Objective:To investigate the relationship between upper gastrointestinal bleeding and neutrophil-lymphocyte ratio(NLR),platelet-lymphocyte ratio(PLR),and lymphocyte-monocyte ratio(LMR),and examine whether they can be used as markers of ***:The retrospective single-center study included a total of 189 patients with upper gastrointestinal bleeding admitted to the tertiary emergency department between January 2018 and January ***,59 patients with similar demographic characteristics were selected as the control ***,42 patients with active bleeding and 147 patients without active bleeding were categorized into two groups according to their endoscopy *** NLR,PLR,LMR values,potential risk factors,and demographic characteristics were ***:The mean NLR levels were found significantly higher in the patient group compared to the control group(P<0.001),whereas the mean LMR levels were significantly lower in the patient group(P<0.001).The mean NLR and PLR levels were significantly higher in patients with active bleeding compared to those without active bleeding(P<0.001),whereas the mean LMR levels were significantly lower(P<0.001)for patients with active *** optimal cut-off value of NLR was found 2.1 for predicting uppergastrointestinal bleeding,with a sensitivity of 80.2%and specificity of 78.9%(AUC:0.840;P<0.001).Conclusions:NLR was determined to be a parameter that can be used as an indicator of active bleeding in patients with upper gastrointestinal bleeding.

Lithium aluminum titanium phosphate(LATP)is one of the materials under consideration as an electrolyte in future all-solid-state lithium-ion *** ceramic processing,the presence of secondary phases and porosity play an important *** a presence of more than one secondary phase and pores,image analysis must tackle the difficulties about distinguishing between these microstructural *** this study,we study the phase evolution of LATP ceramics sintered at temperatures between 950 and 1100℃by image segmentation based on energy-dispersive X-ray spectroscopy(EDS)elemental maps combined with quantitative analysis of LATP *** found aluminum phosphate(AlPO4)and another phosphate phase((Lix)PyOz).The amount of these phases changes with sintering ***,since the grains act as an aluminum source for AlPO4 formation,the aluminum content in the LATP grains ***,the amount of secondary phase changes from more(Lix)PyOz at 950℃to mainly AlPO4 at 1100℃sintering *** also used scanning electron microscopy(SEM)and confocal laser scanning microscopy(CLSM)to study the evolution of the LATP grains and AlPO4,and LATP grain size increases with sintering *** addition,transmission electron microscopy(TEM)was used for the determination of grain boundary width and to identify the amorphous structure of AlPO4.

Aim: To investigate the effect of melatonin on ischemia–reperfusion (I/R) injury in rat ovary. Material and Method: A total of 24 Wistar rats were divided into four groups. The rats were first numbered randomly and then randomly divided into four equal groups: sham, torsion, detorsion and melatonin groups. In group I (n = 6) sham, as well as in group II ovary torsion was not performed and no drug was administered. In group III, 1 hour of ischemia and 2 hours of reperfusion were performed and no drug was given. In group IV, melatonin was dissolved in 1% ethanol just before use. Melatonin was injected 1 hour before torsion to the torsion plus melatonin group. Right ovaries were surgically removed in all groups. Result: Malondialdehyde (MDA) levels were measured. We suggested that administration of melatonin would be in treating ovaries from torsion/detorsion induced damage in humans. The histopathological changes such as vascular congestion, edema, hemorrhage, and follicular degeneration were found to be decreased in the melatonin + ischemia-reperfusion group. Ischemia-reperfusion group: PECAM-I expression is positive in vessel of the stromal area. The follicles and interfolicular area infiltrated with CD68 positive cells were increased in a time of ischemia-reperfusion exposure dependent manner. Conclusions: Melatonin after ischemia-reperfusion CD31 and CD68 expressions is weak, due to the reduction of the inflammatory effects and hemorrhage is thought that melatonin is effective.

Drought becomes a serious issue in Iraq as upstream countries water policies and climate change consequences. It has negative impacts on different sectors, the environment, biodiversity, economy, and water resources. Long periods of severe drought and no rainfall winter are continuing during the last decades and the trend lines of the drought index gravitate toward unrecorded levels. This research studied the drought by using the standardized precipitation index “SPI” by analyzing the rainfall record since 1980 for the northern prone of Iraq which includes Kurdistan Region Governorates “KRG” (Sulaymaniya, erbil, and Dohuk), Mosul, and Kirkuk to find out the drought pattern, magnitude, and duration, and mapping the results. This index is a very powerful index that is used worldwide when only rainfall data are available. SPI generator is used for more accuracy and confidence. The results show that all governorate has a vital issue and has a drought magnitude passed the thresholds of M = -1.0, -1.5, -2.0, -2.5, -3.0, and -4.0 which means a deficit in the soil moisture content, surface water, and groundwater. In Sulaymaniyah, the record for SPI3, SPI6, and SPI48 respectively is about -3.4, -3.54, and -2.63;in erbil -2.73, -4.67, and -2.72;in Dohuk -4.22, -4.34, and -2.25;in Mosul -2.57, -2.16: in Kirkuk -3.39, -3.04, and -3.41. It is clear that all governorates have depletion in groundwater except Mosul which has contentious recharge, and in erbil, both soil moisture and surface water has no huge deficit due to high rainfall and snowpack in the region. The results concluded that the whole region is subject to drought and under threat of water resources depletion;it needs urgent long-term plans in a sustainable manner to manage and conserve those sources and mitigate the climate change consequences.

PURPOSE: Cases treated surgically using wide excision plus classic Limberg flap or wide excision plus asymmetricmodified Limberg flap were compared with respect to complications and patient comfort in the postoperative period. METHODS: In this prospective, randomized study, 68 of 70 patients were followed for a mean of 29.22 (range, 6- 44) months after wide excision plus classic Limberg flap (Group 1, n = 35) and after asymmetric modified Limberg flap closure (Group 2, n = 33). RESULTS: There were significantly more macerations in Group 1 (P < 0.001). All macerations were detected on the lower part of the incision left on the intergluteal sulcus, and infections occurred subsequent to maceration. The infection rate was statistically higher in Group 1 than in Group 2 (P = 0.028). We noted that as a result of these complications, time to suture removal (P = 0.001), discharge from hospital (P = 0.001), and time off from work (P = 0.001) were significantly longer for Group 1 than for Group 2. There were two recurrences in the inferior part of the suture line in Group 1 and none in Group 2, which showed no statistical difference (P = 0.493). CONCLUSIONS: The deep intergluteal sulcus and midline gap were slightly flattened over the anococcygeal region. The vacuum effect was decreased, and there were less macerations and fewer infections. Time off from work and discharge time from hospital were shortened by eliminating the moisture effect and reducing complications by lateralizing the lower part of the suture line.

Juvenile idiopathic arthritis(JIA) is the most common chronic rheumatologic disease in childhood, which represents a nonhomogeneous group of disorders that share the clinical manifestation of arthritis lasting at least 6 wk under the age of 16. The exact diagnosis requires exclusion of other diseases that cause arthritis. The exact etiopathogenesis of JIA is still unknown. The interactions between genetic factors, environmental exposures and immune mechanisms are thought to contribute to pathogenesis of the disease. The "International League Against Rheumatism" classification divides JIA into 7 subtypes: oligoarticular JIA, rheumatoid factor(RF) positive polyarticular JIA, RF negative polyarticular JIA, systemic-onset JIA, enthesitis-related arthritis, juvenile psoriatic arthritis and undifferentiated JIA. Each subgroup of JIA is characterized by a different mode of presentation, disease course and outcome. The improvements in treatment of JIA in the last 2decades, such as the early introduction of intraarticular corticosteroids, methotrexate and biologic agents, have dramatically upgraded the prognosis of the disease. If untreated, JIA may cause devastating results, such as disability from joint destruction, growth retardation, blindness from chronic iridocyclitis, and even multiple organ failure and death in systemic-onset JIA. The aim of treatment is the induction of remission and control the disease activity to minimize the pain and loss of function, and to maximize quality of life. JIA is a disease having a chronic course, which involves active and inactive cycles over the course of years. Recent studies showed that nearly half of the patients with JIA enter adulthood with their ongoing active disease. This review elucidates how recent advances have impacted diagnosis, pathogenesis and current treatment.
