Application of a bioch.mical and clinical model to predict individual survival in patients with.end-stage liver disease

AIM:To investigate th. capability of a bioch.mical and clinical model,BioCliM,in predicting th. survival of cirrh.tic ***:We prospectively evaluated th. survival of 172 cirrh.tic *** model was constructed using clinical(ascites,enceph.lopath. and variceal bleeding) and bioch.mical(serum creatinine and serum total bilirubin) variables th.t were selected from a Cox proportional h.zards *** was applied to estimate 12-,52-and 104-wk *** model's calibration using th. h.smer-Lemesh.w statistic was computed at 104 wk in a validation ***,th. model's validity was tested among an independent set of 85 patients wh. were stratified into 2 risk groups(low risk≤8 and h.gh.risk>8).RESULTS:In th. validation coh.rt,all measures of fi t,discrimination and calibration were improved wh.n th. bioch.mical and clinical model was *** proposed model h.d better predictive values(c-statistic:0.90,0.91,0.91) th.n th. Model for End-stage Liver Disease(MELD) and Ch.ld-Pugh.CP) scores for 12-,52-and 104-wk mortality,*** addition,th. h.smer-Lemesh.w(h.L) statistic revealed th.t th. bioch.mical and clinical model(h.L,4.69) is better calibrated th.n MELD(h.L,17.06) and CP(h.L,14.23).Th.re were no significant differences between th. observed and expected survival curves in th. stratified risk groups(low risk,P=0.61;h.gh.risk,P=0.77).CONCLUSION:Our data suggest th.t th. proposed model is able to accurately predict survival in cirrh.tic patients.

Evaluation of Coronary Venous Anatomy by Multislice Computed Tomograph.

Background:Th. coronary venous system is increasingly targeted for pacing in patients with.severe h.art failure. Th. recent advancement of Multi-Detector Computed Tomograph. (MDCT) allows accurate analysis of th. coronary arteries, but little data exist on its role in assessing cardiac venous anatomy. Th. aim of th. present study was to investigate th. feasibility of using MDCT in evaluating th. cardiac venous anatomy in patients with.h.art disease;Meth.ds and Results: One h.ndred and eigh.een subjects (59 ± 11 years, 100 males) were studied by contrast enh.nced 16-slice CT with.retrospective ECG-reconstructions. Th. diameter, length. and angulations of coronary veins were measured from both.volume rendered 3-dimensional images and curved multi-planar images. Th. coronary sinus vein was visualized in all of patients. h.wever, th. posterior, postero-lateral, lateral, antero-lateral and anterior veins were found in 71.2%, 50.0%, 65.3%, 9.3% and 96% patients, respectively. Twenty-th.ee (19.5%) subjects h.d neith.r postero-lateral nor lateral cardiac veins. Th. ostial diameter angle of take-off and total length.of th. postero-lateral and lateral veins ranged from 1.7 - 7.0 mm, 38 - 160 degrees and 2.6 - 10.6 mm, respectively;Conclusions: Th.s study confirms th. feasibility of assessing diameter, length. and angulations of coronary veins by MDCT. Th.s non-invasive information sh.uld be useful for pre-operative lead placement planning for patients sch.duled to h.ve cardiac resynch.onization th.rapy.

Evaluating ph.sical ch.nges of iron oxide nanoparticles due to surface modification with.oleic acid

Th. ph.sical ch.racterization of a colloidal system of superficially modified magnetic nanoparticles (MNPs) is presented. Th. system consists of oleic acid-coated iron oxide nanoparticles (OAMNP) suspended in water. A structural analysis is carried out by using standard ph.sical tech.iques to determine th. diameter and sh.pe of th. MNPs and also th. width.of th. coating sh.ll. Th. colloidal stability and th. polydispersity index of th.s ferrofluid are determined by using Zeta potential measurements. Additionally, th. magnetic ch.racterization is conducted by obtaining th. DC magnetization loops, and th. blocking temperatures are determined according to th. ZFC–FC protocol. Finally, th. values of power absorption density P of th. ferrofluid are estimated by using a magneto-calorimetric procedure in a wide range of magnetic field amplitude h.and frequency f. Th. experimental results exh.bit sph.rical-like sh.pe of OAMNP with.(20 ± 4) nm in diameter. Due to th. use of coating process, th. parameters of th. magnetization loops and th. blocking temperatures are significantly modified. h.nce, wh.le th. uncoated MNPs sh.w a blocking state of th. magnetization, th. OAMNP are superparamagnetic above room temperature (300 K). Furth.rmore, th. reach.d dependence P versus f and P versus h.of th. ferrofluid with.coated MNPs are clearly fitted to linear and quadratic correlations, respectively, sh.wing th.ir accordance with.th. linear response th.ory.

AIM:To investigate wh.th.r we could create natural autologous tissue patch.s in th. subcutaneous space for organ repair. METh.DS: We implanted th. following th.ee types of inert foreign bodies in th. subcutaneous tissue of rats to produce autologous tissue patch.s of different geometries:(1) a large-sized polyvinyl tube(L = 25 mm,internal diameter = 7 mm) sealed at both.ends by h.at application for obtaining a large flat piece of tissue patch.for organ repair;(2) a fine polyvinyl tubing(L = 25 mm,internal diameter = 3 mm) for creating cylindrically sh.ped grafts for vascular or nerve repair; and(3) a slurry of polydextran particle gel for inducing a bladder-like tissue. Implantation of inert materials was carried out by making a small incision on one or eith.r side of th. th.racic-lumbar region of rats. Subcutaneous pockets were created by blunt dissection around th. incision into wh.ch.th. inert bodies were inserted(1 or 2 per rat). Th. incisions were closed with.silk sutures,and th. animals were allowed to recover. In case of th. polydextran gel slurry 5 m L of th. slurry was injected in th. subcutaneous space using an 18 gauge needle. After implanting th. foreign bodies a newly regenerated encapsulating tissue developed around th. foreign bodies. Th. tissues were h.rvested after 4-42 d of implantation and studied by gross examination,h.stology,and h.stoch.mistry for organization,vascularity,and presence of mesench.mal stem cells(MSCs)(CD271+CD34+ cells). RESULTS: Implanting a large cylindrically sh.ped polyvinyl tube resulted in a large flat sh.et of tissue th.t could be tailored to a specific size and sh.pe for use as a tissue patch.for repairing large organs. Implanting a smaller sized polyvinyl tube yielded a cylindrical tissue th.t could be useful for repairing nerves and blood vessels. Objective: To determine th. association between th. acute inflammatory markers and Diabetic Renal-Retinal Syndrome in our patients. Meth.ds: A total of 44 patients were included in th.s transverse (cross sectional), observational, analytical, and comparative study. Two groups of patients were created. Results: In patients with.ch.onic renal failure, stages 3, 4, or 5 (scale 1 to 5) were identified and divided into two groups: Group I (Patients with.concomitant diabetic retinopath. stabilized with.ph.tocoagulation) 22 patients. Group II (Patients with.progression of retinopath., after ph.tocoagulation, in th. form of intraocular bleeding) 22 patients. Th. number of patients with.ch.onic renal failure in stage 3 was: Group I;16. In Group II;12 patients (OR: 1.0). Stage 4: Group I;4 patients. Group II: 3 patients (OR: 1.0, CI 95%;0.3843 - 2.021). Stage 5: Group I;2 patients. Group II;7 patients (OR: 1.8148, CI 95%;1.0448 - 3.1523;Ch.-squared (x2) 2.7450, p = 0.097). t-Test found statistical significance in Eryth.ocyte Sedimentation Rate (ESR): p h.binomial test sh.wed p h.wed p h.U of Mann Wh.tney. Conclusions: With.th.s study revealed a possible association of inflammatory markers with.Diabetic Renal-Retinal Syndrome.
