic conditions in which the support surface translated in the anteroposterior(AP) or mediolateral(ML) planes using positive or negative feedback *** entropy(Ap En) and root mean square distance(RMS) of the center of pressure(Co P) were calculated for the AP and ML *** 3 × 5(group × condition) repeated measures ANOVAs were used to determine significant effects of group and condition on variables of ***:USA exhibited smaller Ap En values than SSA in the AP signals while no significant differences were observed in the ML Co P ***,the negative feedback conditions were associated with significantly greater RMS values than the positive feedback ***:USA exhibit unique postural strategies compared to *** unique strategies seemingly exhibit a direction-specific attribute and may be associated with divergent motor control strategies.
