Efficacy and safety of Tuomin Zhiti decoction(脱敏止嚏汤) on patients with seasonal allergic rhinitis: A randomized， double-blind， placebo-controlled trial

Objective: To evaluate the clinical efficacy and safety of Tuomin Zhiti decoction (TZD) in the short-term treatment of seasonal allergic rhinitis (SAR). Methods: This study is a randomized， double-blind placebo-controlled， and single-center clinical trial. In April 2021， during the spring pollen season in Beijing， 94 SAR patients aged 19-60 years were randomized (1:1 ratio) to receive two weeks of TZD or placebo. The primary outcome was the change of Total Nasal Symptom Score (TNSS) and Total Ocular Symptom Score (TOSS) from baseline to the end of treatment. Secondary outcomes were the changed score of the mini Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ)， Rescue Medication Score (RMS)， and Patient Global Impression of Change (PGIC). All adverse events were recorded and evaluated by two senior physicians. Results: TZD significantly reduced both the total nasal symptom scores and total ocular symptom scores compared to scores at baseline， while the placebo group showed an increasing trajectory in both symptom scores. Compared to the placebo group， the TZD group also showed a greater improvement in the quality of life， scores of RMS， nasal and eye symptoms scores after TZD treatment (P < 0.001). Most notably， at the end of treatment， the proportion of remission measured by PGIC was significantly higher in the TZD group (82.97%) compared with the placebo group (47.72%， P < 0.001). Conclusion: This study suggested that two weeks of TZD is an effective and safe treatment for SAR patients and spring pollen allergy， TZD could significantly improve the nasal and eye symptoms and improve the quality of life of patients.

Assessing the Quality of the First Batch of evidence-based clinical Practice Guidelines in Traditional Chinese medicine

Objective: To assess the quality of the first batch of Chinese evidence-based clinical practice guidelines (CPGs) in Traditional Chinese medicine (TCM) using the Appraisal of Guidelines for Research and Evaluation (AGREE) instrument. Methods: evidence-based CPGs in TCM supported by the World Health Organization Western Pacific Regional Office (WHO/WPRO) and whose development was organized by the China Academy of Chinese Medical Sciences were identified and manually retrieved. CPGs were assessed using the AGREE instrument, and the data in each CPG were analyzed in terms of the six domains in the AGREE instrument: scope and purpose, stakeholder involvement, rigor of development, clarity and presentation, applicability, and editorial independence. Results: Twenty-eight CPGs were identified, of which 26 were included in the study. The AGREE instrument rated the 26 CPGs in terms of the six domains. The assessment results showed the following average scores: for editorial independence, 84.16%; for rigor of development, 80.95%; for scope and purpose, 79.96%; for clarity and presentation, 70.88%; for stakeholder involvement, 61.28%; for applicability, the average score was only 27.09%. In summary, nine CPGs were rated as "strongly recommended", six as "recommended with provision or alternation", and 11 as "unsure". Conclusion: Most of the first batch of Chinese evidence-based CPGs in TCM had significant shortcomings in applicability. It is suggested that special attention be paid to enhancing the quality of applicability when developing evidence-based CPGs in TCM.

Effect of add-on therapy with Traditional Chinese medicine on the survival of patients with anemic lower-risk myelodysplastic syndromes in the real-world setting:a retrospective study

OBJECTIVE: To illustrate the effect of add-on therapy of Traditional Chinese medicine(TCM) on the long-term survival of anemic lower-risk myelodysplastic syndromes(LR-MDS) patients. METHODS: In this study, we conducted a retrospective analysis of the data of patients with LR-MDS who were identified from Shuguang Hospital between January 2006 and June 2020. The Kaplan-Meier method was used to calculate overall survival(OS), while the Cox model was employed to analyze prognostic factors of long-term survival. A total of 162 patients who met the inclusion criteria were divided into two groups: the TCM group(n = 78) or the non-TCM group(n = 84). RESULTS: There were no significant differences in baseline characteristics. The cumulative survival rate of the TCM group was significantly higher than that of the non-TCM group [log-rank P = 0.029, hazard ratio(HR) = 0.484, 95% confidence interval(CI)(0.249, 0.942), P medicine may be a protective factor affecting the long-term survival of LR-MDS patients [HR = 0.509, 95% CI,(0.261, 0.993), P = 0.048]. The hematologic improvement erythroid response(HI-E) rate of the TCM group was significantly higher than that of the non-TCM group(69.23% vs 52.38%; P = 0.028). However, the duration of HI-E and cases of disease progression after HI-E did not significantly differ from the non-TCM group(P > 0.05). CONCLUSIONS: The integration of TCM and Western medicine has the potential to extend the OS of LR-MDS patients when compared to the use of Western medicine alone.

Engineered iron-based metal-organic frameworks nanoplatforms for cancer theranostics: A mini review

Up to now, numerous emerging methods of cancer treatment including chemodynamic therapy, photothermal therapy, photodynamic therapy, sonodynamic therapy, immunotherapy and chemotherapy have rapidly entered a new stage of development. However, the single treatment mode is often constrained by the complex tumor microenvironment. Recently, the nanomaterials and nanomedicine have emerged as promising avenues to overcome the limitation in cancer theranostics. Especially, metal-organic frameworks (MOFs) have gained considerable interests in cancer therapy because of their customizable morphologies, easy functionalization, large specific surface area, and good biocompatibility. Among these MOFs, iron-based MOFs (Fe-MOFs) are particularly promising for cancer treatment due to their properties as nano-photosensitizers, peroxidase-like activity, bioimaging contrast capabilities, and *** their structural regularity and synthetic tunability, Fe-MOFs can be engineered to incorporate organic molecules or other inorganic nanoparticles, thereby creating multifunctional nanoplatforms for single or combined theranostic modes. Herein, the minireview focuses on the recent advancements of the Fe-MOFs-based nanoplatforms for self-enhanced imaging and treatment at tumor sites. Furthermore,the clinical research development of Fe-MOFs-based nanoplatforms is discussed, addressing key challenges and innovations for the future. Our review aims to provide novice researchers with a foundational understanding of advanced cancer theranostic modes and promote their clinical applications through the modification of Fe-MOFs.

Background:Urinary tract infections(UTI),urolithiasis,and benign prostatic hyperplasia(BPH)are three of the most common nonmalignant conditions in ***,there is still a lack of comprehensive and updated epidemiological *** study aimed to investigate the disease burden of UTI,urolithiasis,and BPH in 203 countries and territories from 1990 to ***:Data were extracted from the Global Burden of Disease 2019,including incident cases,deaths,disabilityadjusted life-years(DALYs)and corresponding age-standardized rate(ASR)from 1990 to *** annual percentage changes(EAPC)were calculated to evaluate the trends of *** associations between disease burden and social development degrees were analyzed using a sociodemographic index(SDI).Results:Compared with 1990,the incident cases of UTI,urolithiasis,and BPH increased by 60.40%,48.57%,and 105.70%in 2019,*** age-standardized incidence rate(ASIR)of UTI increased(EAPC=0.08),while urolithiasis(EAPC=–0.83)and BPH(EAPC=–0.03)decreased from 1990 to *** 2019,the age-standardized mortality rate(ASMR)of UTI and urolithiasis were 3.13/100,000 and 0.17/100,000,*** had the largest increase(110.56%)in DALYs in the past three decades,followed by UTI(68.89%)and urolithiasis(16.95%).The burden of UTI was mainly concentrated in South Asia and Tropical Latin America,while the burden of urolithiasis and BPH was recorded in Asia and Eastern ***,the ASIR and SDI of urolithiasis in high-SDI regions from 1990to 2019 were negatively correlated,while the opposite trend was seen in low-SDI *** 2019,the ASIR of UTI in females was 3.59 times that of males,while the ASIR of urolithiasis in males was 1.96 times higher than that in *** incidence was highest in the 30–34,55–59,and 65–69 age groups among the UTI,urolithiasis,and BPH groups,***:Over the past three decades,the disease burden has increased for UTI but decreased for urolithiasis and BPH
