SUMMARY OF CLINICAL STUDY ON ACUPUNCTURE COMBINED WITH EPIDURAL ANESTHESIA FOR CHOLECYSTECTOMY

Objective: TO study clinical effect and anesthetic method of acupuncture anesthesia combined with epidural administration of small dose of anesthetic for cholecystectomy. Methods: A total of 194 cases of cholecystectomy patients were randomly divided into acupuncture combined with epidural anesthesia group (group A, n = 66), acupoint-skin electrical stimulation combined with epidural anesthesia group (group B, n = 63) and simple epidural anesthesia group (group C, n=65). Observations were conducted using single-bland method. Bilateral Neiguan (PC 6) and Zusanli (ST 36) were punctured and stimulated electrically in group A and only stimulated electrically via cutaneous electrodes in group B. Epidural anesthetic used was 1.5% Lidocaine and the anesthetic level was controlled to reach T4-11 . Results: The class-I (excellent) rates of group A, B and C were 75.76%, 60.32% and 13.85% respectively, showing significant differences between group A and C and group B and C (P <0.001). The initial dose, doses of every hour and every case of group A and B were lower than those of group C. The dose of every hour of group C was 36.23% and 3. 75% higher then group A and B respectively (P < 0. 001), suggesting that acupuncture or acupoint-skin electrical stimulation could strengthen anesthetic effect and reduce the dose of epidural anesthetic. During operation, indexes of the life signs as HR, MAP, RR, TV, MV, SpO2 and ECG kept basically stable and all patients in group A passed surgical operation safely. Conclusion: Acupuncture or acupoint-skin electrical stimulation combined with epidural anesthesia can be used as one of the anesthetic methods for cholecystectomy.

Meta-analysis of ear acupoint therapy to treat sleep disorders in hemodialysis patients

Background:To evaluate the therapeutic effect of auricular therapy on sleep disorders in hemodialysis patients using meta-analysis methods,and to provide evidence-based support for clinical ***:PubMed,Embase,Cochrane Library,Chinese Biomedical Literature Database,China Journal Full-text Database,Chinese Science and Technology Journal Full-text Database,and Wanfang Resource Database were searched up to February *** controlled trials of auricular therapy for sleep disorders in hemodialysis patients were collected and included in the ***-analysis was performed using RevMan 5.3 ***:587 patients from 7 randomized controlled trial studies were *** auricular therapy group had better sleep conditions than the conventional care group,as evaluated by the Pittsburgh Sleep Quality Index,in terms of total Pittsburgh Sleep Quality Index score(mean difference(MD)=–5.11,95%confidence interval(CI)(–5.25,–4.96),P<0.00001),subjective sleep quality(MD=–0.64,95%CI(–0.72,–0.56),P<0.00001),falling asleep time(MD=–0.37,95%CI(–0.49,–0.26),P<0.00001),sleep time(MD=–0.38,95%CI(0.44,–0.32),P<0.00001),sleep efficiency(MD=–0.15,95%CI(–0.23,–0.07),P<0.0001),sleep disturbances(MD=–0.18,95%CI(–0.28,–0.09),P=0.0002),daytime dysfunction(MD=–0.89,95%CI(–1.18,–0.6),P<0.00001),and hypnotic medication(MD=–0.22,95%CI(–0.4,–0.03),P=0.02).Conclusion:Auricular therapy is an effective method for improving sleep quality in hemodialysis patients,providing evidence-based support for clinical practice.

BACKGROUND: Some factors have been reported to besassociated with a greater likelihood of sustained viral re-sponse ( SVR) in the interferon (IFN) treatment of chronichepatitis C. The factors include HCV genotype, HCVRNA level in serum, state of liver disease, baseline bodyweight, age, sex, and race. The aim of this trial was to in-vestigate the influence of HCV genotype on the IFN treat-ment of patients with chronic hepatitis ***: The genotypes of HCV virus were determinedin the patients with chronic hepatitis C from several hospi-tals of China enrolled into the randomized, opened andcontrolled trial of Peg-IFN alpha-2a (pegasys) treatment,controlled with IFN-α-2a (roferon-A). The serum ALTlevels and HCV RNA concentrations of the patients weredetected before and at the end of treatment and during thefollow-up. The influence of HCV genotype on the IFNtreatment of patients with chronic hepatitis C was analyzedin intention-to-treat (ITT) ***: The HCV genotypes of 202 patients were deter-mined. Of these patients, 158(78.22%) were infected withgenotype 1 HCV and 44(21.78%) with genotype *** viral response at the end of treatment (ETVR) andsustained viral response (SVR) rates were 53.80% and25.32% respectively in patients with genotype 1 HCV, butthey were 61.36% and 43.18% in patients with genotypenon-1. The difference of SVR between patients with geno-type 1 HCV and those with genotype non-1 was significant(P =0.021). After being grouped by the used drugs, theETVR rates of patients infected with genotype 1 and non-1HCV were 76.83% and 80.95% in the patients treated withpegasys (P =0.686); but their SVR rates were 35.37% and66.67% (P =0. 01). The viral relapse rate of genotype 1HCV (55.56%) was significantly higher than that of geno-type non-1 HCV (23.53%) (P=0.02). ＜正＞It is necessary to extract and recognize visual evoked potential（VEP） in the transient VEP based brain-computer interface *** method and wavelet filtering were used to extract the weak VEP signal noisy raw *** vectors gained in wavelet transform domain were input to the perceptron to achieve the recognition of VEP and produce the brain-computer interface control *** showed that the feature extraction in the wavelet domain can effectively extract the VEP signal feature, reduce noise and decrease the dimensionality at the same time,and the VEP classification algorithms can recognize the VEP signal and produce the brain-computer interface control signal correctly, which is useful to increase the information transfer rate of brain-computer interface.

Successful weaning from ventilator play a critical role in improving the prognosis of patients who are treated in the intensive care unit(ICU).Several spontaneous breath trials(SBTs)have been developed to evaluate the ability of an individual patient to remain spontaneous breathing after disconnecting mechanical ***,it remains unclear which SBT techniques should be preferentially taken into consideration for critically ill patients receiving *** performed this network meta-analysis to investigate the comparative efficacy of all common SBTs and then found that automatic tube compensation(ATC)was better than others in increasing initial SBTsuccess rate and successful weaning ***,ATC should be optimally considered to assess the ability of liberating ventilator in critical ill.

Obviously, as human beings survive and prosper better with fire than without it, it follows that possible burn injuries will stay with human beings.

Dear Editor,Worldwide,around 55 million people had prevalent dementia in 2019,which is expected to triple by 2050,especially in low-and middle-income countries[1].Lacking timely diagnosis and limited effective treatment for dementia make identifying risk factors crucial for its early prevention,among which dietary factors have received increasing attention[1].Recently,accumulating evidence from population-based studies has linked the temporal patterns of energy intake(TPEI),usually defined as the temporal distribution of energy intake during a day,to mortality and metabolic diseases[2],such as diabetes and *** vitro and in vivo studies also revealed that meal timing could drive metabolic alterations and circadian regulation[3],and disrupted meal timing altered the peripheral circadian clocks in the hippocampus and consequently affected cognitive function[4].

BACKGROUND: The resection rate of primary liver tumor in China is only about 20%. A lot of patients with moderate and advanced liver tumor may lose the chance of opera- tion. The objective of present research was to study the ef- ficacy of transcatheter arterial chemoembolization (TACE) combined with percutaneous injection of chemical agents and acetic acids in the treatment of patients with primary liver cancer (PLC). METHODS: Thirty-three patients with middle and ad- vanced stage of PLC were divided into two groups: percu- taneous injection of chemical agents and acetic acids (15 patients, group A) and TACE (18 patients, group B). RESULTS: Tumor diameter and serum AFP level reduced to 86.6% and 83.3% in group A, and 55.5% and 40% in group B, respectively. There was significant difference be- tween the two groups ( P < 0 . 0 1 ) . The 1-, 2-, 3-, 4-year survival rates of group A were 96.7%, 86.6%, 51.3%, 33.3%, respectively and in group B were 66.7%, 44.4%, 16.7%, 0%, respectively (P<0.01). CONCLUSION: TACE combined with percutaneous injec- tion of chemical agents and acetic acids is efficacious to in- crease the survival rate of patients with PLC.
