AIM To analyse clinical and long-term oncologic results after laparoscopic complete mesocolic excision(CME) for colonic cancer over a 10-year *** Consecutive patients who received laparoscopic CME at our hospital from 2007 to 2017 were prospectively registered and retrospectively analysed. In total, 341 patients were included with tumour-nodal-metastasis(TNM) stages 0-Ⅲ.RESULTS The mean age of the patients was 71.9 years. The median length of stay was 5 d. The mean lymph node harvest was 17.8. The mortality rate was 1.2%. Fifteen patients were reoperated on for anastomotic leaks. The local recurrence rate was 2.3%. Five-year TTR and cancer-specific survival CSS were 83.1% and 90.3%. The location of the tumour was not a significant variable for survival in unadjusted and adjusted survival analysis. TNM stage and anastomotic leaks were significant variables with respect to *** Laparoscopic CME results in acceptable complication rates and long-term oncologic results. It is important to avoid anastomotic leaks because of their negative effect on survival.

The widespread popularity of new surgical technologies such as laparoscopy, thoracoscopy and robotics has led many surgeons to treat esophageal diseases with these methods. The expected benefits of minimally invasivesurgery(MIS) mainly include reductions of postoperative complications, length of hospital stay, and pain and better cosmetic results. All of these benefits could potentially be of great interest when dealing with the esophagus due to the potentially severe complications that can occur after conventional surgery. Moreover, robotic platforms are expected to reduce many of the difficulties encountered during advanced laparoscopic and thoracoscopic procedures such as anastomotic reconstructions, accurate lymphadenectomies, and vascular sutures. Almost all esophageal diseases are approachable in a minimally invasive way, including diverticula, gastro-esophageal reflux disease, achalasia, perforations and cancer. Nevertheless, while the limits of MIS for benign esophageal diseases are mainly technical issues and costs, oncologic outcomes remain the cornerstone of any procedure to cure malignancies, for which the long-term results are critical. Furthermore, many of the minimally invasive esophageal operations should be compared to pharmacologic interventions and advanced pure endoscopic procedures; such a comparison requires a difficult literature analysis and leads to some confounding results of clinical trials. This review aims to examine the evidence for the use of MIS in both malignancies and more common benign disease of the esophagus, with a particular emphasis on future developments and ongoing areas of research.

Objective:The aim of this study was to evaluate the effect of the excision repair cross-complementing(ERCC1) expression on survival in advanced gastric cancer patients who underwent surgical resection and treated with oxaliplatin-based adjuvant ***:Sixty-three patients who underwent surgical resection for cure and treated with oxaliplatin-based adjuvant chemotherapy were included in this *** expressions of ERCC1 of gastric cancer were examined by immunohistochemistry and the patients were categorized into ERCC1-(+) and ERCC1-(-) *** relation between ERCC1 expression and survival of patients was ***:Of the 63 eligible patients,36 patients(57.1%) had tumor with a positive expression of ERCC1 and the remaining 27 patients had tumor with a negative ERCC1 *** differences of ERCC1 didn't correlated with age(P = 0.827),gender(P = 0.12),differentiation(P = 0.113),histological type(P = 0.942),site of tumor(P = 0.221),size of tumor(P = 0.608),stage(P = 0.815) and lymphatic invasion(P = 0.165).Overall survival(OS) was significantly longer in patients without ERCC1 expression,when compared to patients with ERCC1 expression(P = 0.023).Multivariate analysis revealed that ERCC1 expression significantly impacted on OS(HR:4.049;P = 0.000).Conclusion:We concluded that resected and treated with oxaliplatin-based adjuvant chemotherapy gastric cancer patients without ERCC1 expression have a better survival when compared to patients with ERCC1 ***1 expression will hopefully provide a rational basis for improving adjuvant chemotherapeutic strategies for gastric cancer ***1,itself,may be a prognostic factor for gastric cancer.

Introduction:Early and normative surgery is the only curative method for multiple endocrine neoplasia type 2(MEN 2)-related medullary thyroid carcinoma(MTC). Aims:To study the timing of prophylactic total thyroidectomy(TT) for MEN 2-related MTC with different RET mutations in a Chinese population,and to compare the sensitivity and accuracy of fully-automated chemiluminescence immunoassay(FACLIA) and radioimmunoassay(RIA) for serum calcitonin(Ct). Msthods:We collected 24 asymptomatic individuals from 8 unrelated Chinese families with MEN 2,and analyzed RET mutation and Ct *** we performed TT on 17 of the 24 individuals,including TT (2/17),TT with bilateral level VI lymph-node dissection(B-LND(VI);12/17) and TT with B-LND(VI) + modified unilateral/bilateral/local neck dissection(3/17). Results:Histopathology revealed bilateral/unilateral MTC in 15/17(88.2%;median diameter,1.0 cm) and bilateral C-cell hyperplasia in 2/17(11.8%;p.V292M/R67H/R982C and p.C618Y).Lymph-node metastasis/ fibro-adipose tissue invasion(p.C634R) or solely fibro-adipose tissue invasion(p.C634Y) were found in 2/ 17(11.8%).Elevated pre-surgical Ct(pre-Ct) was identified by FACLIA in 17/17(median age,24.0), while pre-Ct by RIA was found in only 6/15(P <0.001).The median follow-up was 22.0 months,during which 16/17 had no abnormality(one p.C634R individual had elevated Ct),and another 7 carriers still had consistently undetectable Ct by FACLIA. Conclusions:Our study highlights the importance and feasibility of individualized prophylactic TT for MEN 2-related MTC,based on predictive integrated screening of RET and pre-Ct ***,we recommend FACLIA to measure Ct for earlier diagnosis,treatment and follow-up monitoring of MTC.

Vascular targeted photochemotherapy(VTP) holds promise as a novel strategy of the focal treatment of localised prostate cancer(LPCa). It is convenient to perform, minimally invasive and can be conduct in ambulatory conditions. In this review, methodologic aspects of padoporfin- and padeliporfin-mediated VTP and its clinical application in focal treatment of LPCa as well as future perspective of this method were presented. Physicochemical and pharmacokinetic parameters of padoporphin and padeliporfin using as VTP photosensitizers were described, as well as methodologic question of radiation delivery and dosimetry, and oxygen monitoring in cancer tissue in context of VTP safety and efficiency of LPCa focal therapy were discussed. The results of clinical trials concerning application of padoporfin- and padeliporfin-mediated VTP in LPCa were also presented. The future of VTP is development of protocols, founded on the realtime feedback and rules-based approach to make this strategy a standard procedure in LPCa treatment. To evaluate clinical potential of this procedure, a costeffectiveness analysis is also necessary.

Renal cell carcinoma （RCC） is the most common renal tumor, accounting for 2%-3% of all malignancies. Though RCC is known to spread hematogenously, isolated RCC metastasis to the stomach is a rare event. In this article, we describe the clinical course of a patient who developed a pancreatic recurrence of RCC and 1 year later a gastric recurrence of RCC treated 10 years ago with a resection and interleukin-2 （IL-2）. Accumulating evidence indicates that metastatic involvement of the pancreas and stomach should be suspected in any patient with a history of RCC who presents with gastrointestinal symptoms even 10 years after RCC resection and immunotherapy.
