Knowledge, Attitudes and Practices (K.A.P) of Pharmacy Pharmacists of the Department of Dakar in the Care of Diabetics

Aim: To evaluate the attitude practice and knowledge of pharmacists in the management of diabetes in the department of Dakar. Methodology: Cross-sectional study, conducted from 01 March to 30 September 2017 including all pharmacists in the Dakar region who agreed to participate in the survey. Data was collected through a pre-established questionnaire. Results: A total of 144 pharmacists were included in the study, representing a participation rate of 87.58%. The average age was 33.90 years, with a sex ratio of 1.69. Pharmacists were single in 50.88% of cases, and a minimum experience of 5 years was found in 64.03% of cases. Pharmacist assistants accounted for 57.9% of our study population. The assessment of knowledge based on 16 items shows an average score of 9.02. The level of knowledge was low to insufficient in 37.21% of cases, average in (50%) half of cases and good for (13.16%). Less than half (34.21%) of pharmacists had received training and the average date of last training was 33.76 ± 24.6 months. Training was provided by medical representatives and academics in 66.66% and 26.67% respectively. The most felt positive feelings were respectively the need for help, the compassion in respectively 98.42%, 42.11%. Feelings felt and simultaneously cited were: need for help and compassion in more than 40% of respondents. Only 29% of pharmacists did not have a lot of difficulty with patient questions and the only reason was the lack of awareness (84.68%) of patients about their disease. Screening for diabetes by capillary blood glucose was performed by 57% of pharmacists. Dietary advice (76.15%) and referral to the hospital (94.41%) were the most common practices in case of blood sugar disruption. The dialogue with patients was impossible in 22.81% of pharmacists and the main reasons given were lack of time (53.85%) and lack of knowledge (34.62%). In this issue of the Chinese Journal of Cancer, European experts review current standards, trends, and future prospects in the difficult domain of high-grade glioma. In all fields covered by the different authors, the progress has been impressive. For example, discoveries at the molecular level have already impacted imaging, surgery, radiotherapy, and systemic therapies, and they are expected to play an increasing role in the management of these cancers. The European Organization for Research and Treatment of Cancer(EORTC) has pioneered new treatment strategies and contributed to new standards. The articles in this issue will cover basic molecular biological principles applicable today, novel surgical approaches, innovations in radiotherapy planning and delivery, evidence-based standards for radiotherapy alone or combined with chemotherapy, current standards and novel approaches for systemic treatments, and the important but often neglected field of health-related quality of life. Despite the advances described in these articles, the overall prognosis of high-grade glioma, especially glioblastoma, remains poor, and more research is needed to address this problem.

Acute Kidney Injury during Malaria in Togolese Children

Objective: To describe clinical, biological and evolutionary profile of Acute Kidney Injury (AKI) due to Severe Malaria in the pediatric department. Methodology: This was a retrospective descriptive study that took place from January to December 2012. It has been included children aged 0 - 15 years admitted for severe malaria with positive thick drop. AKI was defined by using the modified RIFLE (Risk Injury Failure Loss End stage) classification for Pediatrics. Results: 338 children admitted for severe malaria were included. AKI was diagnosed in 24 children, a prevalence of 7.1% according to pRIFLE classification: RISK in 10 (3%), InJURY in 9 (2.6%) and FAILURE in 5 (1.5%). The average age was 8.16 ± 4.2 years. Clinical features were dominated by hemoglobinuria in 87.5%, oliguria, vomiting and fever in 75%. The biological features were dominated by severe anemia (Hb 5.5 mmol/l) was found in 2 cases. The mean parasitic density was 22,120 trophozoites. Malaria was treated primarily with artemether in 83.3% of the cases. Dialysis was indicated in 2 cases for menacing hyperkalaemia, but was not done because of lack of financial means. In one case, hyperkalaemia was successfully treated with symptomatic measures, but in the second case, these measures were not allowed to normalize kalaemia, and death occurred. Conclusion: Acute post-malarial renal failure secondary to tubular necrosis may be the dominant clinical features of severe malaria. Death may occur. Preventing malaria would be the best way to avoid it.

Rheumatoid Arthritis of Man: A Study of 35 Cases in a Senegalese Hospital

Background: Rheumatoid arthritis is predominant in women. In men certain clinical signs can delay diagnosis. Methods: A cross-section study of the epidemio-clinical and immunobiological features of rheumatoid arthritis in a male, black African population was carried out at the Aristide Le Dantec, University hospital of Dakar. Results: we studied 35 male subjects with rheumatoid arthritis. Their mean age was 42 years and the average time between the onset of symptoms to diagnosis was 44 months. In 70% of these patients, at least one joint deformation was present, most frequently an ulnar deviation of the fingers (34.3%). Extra-articular symptoms were dominated by dry eye syndrome (34%) and anemia (17%). Anemia was significantly less frequent in men than in women. The erythrocyte sedimentation rate was accelerated in 51.4% and C reactive proteins were increased in 92% of patients. The rheumatoid factor was positive in all patients and the anticyclic Citrullinated Peptide (CCP) antibody was positive in 80% of cases. Conclusion: Male rheumatoid arthritis, relatively less studied, was associated with strongly positive immunological markers and a high rate of joint disorders.

Is chronic hepatitis C virus infection a risk factor for breast cancer?

Clinical Use of Extracorporeal Shockwave Therapy(ESWT)for the Treatment of Carpus Valgus Deformities in Young Foals:A Retrospective Study of 64 Cases(2006-2009)

The purpose of this study was to report the use and assess the effects of extracorporeal shockwave therapy (ESWT) for the treatment of carpal joint valgus deformities (CJVDs) in young foals. Only foals with CJVDs greater than 5° were included in the study. Foals were assigned to 3 treatment groups based on their degree of CJVD measured during the initial evaluation: valgus deformity (VD) of 5° to 8.9° (group 1), VD of 9° to 11.9° (group 2) and VD greater than or equal to 12° (group 3). ESWT was applied on the convex side of the angular deformity immediately following the initial radiographic evaluation. Foals were subsequently evaluated clinically and radiologically followed by treatment every 10 days until resolution of the VD, with resolution defined as a deviation less than 5 degrees. Each treatment group received specific exercise, hoof trimming and hoof/shoe extension recommendations. Sixty-four (64) foals were included in the study;ages ranged from 8 to 60 days old at inclusion in the study with a mean age of 26.7 days. Of the 28 foals included in group 1, 10 had bilateral CJVD. There were 21 in group 2, and 15 in group 3. Treatment success was defined as a VD angle less than 5°, and was reached in all foals in groups 1 and 2. Five (5) foals in group 3 completed the study with a VD angle of 5° to 6.7° at the last radiographic assessment. no major complications were observed during the study. In conclusion, ESWT in conjunction with controlled exercise, hoof trimming and hoof/shoe extensions corrected severe CJVDs in young foals. The use of ESWT eliminates possible negative side effects of general anesthesia and surgical techniques to treat VDs. Future studies should include a control population, more severe cases, other types of angular limb deformities, and older foals.

Sonographic evaluation of the post-operative rotator cuff: Does tendon thickness matter?

The goal of this work is to determine whether sonographic measures of tendon thickness correlate with post-operative functional parameters or re-tear rates. 53 consecutive patients with supraspinatus tears on MRI were examined by an orthopedic surgeon to determine: pre-and post-operative patient pain (via Constant-Murley scale), night time pain, maximum force production by the affected arm (in pounds), and range of motion deficits. Post-operative ultrasound at 3 and 12 months following surgery was performed to evaluate for recurrent tear and to measure tendon thickness. Post-operative tendon thickness was inversely related to patient age (r = -0.24;p value of tendon thickness. Post-operative tendon thickness did not successfully predict post-operative functional outcomes or pain levels (p > 0.05). normal post-operative tendon thickness of the rotator cuff decreases from 3 to 12 months following surgery. Thickness is reduced in patients with more severe tears and in older patients but does not correlate with post-operative patient pain or functional outcomes.

This paper reviews the data which demonstrate that Lyon genetically hypertensive (LH) rats exhibit a low renin form of hypertension. Since when compared to normotensive controls, LH rats exhibit a low renin release and are salt-sensitive. Despite this, the blockade of the renin-angiotensin system normalizes the blood pressure level and the regional blood flows in LH rats. Such a discrepancy between the compulsory presence of angiotensin Ⅱ for the hypertension to develop and the low level of renin release seen in LH rats led to two hypotheses: 1) the existence of an early, short lasting increase of renin release which would be sufficient for the occurrence of a stable hypertension; 2) an hypersensitivity to the effects of angiotensin Ⅱ. The study of the long-term effects of early short lasting blockades of the renin-angiotensin system allowed to exclude the first hypothesis.
