Use of Evidence-Based Pharmacotherapy for Secondary Prevention of Coronary Heart Disease:A chinese medicine hospital Versus A General hospital

Objective： To determine differences in adherence to secondary prevention guidelines （pharmacological interventions） among coronary heart disease （CHD） patients between a chinese medicine （CM） hospital and a general hospital in a chinese city. Methods： Medical records of 200 patients consecutively discharged from the CM hospital and the general hospital for CHD were reviewed to determine the proportions of eligible patients who received antiplatelet agents, 13-blockers, angiotensin-converting enzyme （ACE） inhibitors or angiotensin receptor blockers （ARBs） and statins at discharge. The effects of patient characteristics and hospital type on the use of these medicines were estimated using logistic regression models. Results： Patients discharged from the CM hospitals were older; more likely females; had greater history of hyperlipidemia, cerebrovascular diseases and less smoker （P〈0.01 or P〈0.05）. They were less likely to receive coronary angiography and percutaneous coronary intervention, and had a longer length of stay than those discharged from the general hospital （P〈0.01 or P〈0.05）. There were no significant differences in antiplatelet agents （96% vs. 100%, P=0.121） or statins （97.9% vs. 100%, P=0.149） use between the CM hospital and the general hospital. In multivadable analyses that adjusted for patient characteristics and hospital type, there was no significant difference in use of 13 -blockers between the CM hospital and the general hospital. In contrast, patients discharged from the CM hospital were less likely to receive ACE inhibitors/ARBs compared with those discharged from the general hospital （odds ratio： 0.3, 95% confidence interval： 0.105-O.854）. Conclusion： In this study, the CM hospital provides the same quality of care in CHD for prescribing evidence-based medications at discharge compared with another general hospital except for ACE inhibitors/ARBs use.

Acute Myocardial Infarction in chinese medicine hospitals in China from 2006 to 2013: An Analysis of 2311 Patients from hospital Data

Scraping therapy for knee osteoarthritis: a systematic review and Meta-analysis

OBJECTIVE: To systematically evaluate the efficacy of scraping therapy on knee osteoarthritis(KOA). METHODS: A computerized search was conducted in several databases, including PubMed, Web of Science, Cochrane Library, China National Knowledge Infrastructure Database, China Science and Technology Journal Database,Wanfang, and chinese BioMedical Literature Database,from the establishment date of these databases to April 9,2023. Randomized controlled trials(RCTs) on scraping therapy for KOA were collected in accordance with the “Participants-Intervention-Control-Outcome-Study design”criteria. The methodological evaluation of the included studies was carried out using RoB 2.0, and Meta-analysis was conducted using RevMan. The Grades of Recommendations Assessment, Development and Evaluation(GRADE System) was applied to grade the quality of evidence. RESULTS: A total of 12 RCTs with 997 subjects were included in the Meta-analysis. Results indicated that compared with the control group, the scraping therapy treatment group exhibited increased Lysholm scores [mean difference(MD) = 4.96, 95% confidence intervals(CI)(3.18, 6.74), P hospital for Special Surgery scores [MD = 8.35, 95% CI(3.92, 12.78), P = 0.0002] and decreased visual analog scale scores [MD =-2.11, 95% CI(-3.79,-0.44), P = 0.01] and Western Ontario and McM aster University Osteoarthritis Index scores [MD =-6.77, 95% CI(-8.99,-4.56), P < 0.000 01)]. The quality of evidence obtained in this Meta-analysis was low according to the GRADE system. CONCLUSION: The available evidence suggests that scraping therapy may have certain clinical efficacy in the treatment of KOA. However, further high-quality RCTs are needed to confirm the findings.

OBJECTIVE: To investigate blood pressure rhythm(BPR)in Yin deficiency syndrome of hypertension(YDSH)patients and develop a random forest model for predicting ***: Our study was consistent with technical processes and specification for developing guidelines of Evidence-based chinese medicine clinical practice(T/CACM 1032-2017). We enrolled 234 patients who had been diagnosed with primary hypertension without antihypertensive medications prior to the enrollment. All participants were divided into Yin deficiency group(YX, n = 74) and non-Yin deficiency group(NYX, n = 160).Participants were professionally grouped by three experienced chief Traditional chinese medicine(TCM)physicians according to four examinations(i.e.,inspection, listening and smelling, inquiry and palpation).We collected data on 24 h ambulatory blood pressure monitoring(ABPM) and YDSH rating scale. We divided 24 h of a day into 12 two-hour periods [Chen-Shi(7:00-9:00), Si-Shi(9:00-11:00), Wu-Shi(11:00-13:00), Wei-Shi(13:00-15:00), Shen-Shi(15:00-17:00), You-Shi(17:00-19:00), Xu-Shi(19:00-21:00), Hai-Shi(21:00-23:00), ZiShi(23:00-1:00), Chou-Shi(1:00-3:00), Yin-Shi(3:00-5:00), Mao-Shi(5:00-7:00)] according to the theory of “midnight-midday ebb flow”. We used random forest to build the diagnostic model of YDSH, with whether it was Yin deficiency syndrome as the outcome. RESULTS: Compared with NYX group, YX group had more female participants with older age, lower waist circumference, body mass index(BMI), diastolic blood pressure(DBP), and smoking and drinking rate(all P < 0.05). The YDSH rating scores of YX group [28.5(21.0-36.0)] were significantly higher than NYX group [13.0(8.0-22.0)](P < 0.001), and the typical symptoms of YX group included vexing heat in the chest, palms and soles, dizziness, dry eyes, string-like and fine pulse, soreness and weakness of lumbus and knees, palpitations, reddened cheeks, and tinnitus(all P < 0.05). Luteolin promotes neuronogenesis in hippocampus of chronic unpredictable mild stress rats and primary hippocampus of fetal rats

OBJECTIVE: To investigate the effects of luteolin on chronic unpredictable mild stress(CUMS)-induced depressive rats and corticosterone(CORT)-induced depressive primary hippocampal neurons, and to elucidate the mechanism behind the ***: The antidepressant mechanism of luteolin was studied by using CUMS rat model and primary hippocampal neurons in fetal rats. In vivo, novelty suppressed feeding, open-field and sucrose preference tests as well as Morris water maze were evaluated. The content of brain derived neurotrophic factor(BDNF), 5-hydroxytryptamine(5-HT), norepinephrine(NE), and dopamine(DA) in serum were detected by enzyme-linked immunosorbent assay. The mechanisms of luteolin were explored based on neurotrophin and hippocampal neurogenesis, and proliferation. Survival of the septotemporal axis in hippocampus was assayed using the 5-bromo-2-deoxyuridine(BrdU), the expression of BDNF,neurotrophin-3(NT-3), and nerve growth factor(NGF) in hippocampus dentate gyrus region were measured by Western-blotting. In vitro, BDNF, NT-3, tropomyosin receptor kinase B(TrkB), and phosphorylated cyclic adenosine monophosphate responsive element binding protein(p-CREB) were detected through the high content analysis(HCA) to investigate neurotrophin and ***: Induction of CUMS in rats induced depressive symptoms, while luteolin significantly enhanced sucrose consumption, decreased feeding latency, increased locomotor activity, escape latency, distance of target quadrant and regulated the content of depressive-like biomarkers. Histology analysis revealed that luteolin increased the abundance of new born neurons that had been labeled with BrdU, BrdU + neuronal nuclear antigen,and BrdU + doublecortin in septo-temporal axis of S2(mid-septal) and T3(mid-temporal). hospital-based health technology assessment(HB-HTA)is highly valuable in the application of hospital medical technology,cost control,improvement of medical quality,and protection of medical safety,and its use is increasing daily ***,in China,HB-HTA is still in its infancy,and it is yet to be adopted in traditional chinese medicine(TCM).Therefore,this article introduces the application and development of HB-HTA,as well as discusses the current situation of TCM and related future development strategies to provide ideas and references for the development of HB-HTA in *** conclude and recommend that all major stakeholders,including the state,health departments,and hospital leaders,support and promote the exploration and development of HB-HTA in TCM hospitals and integrative medicine *** also suggest the utilization of existing talents and introduction of other necessary talents to promote better and faster organizational development,as well as suggest the formation of a national and internationally recognized HB-HTA guidance manual and toolkit for TCM hospitals based on both the national HTA guidelines and the evaluation of TCM hospitals pilot ***,we recommend that HB-HTA pilot study should be executed with chinese patent medicine as the starting point to form the implementation process and framework in the TCM arena.

OBJECTIVE: To evaluate the protective effects of Chang'an decoction(肠安方, CAD) on colitis, and investigate the potential mechanisms underlying these effects from the perspectives of endoplasmic reticulum(ER) stress induced by mitofusin 2(MFN2). METHODS: The composition of CAD was identified by liquid chromatography-mass spectrometry technology. A mice model of dextran sulfate sodium(DSS) induced colitis was established and therapeutic effects of CAD were determined by detecting body weight, disease activity index, colon length and histopathological changes. Then, the expression levels of MFN2, ER stress markers and Nucleotide-binding domain and leucine-rich repeat protein3(NLRP3) relevant proteins were detected by polymerase chain reaction(PCR), Western blot, immunohistochemistry and immunofluorescence staining. Subsequently, knockdown and overexpression cell model were constructed to further investigate the underlying mechanism of MFN2 mediating ER stress and energy metabolism by PCR, Western blot, electron microscopy and reactive oxygen species(ROS) staining. Finally, inflammatory indicator and tight junction proteins were measured by PCR and immunofluorescence staining to evaluate the protective effects of CAD. RESULTS: Results showed that the indispensable regulatory role of MFN2 in mediating ER stress and mitochondrial damage was involved in the protective effects of CAD on colitis in mice fed with DSS. Network pharmacology analysis also revealed CAD may play a protective effect on colitis by affecting mitochondrial function. In addition, our data also suggested a causative role for MFN2 in the development of inflammatory responses and energy metabolic alterations by constructing a knockdown and overexpression cell model whereby alter proper ER-mitochondria interaction in Caco-2 cells.
