OBJECTIVES To investigate the value of CCKBR^(fl/fl)villin-Cre mice as a mouse model of salt-sensitive hypertension(SSH).METHODS In the first part,2-month-old CCKBR^(fl/fl)villin-Cre mice(CKO)and control CCKBR^(fl/fl)mice(WT)were fed with normal diet(0.4%NaCl)or high salt diet(4%NaCl),separately for 6 weeks.In the rescue study,one week of hydrochlorothiazide or saline injection were treated with the CKO mice fed high salt diet.The blood pressure,biochemical indexes,and the expression of small intestinal sodium transporters(NHE3,NKCC1,eNaC)was detected.The organ injury markers(MMP2/MMP9)and the histopathological changes of kidneys were observed,whereas the changes of duodenal sodium absorption were detected by small intestinal perfusion in vivo.RESULTS The CCKBR^(fl/fl)villin-Cre mice with high salt intake exhibited high blood pressure,increased duodenal sodium absorption and urinary sodium excretion,and with renal injury.The protein expression of NHE3,NKCC1 and eNaC were also significant increase in the intestine of CKO-HS mice.Treatment with hydrochlorothiazide remarkably attenuated the elevated blood pressure by high salt absorption in the CCKBR^(fl/fl)villin-Cre mice,but no significant histopathological changes were observed.CONCLUSIONS These results support a crucial role of intestinal Cckbr deficiency on SSH development and the diuretic antihypertension effect in CCKBR^(fl/fl)villin-Cre mice.The CCKBR^(fl/fl)villin-Cre mice with the high salt intake may serve as a stable model of salt-sensitive hypertensive induced by sodium overloading.

Background:Femoral interlocking intramedullary(IM)nailing fixation is an effective method for the treatment of femoral shaft fractures.Aseptic nonunion of femoral shaft fracture after IM nailing is uncommon.Currently,the treatment for aseptic femoral shaft nonunion is controversial.The aim of this study was to investigate the clinical effect of augmentative antirotational plating plus decortication and autogenic bone grafting for aseptic femoral shaft nonunion after IM nailing failure.Methods:A retrospective study was conducted on 25 cases of aseptic femoral shaft fracture nonunion treated with IM nailing from January 2015 to August 2019.All patients were treated by leaving the nail in situ,debridement of nonunionsites,decortication,autogenous iliac bone grafting,and augmentative antirotational plating fixation.The time to fracture union and complications were recorded.Results:All patients were followed up for 12–18 months.The union rate after revision surgery was 100%.The average union time was 5.5months(range,4-10).Subjective pain symptoms had disappeared in all patients.There were no incision infections or internal fixator fatigue fractures.Average scores of the physical function and bodily pain components of the SF-36 were 95.5(range,91-98)and 94.1(range,90-97),respectively.No other obvious complications occurred postoperatively.Conclusion:Augmentative antirotational plating plus decortication and autogenic bone grafting is an excellent choice for treating femoral shaft fracture nonunion after IM nailing;this approach has an overall high union rate and few complications.

Trigeminal neuralgia refers to the kind of pain occurring in the distribution areas of the trigeminal nerves. The pain attacking periodically is so intense like either knife-cutting or electric stroke. However,no abnormality is found in the patients when the attack ceases. By means of acupuncture plus Chinese traditional drugs, the authors had treated 32cases of the disease from March of 1999 to March of 2003, and its effect is compared with that of carbamazepine in 29 cases. A report follows.

Purpose:Hemodynamically unstable patients with pelvic fractures still represent a challenge to trauma surgeons and have a very high mortality.This study was designed to explore the effect of the interventions of direct preperitoneal pelvic packing for the hemodynamically unstable pelvic fractures.Methods:This retrospective study enrolled 67 cases of severe pelvic fractures with unstable hemodynamics from October 2011 to December 2019.All patients presented in our emergency center and received preperitoneal pelvic packing were included in this study.The indication was persistent systolic blood pressure90 mmHg during initial resuscitation and after transfusion of two units of red blood cells.Patients with hemodynamic stability who need no preperitoneal pelvic packing to control bleeding were excluded.Their demographic characteristics,clinical features,laboratory results,therapeutic interventions,adverse events,and prognostic outcomes were collected from digital information system of electronic medical records.Statistics were described as mean±standard deviation or medium and analyzed using pair sample t-test or Mann-Whitney U-test.Results:The patients’average age was 41.6 years,ranging from 10 to 88 years.Among them,45 cases were male(67.2%)and 22 cases were female(32.8%).Significant difference was found regarding the systolic blood pressure(mmHg)in the emergency department(78.4±13.9)and after preperitoneal pelvic packing in the surgery intensive care unit(100.1±17.6)(pemergency department(median-10,interquartile range-14 to-8)(p<0.05).After preperitoneal pelvic packing,15 patients(22.4%)underwent pelvic angiography for persistent hypotension or suspected ongoing haemorrhage.The overall mortality rate was 29.5%(20 of 67).Conclusions:Preperitoneal pelvic packing,as a useful surgical technique,is less invasive and can be very efficient in early intra-pelvi

Purpose The increasing number of deaths due to road traffic accidents(RTAs)has attracted global attention.However,the influence of road types is rarely considered in the study of RTAs.This study evaluates the influence of different road types in RTAs in northern Guizhou to provide a basis for the formulation of evidence-based policies and measures.Methods We obtained the data from the Zunyi Traffic Management Data Platform for the years 2009–2018.The mortality rates of RTAs were calculated.Descriptive methods and Chi-square tests were used to analyze the characteristics of road traffic collisions on different road types.We also examined the associations between the mortality rate per 10,000 vehicles and the growth of per capital gross domestic product(GDP)with Spearman’s rank correlation analysis.According to the passing volume and the infrastructure,we defined different types of roads,like administrative road,functional road,general urban road and urban expressway.Results In 2012,the traffic mortality rate of administrative roads was 8.9 per 100,000 people,and the mortality rate of functional roads was 7.4 per 100,000 people,which decreased in 2018 to 6.1 deaths per 100,000 people and 5.2 deaths per 100,000 people,respectively.The mortality rate per 10,000 vehicles reached the highest level in 2011(28.8 per 10,000 vehicles and 22.5 per 10,000 vehicles on administrative and functional roads,respectively).The death rate of county roads was the highest among administrative roads(χ^(2)=17.389,p<0.05)and that of fourth-class roads was the highest among functional roads(χ^(2)=21.785,p<0.05).The mortality rate per 10,000 vehicles was negatively correlated with per capital GDP.Conclusion Although our research shows that RTAs in northern Guizhou have steadily declined in recent years,the range of decline is relatively small.Many measures and sustainable efforts are needed to control road traffic death and accelerate the progress in road traffic safety in northern Guizhou.

Dear Editor,A large number of COVID-19 patients experience body-specific conditions such as fatigue,sleep disturbances,anxiety,depression,and dyspnea after the disease has subsided,which is called“long COVID-19”.1 Studies have confirmed reduced microbial diversity,enrichment of opportunistic pathogens and depletion of beneficial symbiotic bacteria in patients with COVID-19.2,3 Long-term dysregulation of the microbiota has also been observed in recovered patients.4 Nevertheless,long-term fungal microbiota follow-up was still blank.

Vacuum sealing drainage (VSD) is frequently used in abdominal surgeries. However, relevant guidelines are rare. Chin ese Trauma Surge on Associati on orga nized a committee composed of 28 experts across china in July 2017, aiming to provide an evidence-based recommendation for the application of VSD in abdominal surgeries.Eleven questions regarding the use of VSD in abdominal surgeries were addressed:(1) which type of materials should be respectively chosen for the intraperitoneal cavity, retroperitoneal cavity and superficial incisions?(2) Can VSD be preventively used for a high-risk abdominal incision w让h primary suture?(3) Can VSD be used in severely contaminated/infected abdominal surgical sites?(4) Can VSD be used for temporary abdominal cavity closure under some special conditions such as severe abdominal trauma, infection, liver transplantation and intra-abdominal volume increment in abdominal compartment syndrome?(5) Can VSD be used in abdominal organ inflammation, injury, or postoperative drainage?(6) Can VSD be used in the treatment of intestinal fistula and pancreatic fistula?(7) Can VSD be used in the treatment of intra-abdominal and extra-peritoneal abscess?(8) Can VSD be used in the treatment of abdominal wall wounds, wound cavity, and defects?(9) Does VSD in crease the risk of bleeding?(10) Does VSD increase the risk of intestinal wail injury?(11) Does VSD increase the risk of peritoneal adhesion? Focusing on these questions, evidence-based recommendations were given accordingly. VSD was strongly recommended regarding the questions 2-4. Weak recommendations were made regarding questions 1 and 5-11. Proper use of VSD in abdominal surgeries can lower the risk of infection in abdominal incisions with primary suture, treat severely contaminated/infected surgical sites and facilitate temporary abdominal cavity closure.

Objective:To investigate the effect of a clinical nursing pathway and visual health education on patients with perianal abscesses was the objective of this study.Methods:Eighty patients with perianal abscesses undergoing in-patient surgical treatment(blinded for review)between December 2019 and November 2020 were divided randomly and evenly into a control group and an experimental group.The control group received routine nursing care,and the experimental group received visual health education nursing care.Digital pain scores,patient satisfaction scores,nursing quality scores,and scores measuring knowledge of perianal abscess disease for the two groups before and after nursing care were compared and analyzed.Results:The experimental group had visual analog scale(VAS)scores significantly lower than those of the control group(P<0.05).For total nursing satisfaction,the experimental group demonstrated a higher satisfaction rate(P<0.05).Clinical nursing quality scores were significantly higher in the experimental group than those in the control group(P<0.05).The perianal abscess disease-related knowledge scores after intervention were significantly higher in the experimental group than those in the control group(P<0.01).Conclusions:Visual health education nursing can help patients better understand and deal with perianal abscess disease.
