Background:Despite major reductions in malaria burden across Myanmar,clusters of the disease continue to persist in specific *** study aimed to assess the predictors of test positivity among people living in Paletwa Township of Chin State,an area of persistently high malaria ***:Four villages with the highest malaria incidence from Paletwa Town ship were purposively *** characteristics of 1045 subjects seeking malaria diagnosis from the four assigned village health volunteers from January to December,2018 were retrospectively *** household conditions and surroundings were also recorded using a *** statistics and logistic regression models were applied to investigate potential associations between individual and household characteristics and malaria ***:In 2017,the Paletwa township presented 20.9%positivity and an annual parasite index of 46.9 cases per 1000 *** falciparum was the predominant species and accounted for more than 80.0%of all *** 1045 people presenting at a clinic with malaria symptoms,31.1% were diagnosed with *** for test positivity included living in a hut[adjusted odds ratios(aOR):2.3,95%confidence intervals(CI ):1.2-4.6],owning farm animals(aOR:1.7,95% CI:1.1-3.6),using non-septic type of toilets(aOR:1.9,95%CI:1.1-8.4),presenting with fever(aOR:1.9,95%CI:1.1-3.0),having a malaria episode within the last year(aOR:2.9,95%CI:1.45.8),traveling outside the village in the previous 14 days (aOR:4.5,95%CI:1.5-13.4),and not using bed nets(aOR:3.4,95%CI:2.3-5.1).There were no statistically significant differences by age or gender in this present ***:The results from this study,including a high proportion of *** infections,little difference in age,sex,or occupation,suggest that malaria is a major burden for these study *** health education campaigns should be introduced to strengthen synchronous diagnosis

Identification of epitopes targeted following virus infection or vaccination can guide vaccine design and development of therapeutic interventions targeting functional sites,but can be ***,we employed peptide microarrays to map linear peptide epitopes(LPEs)recognized following SARS-CoV-2 infetion and *** detected by nonhuman primate(NHP)and patient IgMs after SARS-CoV-2 infection extensively overlapped,localized to functionally important virus regions,and aligned with reported neutralizing antibody binding *** LPE overlap occurred atfter infection and vaccination,with LPE clusters specifc to each stimulus,where strong and conserved LPEs mapping to sites known or likely to inhibit spike protein ***-specifc LPEs tended to map to sites known or likely to be afected by structural changes induced by the proline substitutions in the mRNA vaccine's S *** LPEs to regions of known functional importance in this manner may acelerate vaccine evaluation and discovery of targets for sile secific therapeutic interventions.

背景:无创通气(NIV)已经不局限于在重症监护室(ICUs)使用,在急诊科(EDs)和普通病房,NIV也逐渐成为常规手段。目的:分析不同临床科室中,护理和NIV结局的关系。方法:根据患者特点、所提供的护理和所使用的医疗程序、评估治疗的成功和失败、接口不耐受以及并发症。并发症分析包括支气管吸引术、气胸、皮肤损伤、气道分泌物过多、眼刺激、意识水平恶化、胃扩张和面罩周围过量的空气消耗。结果:387例患者中,194例(50.1%)来自ICU,121例(31.3%)来自ED,38例(9.8%)来自术后病房,34例(8.8%)来自普通病房。校正APACHE评分和NIV适应证后,进行回归分析。结果显示,与社区医院ICU比较,在一所每年NIV病例数低于50例的大学医院的ICU,NIV失败的风险升高3.3倍[95%CI(1.2~9.2)];在ICUs和普通病房,12%的患者由于接口不耐受而中止使用NIV;急慢性肺病(ACLD)患者的NIV失败风险更低[OR 0.2,95%CI(0.06~0.69)];缺乏湿化与治疗失败没有相关性[OR 0.2,95%CI(0.1~0.4)];气道分泌物过多的相关因素是肺炎[OR 2.5,95%CI(1.1~5.9)]和早期中止和(或)拔管[OR 3.3,95%CI(1.2~8.9)];接口不耐受的相关因素是使用传统ICU呼吸机[OR 4.4,95%CI(2.1~9.2)]和过度空气消耗造成的鼻部皮肤损伤[OR 2.4,95%CI(1.1~5.3)],尤其是口鼻面罩的使用[OR 3.5,95%CI(1.1~11.3)]。结论:普通病房收治的急性呼吸衰竭的肺炎患者,其接口不耐受和NIV失败的风险增加。在任何给予NIV治疗的科室,旋转型面罩均能促进NIV的成功。

Emergence of SARS-CoV-2 variants of concern(VOCs)suggests viral adaptation to enhance human-to-human transmission1,*** much effort has focused on characterisation of spike changes in VOCs,mutations outside spike likely contribute to *** we used unbiased abundance proteomics,phosphoproteomics,RNAseq and viral replication assays to show that isolates of the Alpha(B.1.1.7)variant3 more effectively suppress innate immune responses in airway epithelial cells,compared to first wave *** found that Alpha has dramatically increased subgenomic RNA and protein levels of N,Orf9b and Orf6,all known innate immune *** of Orf9b alone suppressed the innate immune response through interaction with TOM70,a mitochondrial protein required for RNA sensing adaptor MAVS ***,the activity of Orf9b and its association with TOM70 was regulated by *** propose that more effective innate immune suppression,through enhanced expression of specific viral antagonist proteins,increases the likelihood of successful Alpha transmission,and may increase in vivo replication and duration of *** importance of mutations outside Spike in adaptation of SARS-CoV-2 to humans is underscored by the observation that similar mutations exist in the Delta and Omicron N/Orf9b regulatory regions.
