Background:Emergency Operations center(EOC)is a place to provide response to public health emergencies.chinese center for disease control and prevention(China CDC)'s EOC was officially established in 2016,which has been the core department for the public health emergencies and risk response.In recent years,we have been continuously improving the function of EOC through many incidents.In the study,we hope to share the construction status,operation management experience of China CDC's EOC and the response process in the human avian influenza A(H7N9)outbreak.Main text:The China CDC's EOC mainly focus on building the five core elements including sites/places and facilities,information and data,plans and procedures,training and exercises,and logistics.Based on summarizing previous emergency respons巳the China CDC's EOC established its own incident management and the standardized response procedures.The event-specific data,context-specific data and event management data could be obtained through various source.The logistics department of the EOC also provides comprehensive support.The well-trained staff is another necessary conditions for its operation.Through sharing the response process of H7N9 outbreak,it further explains the EOC's functions in the five phases of outbreak response,such as the formulation of the incident response framework,monitoring,personnel dispatch and resource mobilization.Conclusions:The EOC con tributes to faster and more efficient responses during emergencies which en able a greater reduction in morbidity and mortality.Compared with the traditional incident response process,under the command and coordination of China CDC's EOC,each group involved in the response has a clearer goal,responsibilities and tasks at each stage.Meanwhile,each group also gave full play to its own expertise and advantages.As a whole,incident response tended to be more specialized and precise,which generally improves the efficiency of incident response.However,different countries and regions have different response proc

Background: With the promotion of national control programs on parasitic and tropical diseases in China,the National Institute of Parasitic diseases(NIPD),chinese center for disease control and prevention has gained significant experience in the global health arena through international cooperation over the last seven decades allowing a multilateral impact in the elimination of major endemic diseases.Methods:: The achievements of NIPD since 1950 has been analyzed with emphasis on the various stages that started with research and control of the endemic parasitic and other tropical diseases at the national level and progressed via international cooperation into a global presence.Results: The major achievements contributed by NIPD consist of(i)improving technical capability;(ii)promoting control and elimination of parasitic and tropical diseases;(iii)participating in global health governance and cooperation;and(iv)developing a cooperation model for technical assistance and global public health development.It is expected that NIPD’s experience of international cooperation will be essential for the dissemination of China's successful experience in global health governance,emergency response and development,with focus on malaria and neglected tropical diseases such as schistosomiasis,soil-borne and food-borne helminthiases and echinococcosis.Conclusions: NIPD’s new tasks will not only continue to promote national control of endemic parasitic infections and disease elimination programs in China,but also play a leading role in global health and disease elimination programs in the future.

China faces the greatest challenge from stroke in the world.The death rate for cerebrovascular diseases in China was 149.49 per 100000,accounting for 1.57 million deaths in 2018.It ranked third among the leading causes of death behind malignant tumours and heart disease.The age-standardised prevalence and incidence of stroke in 2013 were 1114.8 per 100000 population and 246.8 per 100000 person-years,respectively.According to the Global Burden of disease Study 2017,the years of life lost(YLLs)per 100000 population for stroke increased by 14.6%;YLLs due to stroke rose from third highest among all causes in 1990 to the highest in 2017.The absolute numbers and rates per 100000 population for all-age disability-adjusted life years(DALYs)for stroke increased substantially between 1990 and 2017,and stroke was the leading cause of all-age DALYs in 2017.The main contributors to cerebrovascular diseases include behavioural risk factors(smoking and alcohol use)and pre-existing conditions(hypertension,diabetes mellitus,dyslipidaemia and atrial fibrillation(AF)).The most prevalent risk factors among stroke survivors were hypertension(63.0%-84.2%)and smoking(31.7%-47.6%).The least prevalent was AF(2.7%-7.4%).The prevalences for major risk factors for stroke are high and most have increased over time.Based on the latest national epidemiological data,26.6%of adults aged≥15 years(307.6 million adults)smoked tobacco products.For those aged≥18 years,age-adjusted prevalence of hypertension was 25.2%;adjusted prevalence of hypercholesterolaemia was 5.8%;and the standardised prevalence of diabetes was 10.9%.For those aged≥40 years,the standardised prevalence of AF was 2.31%.Data from the Hospital Quality Monitoring System showed that 3010204 inpatients with stroke were admitted to 1853 tertiary care hospitals during 2018.Of those,2466785(81.9%)were ischaemic strokes(ISs);447609(14.9%)were intracerebral haemorrhages(ICHs);and 95810(3.2%)were subarachnoid haemorrhages(SAHs).The average age of patients admitted was 66 years old,and nearl

Temporal and Spatial Trends in HIV Positivity Rate for VCT Clinics—China,2015–2022

Introduction:Human immunodeficiency virus(HIV)voluntary counseling and testing(VCT)clinics play a critical role in identifying and diagnosing HIV cases.This study aimed to describe the trend of HIV positivity rate(HPR)among chinese VCT clinics between 2015 and 2022.Methods:This study utilized data from the China Information System for disease control and prevention to analyze the trend in the HPR for VCT clinics from 2015 to 2022.The HPR was calculated by dividing the number of newly-reported HIV cases by the number of HIV tests,multiplied by 100%.To identify temporal and spatial trends in the HPR,we employed joinpoint regression analysis and the Getis-Ord hotspot analysis.Results:From 2015 to 2022,VCT clinics in China performed a total of 22,075,386 HIV tests,leading to the identification of 260,353 HIV cases,resulting in a HPR of 1.18%.The HPR consistently declined over the study period,with an average annual percent change(AAPC)of−7.5%(95%confidence interval:−12.6%,−2.2%,P<0.05).The number of HPR hotspots also decreased from 41 in 2015 to 23 in 2022.These HPR hotspots were primarily located in Yunnan,Sichuan,Guangdong,and Guangxi provincial-level administrative divisions(PLADs).Among the 31 PLADs,16 showed a significant decrease in HPR during the study period(AAPC<0,PAAPC<0.05).Conclusions:VCT clinics in China have played a significant role in identifying HIV cases.The declining HPR observed in these clinics may indicates the progress has been made in some degree in mitigating HIV among high-risk populations.Therefore,it is crucial to further improving the utilization of VCT clinics for HIV testing.

Anti-OX40 Antibody Combined with HBc VLPs Delays Tumor Growth in a Mouse Colon Cancer Model

Objective Combination immunotherapy strategies targeting OX40,a co-stimulatory molecule that can enhance antitumor immunity by modulating the proliferation,differentiation,and effector function of tumor-infiltrating T cells,have attracted much attention for their excellent therapeutic effects.In this study,we aimed to evaluate the antitumor efficacy of combined anti-OX40 and hepatitis B core viruslike particles(HBc VLPs)therapy using a mouse colon cancer model.Methods Humanized B-h OX40 mice were injected subcutaneously with MC38 colon tumor cells and treated with HBc VLPs+anti-h OX40 antibody.Tumor growth was monitored.Flow cytometric analysis was performed to evaluate the populations of T cell subsets in the tumors.Results The combination of anti-OX40 with HBc VLPs resulted in a significant delay in tumor growth,suggesting that a potent antitumor immunity was induced by the combination therapy.Further studies revealed that HBc VLPs+anti-OX40 treatment induced a significant increase in effector T cells(Teffs)and a significant decrease in regulatory T cells(Tregs)in the tumor microenvironment(TME),which accounted for the synergistic antitumor effect of anti-OX40 in combination with HBc VLPs.Conclusion Combination therapy of anti-h OX40 and HBc VLPs provides synergistic antitumor activity in colon cancer-bearing mice,which may represent a potential design strategy for cancer immunotherapy.

What is already known about this topic?The inclusion of meningococcal vaccines in the National Immunization Program(NIP)over several years has significantly reduced the incidence of meningococcal meningitis in China to historic lows.Worldwide,there has been a diversification of meningococcal serogroups,leading to a shift in dominant serogroups in China from serogroup A to serogroups C and B,accompanied by a rise in reports of serogroups Y and W.What is added by this report?An outbreak of serogroup Y Neisseria meningitidis(Nm)in a secondary vocational school involved a single confirmed severe case and 24 individuals with laboratory-confirmed Nm carriage.Epidemiological investigation revealed that the outbreak was localized to the classroom of the confirmed case.Prolonged close contact within a confined space was identified as a significant risk factor for Nm transmission.The genotype sequence identified was type 1655(ST-1655),which is categorized under clonal complex 23(CC-23)and bears resemblance to 8 previously confirmed cases of serogroup Y meningococcal meningitis within Guangdong Province.This suggests that serogroup Y infections continue to sporadically emerge and have become prevalent strains.What are the implications for public health practice?This outbreak underscores the critical need to enhance surveillance of meningococcal serogroups and population carrier,and advocate for vaccination with MenY-containing vaccines.

What is already known about this topic?Vitamin A deficiency(VAD)is a leading global nutritional concern,ranking among the top four major nutritional deficiencies worldwide.The prevalence of VAD is unevenly distributed across various regions,both within China and globally.What is added by this report?The report adds valuable insights into the vitamin A nutritional status of rural students aged 6–17 years who participated in the Nutrition Improvement Programme for Rural Compulsory Education Students(NIPRCES).Over the decade from 2012 to 2021,there was a modest improvement in vitamin A status.The prevalence of VAD and sub-clinical VAD(SVAD)declined as the students aged.Throughout the majority of the survey years,the incidence of VAD was higher among males and western regions compared to females and central regions,respectively.What are the implications for public health practice?A comprehensive approach,incorporating dietary diversification,nutrition education,and food fortification,should be implemented to prevent VAD and SVAD especially in males,younger children and children in western areas.

Background Brucellosis is a severe zoonotic disease that is often overlooked,particularly in impoverished countries.Timely identification of focal complications in brucellosis is crucial for improving treatment outcomes.However,there is currently a lack of established indicators or biomarkers for diagnosing these complications.Therefore,this study aimed to investigate potential warning signs of focal complications in human brucellosis,with the goal of providing practical parameters for clinicians to aid in the diagnosis and management of patients.Methods A multi-center cross-sectional study was conducted in China from December 2019 to August 2021.The study aimed to investigate the clinical characteristics and complications of patients with brucellosis using a ques-tionnaire survey and medical record system.The presence of warning signs for complications was assessed using univariate and multivariate logistic regression models.Receiver operating characteristic(ROC)curves and the area under the curve(AUC)were used for variable screening and model evaluation.Results A total of 880 participants diagnosed with human brucellosis were enrolled.The median age of the patients Was 50 years[interquartile range(IQR):41.5-58.0],and 54.8%had complications.The most common organ system affected by complications was the osteoarticular system(43.1%),with peripheral arthritis(30.0%),spondylitis(16.6%),paravertebral abscess(5.0%),and sacroilitis(2.7%)being the most prevalent.Complications in other organ systems included the genitourinary system(4.7%),respiratory system(4.7%),and hematologic system(4.6%).Several fac-tors were found to be associated with focal brucellosis.These factors included a long delay in diagnosis odds ratio(OR)=3.963,95%confidence interval(CI):1.906-8.238 for>90 daysl,the presence of underlying disease(OR=1.675,95%CI:1.176-2.384),arthralgia(OR=3.197,95%CI:1.986-5.148),eye bulging pain(OR=3.482,95%CI:1.349-8.988),C-reactive protein(CRP)>10 mg/L(OR=1.910,95%CI:1.310-2.784)and erythrocyte sedimentation rate(ESR)el
