背景:精神分裂症患者长期使用氯氮平易出现代谢综合征,尤其是抗精神病药物引起的体重增加。既往研究表明,二甲双胍可以减少由氯氮平引起的体重增加,但大多数研究的样本量相对较小。方法:通过检索多个中英文数据库,我们检索出在2015年12月15日之前发表的关于对成年精神分裂症患者使用二甲双胍治疗由氯氮平引起的体重增加的随机对照试验(randomized controlled trial,RCT)。两位研究者独立进行筛选和数据提取。我们使用RevM an5.3软件进行Meta分析与偏倚风险评估,并采用Cochrane GRADE(Cochrane Grades of Recommendation,Assessment,Development,and Evaluation)来评估证据质量。结果:共纳入六项研究(合并样本量:2治疗组207例、对照组207例),其中三项研究是双盲、安慰剂对照的RCT,另三项RCT未在对照组中使用安慰剂,也未使用盲法。meta分析发现,与对照组相比服用二甲双胍的患者体重减轻更为显著(MD=-2.89,95%CI:-4.20 to-1.59),并且体质量指数(body mass index,BMI)的降低也更为显著(MD=-0.81,95%CI:-1.16 to-0.45),但两组不良反应的发生率之间无统计学差异。根据GRADE量表,关于体重结果变化的证据强度为"中等",而关于BMI指数变化的证据强度是"高",但关于组间不良反应差异的证据强度是"低"或"非常低"。结论:对成年精神分裂症患者使用二甲双胍辅助治疗可能对治疗由氯氮平引起的体重增加和BMI指数上升是有效的。然而,有关该治疗安全性的证据质量很低,现有研究的随访时间比较短,有一半的研究并没有对结果的进行盲法评估。我们需要进行更大样本量的研究,控制安慰剂效应并随访患者至少24周,且对一系列相关结果要进行盲法评估(体重、BMI指数、血脂、胰岛素抵抗等),从而来证实这些结果。

Objective To study the specific amino acid variation in Nef that may be related to disease progression after infection with HIV-1 subtype B, a predominant strain circulating in China, and to determine whether changes in Nef secondary structure may influence different stages of AIDS development based on the concept that the Nef gene of HIV infection dramatically alter the severity of viral infection and virus replication and disease progression, and that long-term non-progressors （LTNP） of HIV infection are commonly associated with either a deletion of the Nef gene or the defective Nef alleles. Methods The study subjects were divided into LTNPI（n=14）, LTNP2 （n=16） and slow progressor （SP, n=19） groups for mutational analysis of the Nef sequence. The data were obtained by using Bioedit, MEGA, Anthewin and SAS software. Results Residues in Nef TA48/49 and K151 occurred more frequently in the LTNP group while AA48/49 was more frequently observed in the SP group. Of the differences observed in the secondary structure comparison using Nef consensus sequences of these three groups, one was roughly corresponding to the Nef48/49 mutation site. Conclusion TA48/49, Kiss, and AA48/49 in the Nef gene might be associated with the different stages of HIV infection, and there may be a link between the Nef secondary structure and the progression of HIV- 1 infection.

BACKGROUND Neurovascular compression(NVC) is the main cause of primary trigeminal neuralgia(TN) and hemifacial spasm(HFS). Microvascular decompression(MVD) is an effective surgical method for the treatment of TN and HFS caused by NVC. The judgement of NVC is a critical step in the preoperative evaluation of MVD, which is related to the effect of MVD treatment. Magnetic resonance imaging(MRI) technology has been used to detect NVC prior to MVD for several years. Among many MRI sequences, three-dimensional time-of-flight magnetic resonance angiography(3D TOF MRA) is the most widely used. However, 3D TOF MRA has some shortcomings in detecting NVC. Therefore, 3D TOF MRA combined with high resolution T2-weighted imaging(HR T2WI) is considered to be a more effective method to detect *** To determine the value of 3D TOF MRA combined with HR T2WI in the judgment of NVC, and thus to assess its value in the preoperative evaluation of *** Related studies published from inception to September 2022 based on PubMed, Embase, Web of Science, and the Cochrane Library were retrieved. Studies that investigated 3D TOF MRA combined with HR T2WI to judge NVC in patients with TN or HFS were included according to the inclusion criteria. Studies without complete data or not relevant to the research topics were excluded. The Quality Assessment of Diagnostic Accuracy Studies checklist was used to assess the quality of included studies. The publication bias of the included literature was examined by Deeks’ test. An exact binomial rendition of the bivariate mixed-effects regression model was used to synthesize data. Data analysis was performed using the MIDAS module of statistical software Stata 16.0. Inflammatory demyelinating diseases(IDD)of the central nervous system(CNS)occur throughout the world and are the leading cause of nontraumatic neurological disability in young adults.[1]Early diagnosis of IDD allows prompt immunotherapies that minimize relapse,disabilities and mortality.[2]Multiple sclerosis(MS)is the most frequent IDD in *** histological characteristics of chronic MS lesions are well known to neuropathologists;however,diagnosing an inflammatory demyelinating process in biopsy may be challenging since the specimens are frequently small and often represent only parts of a larger lesion.[3]Furthermore,the histopathological hallmarks of acute MS differ fundamentally from the well-known characteristics of chronic demyelinated plaques observed in autopsies.[3]Kuhlmann et al[4]recently proposed a new histological classification system for MS *** this new classification system corresponds to clinical manifestations and thus reflects the temporal multiplicity of MS and whether it can be applied to all IDD cases remain to be further studied.

To understand the long-term performance of bioretention systems under sulfamethoxazole (SMX) stress, an unplanted bioretention system (BRS) and two modified BRSs with coconut-shell activated carbon (CAC) and CAC/zero-valent-iron (Fe^(0)) granules (CAC-BRS and Fe/CAC-BRS) were established. Both CAC-BRS and Fe/CAC-BRS significantly outperformed BRS in removing total nitrogen (TN)(CAC-BRS:82.48%;Fe/CAC-BRS:78.08%;BRS:47.51%), total phosphorous (TP)(CAC-BRS:79.36%;Fe/CAC-BRS:98.26%;BRS:41.99%),and SMX (CAC-BRS:99.74%, Fe/CAC-BRS:99.80%;BRS:23.05%) under the long-term SMX exposure (0.8 mg/L, 205 days). High-throughput sequencing revealed that the microbial community structures of the three BRSs shifted greatly in upper zones after SMX *** functional genera, dominantly Nitrospira, Rhodoplanes, Desulfomicrobium, Geobacter,were identified by combining the functional prediction by the FAPROTAX database with the dominant genera. The higher abundance of nitrogen functional genes (nirK, nirS and nos Z) in CAC-BRS and Fe/CAC-BRS might explain the more efficient TN removal in these two systems. Furthermore, the relative abundance of antibiotic-resistant genes (ARGs)sul I and sulII increased in all BRSs along with SMX exposure, suggesting the selection of bacteria containing sul genes. Substrates tended to become reservoirs of sul genes. Also,co-occurrence network analysis revealed distinct potential host genera of ARGs between upper and lower zones. Notably, Fe/CAC-BRS succeeded to reduce the effluent sul genes by1-2 orders of magnitude, followed by CAC-BRS after 205-day exposure. This study demon-strated that substrate modification was crucial to maintain highly efficient nutrients and SMX removals, and ultimately extend the service life of BRSs in treating SMX wastewater.

Assessment of antibiotic resistance genes in dialysis water treatment processes

Dialysis water is directly related to the safety of hemodialysis patients, thus its quality is generally ensured by a stepwise water purification cascade. To study the effect of water treatment on the presence of antibiotic resistance genes (ARGs) in dialysis water, this study used propidium monoazide (PMA) in conjunction with high throughput quantitative PCR to analyze the diversity and abundance of ARGs found in viable bacteria from water having undergone various water treatment processes. The results indicated the presence of 35 ARGs in the eflfluents from the different water treatment steps. Twentynine ARGs were found in viable bacteria from the effluent following carbon filtration, the highest among all of the treatment processes, and at 6.96 Log (copies/L) the absolute abundance of the cphA gene was the highest. Two resistance genes, erm (36) and mtrD-02, which belong to the resistance categories macrolides-lincosamides-streptogramin B (MLSB) and other/efflux pump, respectively, were detected in the effluent following reverse osmosis treatment. Both of these genes have demonstrated the potential for horizontal gene transfer. These results indicated that the treated effluent from reverse osmosis, the final treatment step in dialysis-water production, was associated with potential health risks.
