The discovery of hepatitis C has been a landmark in public health as it brought the opportunity to save millions of lives through the diagnosis,prevention and cure of the *** combined work of three researchers,Alter H,Houghton M and Rice C,which set the basis for the diagnosis,treatment and prevention of hepatitis C apart from laying the ground work for a new approach to study infections in general and developing new antiviral *** is a story of a transfusion-associated infection.A series of clinical studies demonstrated the existence of an infectious agent associated with *** was followed by the identification of what was later known to be the hepatitis C virus(HCV)and the development of diagnostic *** all preceded the full molecular identification and demonstration of a causal *** it ended up with the development and discovery of a new class of therapeutic drugs,the direct acting antivirals,which are now used not only to cure the disease but most probably,to eliminate the *** work started with Dr Alter H who demonstrated that a new virus was responsible for the majority of post-transfusion hepatitis followed by Houghton M who cloned the virus and developed the blood test to identify those cases that carried the ***,the work of Rice C demonstrated that a cloned HCV produced after applying molecular biology techniques could cause long-standing infection and cause the same disease as the one observed in humans.

Diet switch and omega-3 hydroxy-fatty acids display differential hepatoprotective effects in an obesity/nonalcoholic fatty liver disease model in mice

AIM To study the effect of 18-hydroxy-eicosapentaenoic acid(18-HEPE) and 17-hydroxy-docosahexaenoic acid(17-HDHA) in a murine model of obesity/nonalcoholic fatty liver *** C57 BL/6 mice were fed with standard chow diet(CD) or high-fat, fructose-enriched diet(HFD) for 16 wk. Then, three groups were treated for 14 d with either, diet switch(HFD for CD), 18-HEPE, or 17-HDHA. Weightand fasting glucose were recorded on a weekly basis. Insulin tolerance test was performed at the end of treatment. Histological analysis(HE and Masson's trichrome stain) and determination of serum insulin, glucagon, glucagon-like peptide 1(GLP-1), glucose-dependent insulinotropic polypeptide, adiponectin and resistin were carried out as well as liver proteins by western *** Mice treated with hydroxy-fatty acids 18-HEPE and 17-HDHA displayed no weight loss or improved insulin sensitivity. However, these mice groups showed a significant amelioration on serum GLP-1, adiponectin and resistin levels. Also, a significant reduction on inflammatory infiltrate was observed at both portal and lobular zones. Furthermore, up-regulation of PPARα/γ protein levels was observed in liver tissue and it was associated with decreased levels of NF-κB also determined by western blot analysis. On the other hand, diet switch regimen resulted in a marked improvement in most parameters including: weight loss, increased insulin sensitivity, decreased steatosis, restored levels of insulin, glucagon, leptin, adiponectin and resistin. However, no significant changes were observed regarding inflammatory infiltrate in this last *** 18-HEPE and 17-HDHA differentially exert hepatoprotective effects through up-regulation of nuclear receptors PPARα/γ and amelioration of serum adipokines profile.

Fully Intermittent, Thrice-Weekly Dose of Anti-Tuberculosis Drugs; Their Efficacy under Routine Program Conditions of India

When discovered, anti-tuberculosis drugs were always administered in Daily-doses; it worked wonders! Later, studies claimed these drugs could cure even in thrice-weekly doses （intermittently）. Not everyone believed that. Three Reviews （Cochrane, Thorax, Azhar） appear unconvinced! Also, most nations （including 20 High TB Burden Countries） continue to practice time-tested Daily dosing! INDIA adopted a unique low-cost, thrice-weekly regimen around 2000. During 15 years, under national program, Thrice-weekly doses have been administered to 15 million TB patients, self-claiming 85% success. To revisit long-term outcome of all registered patients in a district; identify the one, who didn＇t achieve long-lasting recovery with a single course, was later found re-registered for a second innings; his name figures twice in national registry （a Repeater）. Author heads District Tuberculosis Center, Faridabad, India; monitors thousands of current/ex-patients on Thrice-weekly regimen. By providence, he enjoys unique access to registered patients, data, district TB-workers. Two methods of data collection： 1. Author motivated district work-force; jointly conducted systematic Retrospective Record Review of hand-written registers of past 15 years, one at a time--thus identified over 1,575 Repeaters. 2. Author has been conducting ongoing Clinical Audit in his busy TB OPD for 6 years. He is able to cherry-pick （average） one Repeater per day; thus tracked 3,100 Repeaters-about half come in from Delhi＇s premier TB institutes, duly diagnosed, referred in for local registration for Re-treatment. Long-term fate of 36,785 registered TB patients could be analyzed. 4,675 patients observed to have returned sick, meriting Re-registration, Re-treatment; thus over 12.7 % registered twice/thrice （names, unique govt. ID＇s, irrefutable evidence enclosed）-an adverse outcome, unreported thus far-a bubble. Besides, study recorded 1,590 （5%） Deaths; 2,590 （8%） Defaulters. Under India＇s routine program cond

A Blockchain-Based Life-Cycle Environmental Management Framework for hospitals in the COVID-19 Context

During the coronavirus disease 2019 (COVID-19) emergency, many hospitals were built or renovated around the world to meet the challenges posed by the rising number of infected cases. Environmental management in the hospital life cycle is vital in preventing nosocomial infection and includes many infection control procedures. In certain urgent situations, a hospital must be completed quickly, and work process approval and supervision must therefore be accelerated. Thus, many works cannot be checked in detail. This results in a lack of work liability control and increases the difficulty of ensuring the fulfillment of key infection prevention measures. This study investigates how blockchain technology can transform the work quality inspection workflow to assist in nosocomial infection control under a fast delivery requirement. A blockchain-based life-cycle environmental management framework is proposed to track the fulfillment of crucial infection control measures in the design, construction, and operation stages of hospitals. The proposed framework allows for work quality checking after the work is completed, when some work cannot be checked on time. Illustrative use cases are selected to demonstrate the capabilities of the developed solution. This study provides new insights into applying blockchain technology to address the challenge of environmental management brought by rapid delivery requirements.

Comparative study between intramedullary interlocking nailing and minimally invasive percutaneous plate osteosynthesis for distal tibia extra-articular fractures

Purpose:Treatment of distal tibia fractures poses significant challenge to orthopedic surgeon because of poor blood supply and paucity of soft tissue *** is considerable controversy regarding the superior option of treatment for distal tibia fracture between the minimally invasive percutaneous plate osteosynthesis(MIPPO)technique and intramedullary interlocking(IMIL)nailing for extra-articular distal tibia *** aim of our study is to compare the functional outcome between the two treatment ***:This was the prospective comparative study of 100 patients with distal third tibia fractures divided into two *** first group of patients were treated with MIPPO technique while the second group of patients were managed by IMIL *** were followed up in outpatient department to assess the functional outcomes,malunion,delayed union,nonunion,superficial and deep infection between the two *** analyses were performed using the SPSS software(version 16.0).Results:Average malunion(degrees)in the MIPPO group was 5(3—7)±1.41 vs.10.22(8—14)±2.04 in the IMIL group(p=0.001).Similarly postoperative knee pain in the IMIL group was 10%vs.2%in the MIPPO group(p=0.001).In terms of superficial infection and nonunion,the results were 8%vs.4%and 2%vs.6%for the MIPPO and IMIL group,respectively(p=0.001).Conclusion:Both procedures have shown the reliable method of fixation for distal extra-articular tibia fractures preserving the soft tissue,bony vascularity and fracture hematoma that provide a favourable biological environment for fracture *** the results of the study,we have slightly more preference for the MIPPO technique.

Effect of Carbohydrate Intolerance and Gestational Diabetes on Obstetric and Perinatal Outcomes

Objectives: To compare the frequency of obstetric and perinatal complications among patients with carbohydrate intolerance (CHI) and gestational diabetes (GD). Materials and Methods: In a prospective study at the civil hospital of Culiacán, we studied 182 pregnant patients between 16 and 30 weeks of gestation (WG) who received a glucose tolerance test following the oral administration of 100 g of glucose. The patients were classified as normal (n = 53), CHI (n = 61) and GD (n = 68) between January 15 to November 30, 2012. The analyzed variables included the frequency of polyhydramnios, preeclampsia-eclampsia, obstetric hemorrhage, fetal macrosomia, and admission to a neonatal intensive care unit (NICU). Results: No significant differences were found between women with CHI and women with GD with respect to obstetric hemorrhage (P = 0.774), preeclampsia-eclampsia (P = 0.590), and macrosomia (P = 0.119). However, polyhydramnios was more frequent in CHI group (OR = 3) compared to GD, whereas admission to the NICU was lower in the CHI group compared to GD group (OR = 0.38). Conclusion: Preeclampsia-eclampsia, obstetric hemorrhage, and macrosomia were similar among patients with CHI and GD. In contrast, polyhydramnios was more frequent in patients with CHI compared to patients with GD, but admission to the NICU was more frequent in patients with GD.

Study of blood exposure-related mental health illness among clinical nurses

Nurses are subjected to high amount of stress in the medical setting, and work-related stress often leads to mental problems. This study aims to investigate the mental health status of nurses exposed to blood through needlestick injuries. A total of 302 nurses working in the hospital of Guangdong, China, participated in this study. Out of the 302 nurses, 140 did not experience any needlestick injuries during the previous week, whereas 162 nurses experienced needlestick injuries. The General Health Questionnaire （GHQ）-28 Standardized Questionnaire, which uses physical, anxiety, social function, and depression subscales, was used in this study. No significant difference between nurses exposed to blood and nurses not exposed to blood was found in terms of gender, age, length of employment, and civil status （P 〉 0.05）. Results from the GHQ-28 Standardized Questionnaire showed that 75.9% （123/162） of nurses exposed to blood were suspected to suffer from mental disorders, whereas 40% （56/140） of nurses not exposed to blood were suspected to suffer from mental disorders. The mean mental health scores of nurses exposed to blood and those not exposed were 8.73 ± 7.32 and 5.69 ± 5.70, respectively. From these results, we can conclude that blood exposure from needlestick injuries leads to higher prevalence of depression, anxiety, and stress symptoms in nurses. This finding highlights the importance of providing efficient, adequate, and appropriate support services after nurses are exposed to blood from needlestick injuries.
