Discovery process, clinical characteristics, and treatment of patients infected with avian influenza virus （H7N9） in Shanghai

During the spring of 2013,a novel avian-origin influenza A （H7N9） virus emerged and spread in Shanghai.On May 9,2013,33 lab-confirmed cases （2 children and 31 adults） have been reported,among whom 13 died.Among the 31 adult cases of H7N9 avian influenza A virus infections in humans,80.6% （25/31） were older men （average age 65 years）.Clear history of poultry exposure was found in eight patients （8/33,24.3%） and the remaining 21 cases had been suspected poultry exposure.Clustered cases were found in two families （two cases from a father and his son,and the other two cases from a wife and her husband）.However,no evidence of human-to-human transmission had been identified.Avian influenza is one of the most dangerous contagions in poultry worldwide,and avian influenza A virus is the major pathogen responsible.Transmission from poultry to humans has now been proven.Some gene segments of the strain have mutated,and further mutations might result in human-to-human transmission.

Ectopic adrenocorticotropic hormone (ACTH) secretion rfrom a non-pituitary tumor is a main cause of Cushing's syndrome, which is associated with significant morbidity and mortality. Although early studies showed that the predominant cause of ectopic Cushing's syndrome is small cell carcinomas of the lungs, the incidence of carcinoid, including thymic carcinoid, has increased substantially in recent surveys) This evidence suggested that the clinical syndrome of ectopic ACTH production only occurs in tumors with high proopiomelanocortin (POMC, the precursor to ACTH) transcripts generated from the start of the pituitary transcription initiation site.2

Objective:To investigate the expressions of MAPK10,c-Jun and Itga6 in laryngeal carcinoma and its influence on the sensitivity to docetaxel,cisplatin and 5-fluorouracil(TPF)chemotherapy.Methods:Fifty-seven patients with supraglottic squamous cell carcinoma,who were treated by two cycles of TPF induction chemotherapy in our hospital,were enrolled in this study and divided into groups by chemotherapy resistance or chemotherapy sensitivity.The expressions of mRNA and protein of MAPK10,c-Jun and Itga6 in tumor tissues were evaluated by immunohistochemistry.The consistency of mRNA and protein expressions was tested,and the relation with the clinicopathological features was analyzed.Results:The positive rates of MAPK10 andc-Jun in the tumor tissues of the sensitive group were significantly higher than those of there assistant group,which was 90.48%and 100.00%,respectively.The expression rate of Itga6 was significantly higher in the resistant group,which was 83.33%(P0.05).However,the expressions of MAPK10 and c-Jun were negatively correlated withclinical stage and pathological grading(P<0.05).Negative correlations between MAPK 10 and Itga6,and between c-Jun and Itga6in tumor tissues were found by Spearman’srank correlation coefficient(P<0.05).The correlation was also negative in the resistant tumor tissues(P<0.05).Conclusion:The MAPK10 and c-Jun expressions were down-regulated,while the Itga6 expression was up-regulated in the chemo-resistant laryngeal carcinoma,and the expression levels of different factors were correlated witheach other.These factorsmight be important biomarkers for predicting outcomes of TPF chemotherapy in

目的:根据糖肽含量、蛋白去除率以及抗肿瘤活性优化守宫硫酸糖肽的提取和纯化工艺.方法:通过正交试验筛选最佳的提取方法,包括水提取法,超声提取法,酶+水提取法以及酶+超声提取法.考察Sevage法、三氯乙酸法(TCA)、酶+Sevage法、酶+TCA法、Sevage+TCA法对守宫糖肽脱蛋白的影响.用苯酚-硫酸法和考马斯亮蓝法分别分析糖肽的含量和蛋白去除率.通过台盼蓝拒染实验和MTS法分别测定守宫糖肽对人肝癌细胞株HepG2、人乳腺癌细胞株SKBR3以及对正常人淋巴细胞增殖的影响.结果:水提取守宫糖肽的最佳提取条件为:在煮沸的条件下,料液比为1:15,共提取3次,每次1 h.在该提取条件下守宫糖肽的提取率为4.049%.守宫糖肽对人肝癌细胞株HepG2、人乳腺癌细胞株SKBR3均有增殖抑制作用,并促进淋巴细胞的增殖.守宫糖肽的最佳脱蛋白法为木瓜蛋白酶+TCA法,在此方法下蛋白质去除率为50.46%,糖肽含量为14.27%,对人肝癌细胞株HepG2的抑制率为49.06%.结论:优化后的守宫糖肽提取、纯化工艺合理、稳定、可行,适合工业化生产.

TRAF6 polymorphisms not associated with the susceptibility to and severity of sepsis ina Chinese population

BACKGROUND:The tumor necrosis factor recepter associated factor (TRAF) 6 is an important intracellular adapter protein that plays a pivotal role in activating multiple inflammatory and immune related processes induced by cytokines. TRAF6 represents a strong candidate susceptibility factor for sepsis. We investigated whether polymorphisms at the TRAF6 gene are associated with the susceptibility to and severity of sepsis.;METHODS:A hospital-based case-control study was conducted with 255 patients with sepsis and 260 controls who were recruited from Zhengzhou, China. Haplotype tagging single nucleotide polymorphisms (htSNPs) were selected from the HapMap database and genotyped using the SNPstream genotyping platform. The associations with the susceptibility and disease severity of sepsis were estimated by logistic regression, and adjusted for age, sex, smoking, drinking, chronic diseases status, APACHEII score and critical illness status.;RESULTS:A total of 13 TRAF6 SNPs were tagged by 7 htSNPs. Five htSNPs (rs5030490, rs5030411, rs5030416, rs5030445 and rs3740961) were genotyped in the case control study. Genotype frequencies of the htSNPs were conformed to the Hardy-Weinberg equilibrium in both patients and controls. No significant association was found between the 5 htSNPs and the susceptibility to and severity of sepsis. Compared with the main haplotype -11120A/-10688T/-9423A/805G/12967G, no certain haplotype was associated with the significantly susceptibility to or severity of sepsis.;CONCLUSIONS:TRAF6 gene polymorphisms might not play a major role in mediating the susceptibility to and severity of sepsis in the Chinese population. A larger population-based case-control study is warranted.
