Transcatheter arterial chemoembolization combined with CT-guided percutaneous thermal ablation versus hepatectomy in the treatment of hepatocellular carcinoma

Introduction:Transcatheter arterial chemoembolization(TACE)plus thermal ablation has been widely used recently in the treatment of hepatocellular carcinoma(HCC).In this study,we aimed to compare results of the combination of TACE and percutaneous thermal ablation with those of hepatectomy in patients with ***:The clinical data of 137 HCC patients who sequentially received TACE and computed tomography(CT)-guided percutaneous thermal ablation as an initial curative treatment(combination group)and 148 matched HCC patients who received hepatectomy(surgery group)between 2004 and 2011 were collected and *** TACE,multiphase contrast-enhanced CT was performed to identify the total number of tumors as well as lipiodol deposition in the *** was calculated by using the Kaplan-Meier method and compared by using the log-rank *** prognostic factors were assessed with multivariate Cox proportional hazards regression ***:Of all 285 patients,225(79.0%)had cancerous lesions≤5 cm in *** preoperative contrast-enhanced CT or magnetic resonance imaging,the number of tumors was 1–4 for each *** 1-,3-,and 5-year overall survival rates were 95,74%,and 67%in the combination group and 88,66,and 47%in the surgery group,respectively(P=0.004);the corresponding recurrence-free survival rates for the two groups were 92,69,and 61%and 75,58,and44%,respectively(P=0.001).In the multivariate analysis,treatment allocation was an independent prognostic factor for *** 60 patients in the combination group had sufficient imaging data,and 135 new lesions with lipiodol deposition were diagnosed as malignancies in 22 of 60 patients,whereas 20 new lesions were found in 11 of 148patients in the surgery ***:The combination of TACE and CT-guided percutaneous thermal ablation for HCC improves survival of HCC patients compared with hepatectomy.

Impact of primary tumor volume and location on the prognosis of patients with locally recurrent nasopharyngeal carcinoma

Introduction:The properties of a tumor itself were considered the main factors determining the survival of patients with locally recurrent nasopharyngeal carcinoma(NPC)treated with intensity-modulated radiotherapy(IMRT).However,recurrent tumors were mainly evaluated by using the American Joint Committee on cancer staging system,which was modeled on primary tumors and did not incorporate the tumor *** study aimed to investigate the prognostic values of the primary tumor location and tumor volume,and to determine whether evaluating these parameters could improve the current staging ***:Magnetic resonance(MR)images for 229 patients with locally recurrent NPC who underwent IMRT were analyzed ***:The skull base,parapharyngeal space,and intracranial cavity were the most common sites of *** was a difference in the survival between patients with T1 and T2 diseases(77.6%vs.50.0%,P38 cm3(48.7%vs.15.2%,P<0.01).Conclusions:A new staging system has been proposed,with T3 tumors being down-staged to T2 and with the tumor volume being incorporated into the staging,which may lead to an improved evaluation of these *** new system can be used to guide the treatment strategy for different risk groups of recurrent NPC.

Mitomycin C synergizes with PD-L1 blockade in treatment of non-small cell lung cancer

OBJECTIVE Programmed death 1 ligand(PD-L1) checkpoint inhibitor was a promising therapy but the response rate was only about 20%. Chemotherapy was reported to final y kil tumor cel s by triggering immune response. To improve the response of PD-L1 blockade,we tried to find chemotherapeuticaldrugs to combine with PD-L1 antibody(Ab). METHODS Non-small cell lung cancer(NSCLC) cells were pre-treated with mitomycin C(MMC) and then co-cultured with PBMCs to explore the effect of the combination of MMC with PD-L1 Ab. Lewis lung cancer(LLC) cells were used to establish xenograft model in mice and subjected to MMC or PD-L1 Ab treatment alone or combo. RESULTS MMC increased the expressions of PD-L1 and MHC-Ⅰ in NSCLC cells in vitro and in vivo and enhanced the activity of lymphocytes against NSCLC in *** mice treated with combo of PD-L1 Ab with MMC showed an improved overall survival and inhibition of the tumor growth compared to monotherapy,which was linked to the increases of lymphocyte infiltration and granzyme B release in LLC cell xenograftmodels in mice. Mechanically, MMC activated the ERK pathwayand subsequently activated c-Jun that bound with PD-L1 promoter and recruited its co-factor STAT3 to enhance PD-L1 expression. Meanwhile, the ERK pathway activated P65 to promote the MHC-I expression. CONCLUSION A combination of MMC with PD-L1 blockade may improve the antitumor response and offer a promising new treatment modality against NSCLC and encourage to further study to confirm in clinic.

Chondrocyte differentiation is a critical process for endochondral ossification,which is responsible for long bone development and fracture *** progress has been made in understanding the transcriptional control of chondrocyte differentiation;however,epigenetic regulation of chondrocyte differentiation remains to be further ***1 is a H3K36(histone H3 at lysine 36)***,we showed that mice with Nsd1 deficiency in Prx1+mesenchymal progenitors but not in col2+chondrocytes showed impaired skeletal growth and fracture healing accompanied by decreased chondrogenic *** combined RNA sequencing(RNA-seq)and chromatin immunoprecipitation sequencing(ChIP-seq)analysis,we identified sex determining region Y box 9(Sox9),the key transcription factor of chondrogenic differentiation,as a functional target gene of ***,NSD1 regulates Sox9 expression by modulating H3K36me1 and H3K36me2 levels in the Sox9 promoter region,constituting a novel epigenetic regulatory mechanism of ***,we found that NSD1 can directly activate the expression of hypoxia-inducible factor 1α(HIF1α),which plays a vital role in chondrogenic differentiation through its regulation of Sox9 ***,the results of our study reveal crucial roles of NSD1 in regulating chondrogenic differentiation,skeletal growth,and fracture repair and expand our understanding of the function of epigenetic regulation in chondrogenesis and skeletal biology.
