Fracture Healing in a Denervation and/or Nerve Ending Interpositioning Model in the Rat

Background: In this experimental study, we aimed to determine the possible changes in fracture healing due to denervation and/or nerve ending interpositioning. Methods: 50 Wistar Albino type male rats were divided into three study groups. A standard transverse diaphysial fracture in the femurs of the same side of all subjects under anesthesia was created and the fracture were fixed intramedullarily. While preserving the structural integrity of the sciatic nerve in the first group, neurectomy to the nerve in the second group was performed. In the third group, following the sciatic nerve cut, the proximal end of the nerve were interposed the fracture line. After a 28-day observational period, the callus formation in the subjects was examined radiologically, biomechanically and histopathologically. Results: Among all groups, the third group subjects showed significant increase in radiological area measurements when they are compared to the second group rats. There was no significant difference in biomechanical measurements of fractured femurs of the three groups. In histopathological evaluations, it was observed that denervation had increased the thickness of the cartilage and the number of the chondrocytes and osteoclasts significantly but decreased the number of fibroblasts compared to the control group. In addition to the denervation nerve ending interpositioning increased the bone thickness and the number of the osteoblasts but decreased the number of the osteoclasts significantly. Conclusions: While radiological observations exhibit that nerve ending interpositioning has resulted more hypertrophic callus formation, histopathological evaluations led us to that denervation created partial (immature) callus formation and nerve ending interpositioning demonstrated larger but immature callus formation.

The use of stem cells as carriers for therapeutic agents is an appealing modality for targeting tissues or organs of interest. Combined delivery of cells together with various information molecules as therapeutic agents has the potential to enhance, modulate or even initiate local or systemic repair processes, increasing stem cell efficiency for regenerative medicine applications. Stem-cell-mediated delivery of genes, proteins or small molecules takes advantage of the innate capability of stem cells to migrate and home to injury sites. As the native migratory properties are affected by in vitro expansion, the existent methods for enhancing stem cell targeting capabilities(modified culture methods, genetic modification, cell surface engineering) are described. The role of various nanoparticles in eq-uipping stem cells with therapeutic small molecules is revised together with their class-specific advantages and shortcomings. Modalities to circumvent common challenges when designing a stem-cell-mediated targeted delivery system are described as well as future prospects in using this approach for regenerative medicine applications.

Study Design: A prospective instrumental study design. Objectives: In this study, the validity of a device was investigated. Background: Frontal tibiofemoral malalignment is a predisposing factor for developing gonarthrosis. In a previous study a repositioning device was developed to clinically measure frontal tibiofemoral alignment. This device appeared to be more accurate than the conventional method. Case description: This study has examined three types of measurement: conventional method, repositioning device and a bilateral full-leg standing radiograph (Rx). We compared the results of the repositioning device and the conventional method to Rx. To determine the validity of the new repositioning device, n = 41 patients under 18 years of age were included in this study. They all underwent a standing Rx in their history. Outcome: In our study we found a negative correlation between the repositioning device and the already known Rx. Conclusion: The repositioning device was validated, but is not accurate enough to replace Rx. However, physiotherapists can use this device to obtain a more accurate clinical measurement of frontal tibiofemoral alignment.

Purpose： Pelvic ring fractures constitute 3%-8% of all fractures of the skeletal system and are generally related with high energy trauma. Sexual dysfunction following pelvic fracture has a high incidence, and affects the male patients both physically and psychologically. In this study, we aimed to investigate the impact and frequencies of comorhidities such as erectile dysfunction （ED） with adverse sociocultural and psychological consequences for the patient who had a pelvic ring fracture. Methods： This study included 26 men who corresponded to the inclusion criteria and agreed to participate our study. Results： According to fracture types, most of our cases were Tile type A1 and type A2. Severe and moderate ED were detected in 46.1% （12/26） of these patients via the International Index of Erectile Function-5 questionnaire.

Cerebral palsy is a multiple disability manifested by motor deficits and impairments of cognition, language, and body perception. To assess if body schema and body image distortion in children and youth with cerebral palsy can be represented through draw-a-person-test, outcome measure of conscious and subconscious body awareness, visual perception and cognition in two age-matched groups, one with cerebral palsy and other with developmental disability, were collected. The outcome was compared within the two groups and with reference data (healthy population) and correlated with the outcome of draw-a-person test. Decreased scores in the draw-a-person test in subjects with cerebral palsy compared both to subjects with developmental disorders and to healthy population had strongly correlated with decreased scores in visual perception. This suggests that draw-a-person test may provide a preliminary guidance for the assessment of visual perception. No statistically significant effect of cognitive abilities on performance in the draw-a-person test was found. The influence of both conscious and subconscious body awareness on the scores in the draw-a-person test was not significant, which confirms that this test alone is not fully reliable to detect the body schema and body image disorder in cerebral palsy.
