BACKGROUND Low grade fibromyxoid sarcoma(LGFMS)is a rare and benign mesenchymal tumor with indolent course,most commonly found in young or middle-aged *** majority of the LGFMSs are located in the trunk and deep soft tissue of the lower *** appear as well circumscribed,although not encapsulated,which often leads to incomplete surgical *** their seemingly benign appearance,these tumors have aggressive behavior with high metastatic and recurrence *** histopathologic examination of the specimen and its immunohistochemical analysis are mandatory for a precise *** SUMMARY We report a case of a 38 year-old-man who presented with jaundice and upper abdominal ***-detector computed tomography and magnetic resonance imaging showed a large left liver tumor mass,extending to the hepatoduodenal *** hepatectomy was performed with resection and reconstruction of hepatic artery and preservation of middle hepatic *** examination confirmed the tumor being a low-grade fibromyxoid *** and a half years after surgery,the patient died after being diagnosed with spine *** Due to poor response to all modalities of adjuvant treatment,we consider that the focus of treatment should be on surgery as the only option for curing the disease.

BACKGROUND Anaplasmosis is a tick-borne disease with a range of clinical manifestations,from a flu-like illness with fever and myalgias to a severe systemic disease with multisystem organ *** renal involvement is not a common presentation,there have been few cases reporting acute kidney injury from *** SUMMARY We present a 55-year-old female with anaplasmosis who developed acute kidney injury due to membranoproliferative glomerulonephritis(MPGN).The patient originally presented with cough and shortness of *** was admitted to the hospital with a diagnosis of community acquired pneumonia and received *** the hospital course she developed severe acute renal *** serological work up didn’t provide any conclusive ***,she underwent kidney biopsy which showed MPGN pattern suggesting autoimmune,multiple myeloma or infectious *** work up was undertaken which was negative for autoimmune diseases,vasculitis panel,paraproteinemias but tested positive for IgG anaplasma with high titers indicating *** Our case shows a unique presentation of severe acute renal failure from MPGN from tick borne *** is usually seen with autoimmune diseases,hepatitis C virus infections,***,we suggest that tick borne illness should also be considered when evaluating acute renal failure cases in tick borne prevalent regions.

Isolation and characterisation of human gingival margin-derived STRO-1/MACS^+ and MACS^-cell populations

Recently,gingival margin-derived stem/progenitor cells isolated via STRO-1/magnetic activated cell sorting（MACS） showed remarkable periodontal regenerative potential in *** a second-stage investigation,the present study＇s aim was to perform in vitro characterisation and comparison of the stem/progenitor cell characteristics of sorted STRO-1-positive（MACS~＋） and STRO-1-negative（MACS^-） cell populations from the human free gingival *** were isolated from the free gingiva using a minimally invasive technique and were magnetically sorted using anti-STRO-1 ***,the MACS~＋ and MACS^- cell fractions were characterized by flow cytometry for expression of CD14,CD34,CD45,CD73,CD90,CD105,CD146/MUC18 and ***-forming unit（CFU） and multilineage differentiation potential were assayed for both cell *** marker expression was examined using real-time polymerase chain reaction（PCR）.MACS~＋ and MACS- cell fractions showed plastic ***~＋ cells,in contrast to MACS- cells,showed all of the predefined mesenchymal stem/progenitor cell characteristics and a significantly higher number of CFUs（P〈0.01）.More than 95%of MACS~＋ cells expressed CD105,CD90 and CD73;lacked the haematopoietic markers CD45,CD34 and CD14,and expressed STRO-1 and CD146/***- cells showed a different surface marker expression profile,with almost no expression of CD14 or STRO-1,and more than 95%of these cells expressed CD73,CD90 and CD146/MUC18,as well as the haematopoietic markers CD34 and CD45 and ***~＋ cells could be differentiated along osteoblastic,adipocytic and chondroblastic *** contrast,MACS- cells demonstrated slight osteogenic *** MACS~＋ cells showed significantly higher expression of collagen I（P〈0.05） and collagen III（P〈0.01）,whereas MACS^- cells demonstrated higher expression of osteonectin（P〈0.05;MannWhitney）.The present study is the first to compare gingival MACS~＋ and MACS- cell populations demo

Expression of p53 predicts risk of prevalent and incident advanced neoplasia in patients with Barrett’s esophagus and epithelial changes indefinite for dysplasia

Background and aims:Patients with Barrett’s esophagus(BE)are at an increased risk for developing esophageal adenocarcinoma(EAC);thus they may undergo regular endoscopic *** epithelial changes cannot be unequivocally classified as negative or positive for dysplasia,a diagnosis of indefinite for dysplasia(IND)is *** biomarkers have been proposed as markers or predictors of neoplasia in the general BE population;however,their significance is not clear in patients with *** therefore performed a retrospective study to determine whether expression of these biomarkers was associated with the development of neoplasia in BE-IND ***:We searched our archives to identify all cases of BE-IND diagnosed between January 1992 and December *** analyses were used to semi-quantify the expression of p53,a-methylacyl-CoA racemase(AMACR),and cyclin D1.A univariate analysis was used to identify predictors for prevalent and incident neoplasia and advanced ***:Among the 103 patients with an index diagnosis of BE-IND who were included in this study,81(78.6%)underwent a follow-up biopsy within 12 months of diagnosis;10(12.3%)had neoplasia,including four(4.9%)with advanced *** 79 patients without prevalent neoplasia who underwent more than 1 year of follow-up,18(22.8%)had developed neoplasia,including four(5.1%)with advanced *** and cyclin D1 expression levels were not correlated with prevalent or incident neoplasia;however,high p53 expression(>5%)was associated with prevalent advanced neoplasia on surveillance biopsy(P=0.04)and with an increased risk of progression to advanced neoplasia(HR=12;P=0.03).Conclusion:In this study,p53 expression was found to be predictive of prevalent advanced neoplasia and progression to advanced neoplasia in patients with BE-IND.

Background Distal pancreatic carcinoma is one of the most lethal cancers largely due to its high incidence of distant *** study aims to assess the prognostic value of splenic-vasculature involvement in resected distal pancreatic *** In this retrospective study,we collected the clinicopathologic information of 454 patients with pancreatic cancer and performed univariate and multivariate analyses to identify factors associated with progression-free survival(PFS)and overall survival(OS),with an emphasis on the prognostic value of splenic-artery and-vein *** Univariate analysis revealed that larger tumor size,non-intraductal papillary mucinous neoplasm(non-IPMN)-associated adenocarcinoma,poor differentiation,stage pT3,nodal metastasis,lymphovascular invasion,perineural invasion,and pathologic and radiographic evidence of splenic-vein invasion were significantly associated with shorter PFS and OS(all P<0.05).Multivariate analysis confirmed non-IPMN-associated adenocarcinoma,stage pT3,stage pN1-2,and postoperative adjuvant chemotherapy as independent risk factors for both PFS and OS,and larger tumor size and radiographic evidence of splenic-artery invasion as predictors of PFS *** Guidelines should be developed for a uniform approach with regard to the examination and reporting of the status of the splenic vasculature when dealing with distal-pancreatic-cancer specimens.

Lymphoepithelioma-like hepatocellular carcinoma(LELHCC)is a very rare neoplasm,with distinct epidemiologic,morphologic and clinical *** mechanistic insight into the pathogenesis of this carcinoma suggests a pivotal role for the host immune system in the proliferation and progression of this ***,while detailed genomic profiling of these hepatic tumors have revealed an intra-tumoral inflammatory mutational signature that may predispose to immune checkpoint inhibitor efficacy,no published report has described their use in this tumor ***,with near 100 cases of LEL-HCC reported in the literature to date and the majority of cases confined to localized and resectable disease,current evidencebased practices in the unresectable setting are lacking,with unknown benefit of chemotherapy or *** report on the case of a 68 year-old man with unresectable,advanced LEL-HCC who had evidence of disease stability after starting on the immune checkpoint inhibitor *** disease response persisted off therapy for over a year and was potentially augmented by radiotherapy at the site of local *** this extremely rare tumor subtype,this case highlights the potential efficacy and safety of immune checkpoint blockade in LEL-HCC and reinforces the need for more robust,large-scale analysis of patients with these rare tumors to better evaluate treatment strategies and outcomes.

Novel MLH1 frameshift mutation in an extended hereditary nonpolyposis colorectal cancer family

AIM： To present novel frameshift mutation c.31delC [p.L11X] in the MLH1 gene identified in an extended Bulgarian hereditary non-polyposis colorectal cancer （HNPCC） family and to analyze the molecular and clinical findings within the pedigree concerning the proposal of adequate individual prophylactic strategy for all mutation carriers.METHODS： The pedigree of the family consists of 42 members in four generations. Search for mutations in the MLH1 and hMSH2 genes was performed in the proband. After PCR amplification of all exons including flanking intronic regions, amplicons were directly sequenced.RESULTS： The mutation was found in nine from the thirteen pedigree members who signed informed consent to participate in the study. In three adenocarcinomas, microsatellite instability and lack of the MLH1 protein expression were detected. The only one tubulovillous adenoma analyzed was microsatellite stable and the MLH1 protein showed an intact staining.CONCLUSION： The newly described mutation c.31delC is HNPCC causative. Besides the typical clinical features of the syndrome, we found a specific pathologic manifestation such as moderate to high differentiated adenocarcinomas of the colon. One of the mutation carriers developed a benign giant cell soft tissue tumor, The primary tumor Iocalizatlons were frequently extracolonic and detailed yearly gastrointestinal and gynecological examinations have been proposed to the mutation carriers. We emphasize the importance of including the HNPCC genetic counseling and testing as well in the following surveillance of all patients at risk in the services covered by the health insurance in Bulgaria.

Significance of the Expression of Rho-GDP Dissociation Inhibitor β,γ in Lung Squamous Cell Carcinoma and Adenocarcinoma

Objective： To study the expression of Rho-GDP dissociation inhibitor β,γ （Rho-GDIβ, Rho-GDIγ） in lung squamous cell carcinoma and adenocarcinoma and its relationship with the expression of RhoC （Ras homologus oncogenes C） and clinicopathologic parameters. Methods： Western blot assay was employed for Rho-GDIβ, Rho-GDIγand RhoC in lung squamous cell carcinoma and adenocarcinoma and non-neoplastic lung tissues of 37 cases with fresh specimens. Results： The study showed that Rho-GDIβ, Rho-GDIγ and RhoC were expressed in lung cancer and non-neoplastic lung tissues, the level in lung cancer tissue was much higher than that in non-neoplastic tissues （P〈0.001）. In lung cancer, the expression of Rho-GDIβwas much higher in patients with lymph node metastasis （P=0.021）, and the expression of Rho-GDIγ was much higher in poorly differentiated tumor than in well-differentiated and moderately differentiated tumor, but both of them were not correlated with other clinicopathologic parameters. The expressions of Rho-GDIβ and Rho-GDIγ were not correlated with the expression of RhoC. Conclusion： In lung cancer, Rho-GDIβand Rho-GDIγ may play a role in the tumorigenesis, Rho-GDIβ may promote metastasis, and Rho-GDIγ may have some relationship with differentiation.
