Burden of pancreatitis and associated risk factors in China,1990 to 2019:a systematic analysis for the Global Burden of Disease Study 2019

Background: Pancreatitis is a common disease of the digestive system. Acute pancreatitis is one of the most common reasons for gastrointestinal hospital admission, and chronic pancreatitis significantly reduces quality of life. However, national epidemiological data on pancreatitis in China are lacking. This study aimed to quantify the disease burden of pancreatitis in China from 1990 to 2019.Methods: This study was based on the Global Burden of Disease Study 2019 dataset. Age-standardized rates of incidence (ASIR), prevalence (ASPR), mortality (ASMR), and disability-adjusted life years (DALYs) were used to describe the disease burden of pancreatitis, and estimated annual percentage change (EAPC) was used to indicate the average change in age-standardized rates. We also described the trend of pancreatitis-related mortality and DALYs, which are attributable to alcohol use by age and sex.Results: From 1990 to 2019, the ASIR, ASPR, ASMR, and age-standardized DALYs of pancreatitis in China decreased by 10.90, 1.50, 0.49, and 15.54 per 100,000, respectively, with EAPCs of -1.35 (95% uncertainty interval [UI]: -1.67, -1.02) and -0.37 (95% UI: -0.43, -0.31), -2.01 (95% UI: -2.07, -1.94) and -2.32 (95% UI: -2.37, -2.28), respectively. Recently, the numbers of incident and prevalent cases have risen, with estimates of 380,018 (95% UI: 308,669-462,767) and 493,765 (95% UI: 416,705-578,675), respectively, in 2019. Among men, the disease burden of pancreatitis was more severe than among women, and with variances in the distribution among different age groups. Age-standardized DALYs caused by alcohol-related pancreatitis have gradually worsened in the past decade, accounting for 34.09% of the total in 2019.Conclusions: The disease burden of pancreatitis in China has declined in the past 30 years, but the exacerbation of population aging poses a challenge to prevention and control of pancreatitis. Alcohol use has gradually become an important factor in the disease burden of pancreatitis in recent years.

Background:Gallbladder and biliary tract cancer(GBTC)has greatly damaged the health of patients and is accompanied by a dismal prognosis.The worldwide distribution of GBTC shows extensive variance and the updated data in China is lacking.This study was to determine the current status,trends,and predictions in the burden of GBTC over the past 30 years in China.Methods:This was a descriptive,epidemiological,secondary analysis of the Global Burden of Disease,Injuries,and Risk Factor Study 2019 data.Data including incidence,prevalence,mortality,and disability-adjusted life years(DALYs)of GBTC in China by year,age,and sex were assessed.Joinpoint regression analysis was conducted to evaluate trends of disease burden due to GBTC from 1990 to 2019.Nordpred age-period-cohort analysis was applied for the projection of mortality and incidence due to GBTC from 2019 to 2044.Results:Nationally,there were 38,634(95%uncertainty interval[UI]:27,350-46,512)new cases and 47,278(95%UI:32,889-57,229)patients due to GBTC,causing 34,462(95%UI:25,220-41,231)deaths,and 763,584(95%UI:566,755-920,493)DALYs in 2019.Both cases and rates of burden owing to GBTC were heavier among males and at old age.From 1990 to 2019,the age-standardized rates of incidence,prevalence,mortality,and DALYs of GBTC generally increased from 1990 to 2019,with average annual percentage change at 0.8%(95%confidential interval[CI]:0.6-1.0%),1.3%(95%CI:1.1-1.5%),0.4%(95%CI:0.2-0.6%),and 0.2%(95%CI:0.1-0.4%),respectively.Even though the age-standardized incidence rate and agestandardized mortality rate in both sexes were predicted to decline gradually from 2019 to 2044,the number of new cases and deaths were expected to grow steadily.Conclusions:GBTC is becoming a major health burden in China,particularly among males and older individuals.Given the aging population and increasing burden,effective strategies and measurements are urged to prevent or reduce the number of new cases and deaths of GBTC.

Inflammation-based different association between anatomical severity of coronary artery disease and lung cancer

BACKGROUND Coronary artery disease(CAD)is associated with cancer.The role of inflammation in the association of CAD with cancer remains unclear.The study investigated whether inflammation could impact the relationship between CAD and lung cancer.METHODS The study involved 96 newly diagnosed lung cancer patients without receiving anti-cancer therapy and 288 matched non-cancer patients.All the patients underwent coronary angiography and were free from previous percutaneous coronary inter-vention or coronary artery bypass grafting.SYNTAX score(SXscore)were used to assess severity of CAD.High SXscore(SXhigh)grade was defined as SXscore>16(highest quartile).Neutrophil-to-lymphocyte ratio(NLR)served as an inflammatory biomarker.NLR-high grade referred to NLR>2.221(median).RESULTS Among 384 study patients,380 patients(98.96%)had NLR value(median:2.221,interquartile range:1.637-3.040).Com-pared to non-cancer patients,lung cancer patients had higher rate of SXhigh among total study patients(P=0.014)and among pat-ients with NLR-high(P=0.006),but had not significantly higher rate of SXhigh among patients with NLR-low(P=0.839).Multiv-ariate logistic regression analysis showed that SXhigh was associated with lung cancer[odds ratio(OR)=1.834,95%CI:1.063-3.162,P=0.029].Subgroup analysis showed that SXhigh was associated with lung cancer among patients with NLR-high(OR=2.801,95%CI:1.355-5.794,P=0.005),however,the association between SXhigh and lung cancer was not significant among pati-ents with NLR-low(OR=0.897,95%CI:0.346-2.232,P=0.823).CONCLUSIONS Inflammation could lead different association between anatomical severity of CAD and lung cancer.Severity of CAD was significantly associated with increased risk of lung cancer among patients with high inflammation rather than am-ong patients with low inflammation.

Oral administration of asparagine and 3-indolepropionic acid prolongs survival time of rats with traumatic colon injury

Background: Traumatic colon injury(TCI) is a common disease during wartime. Prolongation of posttraumatic survival time is an effective approach to patient outcome improvement. However, there is a lack of basic research in this field.This study aimed to elucidate the mechanisms underlying TCI progression and to develop novel regimens to buy time for TCI patients on the battlefield.Methods: A total of 669 Sprague–Dawley rats were used in this study. Surgical colon incision was performed to generate the TCI rat model. The landscape of colon microbiota compositions was depicted using 16S rRNA sequencing and metabolites in the intestinal contents were detected by metabolomics profiling. The signaling transduction in the intestinal epithelium was investigated using antibody microarrays and Western blotting. The enzyme-linked immunosorbent assay(ELISA) was conducted to measure the levels of interleukin-6 and tumor necrosis factor-α in intestines and plasma for the detection of inflammatory responses. Diamine oxidase, D-lactate and endotoxin in plasma and protein expression of zonula occludens 1 and occludin were selected as the indicators of intestinal barrier permeability. To investigate alterations of microbiota symbiosis, the relative abundances of specific bacterial genera were detected using quantitative real-time PCR(qRT-PCR).Results: As a type of lethal injury, TCI induced acute disruption of intestinal homeostasis, characterized by inflammatory responses, intestinal barrier hyperpermeability and microbiota dysbiosis(P<0.05). Significant alterations in bacterial metabolic patterns were detected with decreases in many metabolites. After a series of screenings,we found that oral administration of asparagine(Asn) and 3-indolepropionic acid(IPA) effectively prolonged posttraumatic survival time [Asn plus IPA vs. Vehicle: hazard ratio(HR)=0.105, 95%CI 0.031–0.356, P=0.0003] and restored intestinal homeostasis in TCI rats(P<0.05). Current applications for magnetic resonance-guided focused ultrasound in the treatment of Parkinson’s disease

Magnetic resonance-guided focused ultrasound(MRgFUS)is a novel and minimally invasive technology.Since the US Food and Drug Administration approved unilateral ventral intermediate nucleus-MRgFUS for medication-refractory essential tremor in 2016,studies on new indications,such as Parkinson’s disease(PD),psychiatric diseases,and brain tumors,have been on the rise,and MRgFUS has become a promising method to treat such neurological diseases.Currently,as the second most common degenerative disease,PD is a research hotspot in the field of MRgFUS.The actions of MRgFUS on the brain range from thermoablation,blood-brain barrier(BBB)opening,to neuromodulation.Intensity is a key determinant of ultrasound actions.Generally,high intensity can be used to precisely thermoablate brain targets,whereas low intensity can be used as molecular therapies to modulate neuronal activity and open the BBB in conjunction with injected microbubbles.Here,we aimed to summarize advances in the application of MRgFUS for the treatment of PD,with a focus on thermal ablation,BBB opening,and neuromodulation,in the hope of informing clinicians of current applications.

The development of premature coronary artery disease(PCAD)is dependent on both genetic predisposition and traditional risk factors.Strategies for unraveling the genetic basis of PCAD have evolved with the advent of modern technologies.Genome-wide association studies(GWASs)have identified a considerable number of common genetic variants that are associated with PCAD.Most of these genetic variants are attributable to lipid and blood pressure-related single-nucleotide polymorphisms(SNPs).The genetic variants that predispose individuals to developing PCAD may depend on race and ethnicity.Some characteristic genetic variants have been identified in chinese populations.Although translating this genetic knowledge into clinical applications is still challenging,these genetic variants can be used for CAD phenotype identification,genetic prediction and therapy.In this article we will provide a comprehensive review of genetic variants detected by GWASs that are predicted to contribute to the development of PCAD.We will highlight recent findings regarding CAD-related genetic variants in chinese populations and discuss the potential clinical utility of genetic variants for preventing and managing PCAD.

Effect of grain boundary segregation on aging resistance and mechanical properties of tetravalent element-doped 3Y-TZP ceramics for dental restoration

In the humid oral environment,3Y-TZP ceramics always suffer from low-temperature degradation(LTD)for a long time,which results in the degradation of mechanical properties and catastrophic failure.The low-temperature degradation(LTD)and mechanical properties of low-cost tetravalent(Ge^(4+),Ti^(4+))element-doped 3Y-TZP were investigated by analysing grain boundary segregation in samples with deferent contents.The results show that GeO_(2) is superior to TiO_(2) in limiting LTD but results in lower flexural strength and fracture toughness when the content is≥1.5 mol%.This dilemma can be improved by adding only 0.1%-0.5 wt%Al_(2)O_(3),and the flexural strength and fracture toughness of 0.25 wt% Al_(2)O_(3) zirconia are then increased to 898 MPa and 4.68 MPa·m^(1/2) compared with 1Ge-3Y,respectively.This work is expected to provide an effective reference for the development and application of budget dental materials.

Transformer-Based Deep Learning Network for Tooth Segmentation on Panoramic Radiographs

Panoramic radiographs can assist dentist to quickly evaluate patients’overall oral health status.The accurate detection and localization of tooth tissue on panoramic radiographs is the first step to identify pathology,and also plays a key role in an automatic diagnosis system.However,the evaluation of panoramic radiographs depends on the clinical experience and knowledge of dentist,while the interpretation of panoramic radiographs might lead misdiagnosis.Therefore,it is of great significance to use artificial intelligence to segment teeth on panoramic radiographs.In this study,SWin-Unet,the transformer-based Ushaped encoder-decoder architecture with skip-connections,is introduced to perform panoramic radiograph segmentation.To well evaluate the tooth segmentation performance of SWin-Unet,the plaGH-BH dataset is introduced for the research purpose.The performance is evaluated by F1 score,mean intersection and Union(IoU)and Acc,Compared with U-Net,Link-Net and FPN baselines,SWin-Unet performs much better in plaGH-BH tooth segmentation dataset.These results indicate that SWin-Unet is more feasible on panoramic radiograph segmentation,and is valuable for the potential clinical application.

Mitochondrial quality control mechanisms as molecular targets in cardiac ischemia-reperfusion injury

Mitochondrial damage is a critical contributor to cardiac ischemia/reperfusion(I/R)injury.Mitochondrial quality control(MQC)mechanisms,a series of adaptive responses that preserve mitochondrial structure and function,ensure cardiomyocyte survival and cardiac function after I/R injury.MQC includes mitochondrial fission,mitochondrial fusion,mitophagy and mitochondria-dependent cell death.The interplay among these responses is linked to pathological changes such as redox imbalance,calcium overload,energy metabolism disorder,signal transduction arrest,the mitochondrial unfolded protein response and endoplasmic reticulum stress.Excessive mitochondrial fission is an early marker of mitochondrial damage and cardiomyocyte death.Reduced mitochondrial fusion has been observed in stressed cardiomyocytes and correlates with mitochondrial dysfunction and cardiac depression.Mitophagy allows autophagosomes to selectively degrade poorly structured mitochondria,thus maintaining mitochondrial network fitness.Nevertheless,abnormal mitophagy is maladaptive and has been linked to cell death.Although mitochondria serve as the fuel source of the heart by continuously producing adenosine triphosphate,they also stimulate cardiomyocyte death by inducing apoptosis or necroptosis in the reperfused myocardium.Therefore,defects in MQC may determine the fate of cardiomyocytes.In this review,we summarize the regulatory mechanisms and pathological effects of MQC in myocardial I/R injury,highlighting potential targets for the clinical management of reperfusion.

Marsupialization of conjunctival inclusion cyst in Stevens-Johnson syndrome

1.Background Conjunctival inclusion cyst is formed by hyperplasia and degeneration of epithelial cells implanted in subconjunctival area,which is common after ocular trauma and eye surgery.1 It can also be found in Stevens-Johnson syndrome(SJS),which results in extensive and severe symblepharon and the formation of cysts by encapsulating residual conjunctival epithelium on the fornix.2 The previous treatments of conjunctival inclusion cyst include complete excision surgically,cyst aspiration,foam sclerotherapy after cyst aspiration,3 and fornix reconstruction with cyst marsupialization,4 which owned many advantages but a similar surgery was only presented in a few case reports.In our clinical practice,the cyst wall was used for marsupialization in four patients suffering from both large conjunctival inclusion cyst and severe symblepharon.Owing to the difficulty of the treatment of SJS,the poor therapeutic effect of the traditional method,and the small number of articles on fornix reconstruction with conjunctival inclusion cyst marsupialization,such cases are summarized in the present study to introduce the clinical application of this operation for reference.
