PURPOSE: This prospective study was designed to identify factors that could predict conversion in patients undergoing first laparoscopic ileocecal resection for Crohn’ s disease. METHODS: Between 1998 and 2004, 69 consecutive patients (32 males; mean age, 32 ± 9 years) who had undergone a first laparoscopic ileocecal resection for Crohn’ s disease were included in a prospective study. Twenty-one patients (30 percent) were converted into laparotomy. Possible factors for conversion were analyzed by both univariate and multivariate analyses. RESULTS: No patient died. Four patients (9 percent; 2 in each group) required five reoperations because of intraperitoneal hemorrhage (n = 1), anastomotic fistula (n = 3), and small bowel obstruction (n = 1). Mean hospital stay was significantly increased in converted compared with laparoscopic patients (9 ± 4 vs. 7 ± 3 days; P < 0.05). On univariate analysis, more than three episodes of acute flare of Crohn’ s disease (P = 0.02), male gender (P = 0.03), preoperative immunosuppressive drugs (P = 0.04), intra-abdominal abscess or fistula at the time of laparoscopy (P = 0.02), and resection of other intestinal segment (P = 0.02) were factors that predicted conversion. On multivariate analysis, recurrent medical episodes of Crohn’ s disease (odds ratio, 2; 95 percent confidence interval, 1-4), and intra-abdominal abscess or fistula at the time of laparoscopy (odds ratio, 15; 95 percent confidence interval, 4-78) were the two independent risk factors for conversion. CONCLUSIONS: This prospective study demonstrated that the severity of the disease increased significantly the conversion rate of the first laparoscopic ileocecal resection. Knowledge of these risk factors for conversion could be helpful in preoperative preparation and counseling of patients.

Duodenal obstruction by Superior Mesenteric Artery (SMA) is a misdiagnosed vomiting syndrome in children. Several factors are involved, including rapid weight loss, rapid statural growth without weight augmentation. Diagnosis is suspected when an improvement is achieved by ventral decubitus and it is confirmed by plain films of the abdomen, GI study with barium and echography, measuring the aortomesenteric angle (inferior to 25-30°). Patients must at first be treated conservatively. Surgery is indicated for occlusive episodes with unsuccessful conservative therapy. The authors report 3 cases with different clinical presentation. However, all the patients presented important weight loss and vomiting.

AIM: To evaluate the antiviral potency of a new antihepatitis C virus(HCV) antiviral agent targeting the cellular autophagy machinery. METHODS: Non-infected liver slices, obtained from human liver resection and cut in 350 μm-thick slices(2.7 × 106 cells per slice) were infected with cell culture-grown HCV Con1b/C3 supernatant(multiplicity of infection = 0.1) cultivated for up to ten days. HCV infected slices were treated at day 4 post-infection with GNS-396 for 6 d at different concentrations. HCV replication was evaluated by strand-specific real-time quantitative reverse transcription- polymerase chain reaction. The infectivity titers of supernatants were evaluated by foci formation upon inoculation into naive Huh-7.5.1 cells. The cytotoxic effect of the drugs was evaluated by lactate dehydrogenase leakage assays. RESULTS: The antiviral efficacy of a new antiviral drug, GNS-396, an autophagy inhibitor, on HCV infection of adult human liver slices was evidenced in a dosedependent manner. At day 6 post-treatment, GNS-396 EC50 was 158 nmol/L without cytotoxic effect(compared to hydroxychloroquine EC50 = 1.17 μmol/L).CONCLUSION: Our results demonstrated that our ex vivo model is efficient for evaluation the potency of autophagy inhibitors, in particular a new quinoline derivative GNS-396 as antiviral could inhibit HCV infection in a dosedependent manner without cytotoxic effect.

Hemobilia is an uncommon and potential life-threatening condition mainly due to hepato-biliary tree traumatic or iatrogenic *** ruptured aneurysm of the hepatic artery is seldom *** report the case of an 89-year-old woman presenting with abdominal pain,jaundice and gastrointestinal bleeding,whose ultrasound and computed tomography revealed a non-traumatic,spontaneous aneurysm of the right hepatic *** oeso-gastro-duodenoscopy and colonoscopy did not reveal any bleeding at the ampulla of Vater,nor anywhere *** angiography confirmed the diagnosis of hepatic artery aneurysm and revealed a full hepatic artery originating from the superior mesenteric *** patient was successfully treated by selective embolization of *** discuss the etiologies of hemobilia and its treatment with selective embolization,which remains favored over surgical *** aneurysm of the hepatic artery is rare,especially without trauma,a high index of suspicion is needed in order to ensure appropriate treatment.

Objectives: Traditional methods for evaluating aesthetic perceptions of the teeth have involved panels of people observing photographs, and the person commenting on the appearance of the teeth generally is aware that his opinion of the dental appearance is being sought. The situation is artificial and may involve bias. We propose a novel method for evaluating the effect of dental imperfections on perceptions in which the participant is unaware of participating in a survey and in which his or her opinion is not sought. Rather, involvement in the study betrays the importance of dental aesthetics for the observer. Methods: Starting with a digitally manipulated photograph of a smiling young woman, two portrait photographs A and B were produced in which the only differences were in the dentition revealed by the smile. The two photographs were anonymously posted on an online dating service site covering two large cities in southwestern France. During a period of one month, all “hits” on each of the photographs and all attempts to make contact were counted. Results: There was no significant difference between the number of hits on each of the portraits A and B. On the other hand, the ratio of attempts to contact to hits showed a clear difference: the ratio was 4.8 times greater for Portrait A than for Portrait B

背景:二叶式主动脉瓣反流可由瓣膜自身缺损或主动脉根部结构复合体中的某一个或多个结构功能异常导致。因此,成功的修复需同时矫治各方面的问题。作者对在矫治二叶式主动脉瓣反流时同时矫治瓣膜缺陷和主动脉根部功能异常的经验进行了回顾。方法和结果:1996—2004年间,作者治疗了68例主动脉反流患者。30例患者为单一的主动脉反流,其余38例伴有升主动脉瘤。所有患者接受标化联合手术治疗,包括:正中裂切除或瓣叶折叠,连合下瓣环成形术,强固瓣膜游离缘,窦管结合部位折叠。
