Surveillance of adverse events following H1N1/09 influenza immunisation in Victoria, Australia

BACKGROUND: The H1N1 pandemic in 2009 required a systematic coordinated response, which in Australia included a monovalent (H1N1/09) vaccine (Panvax?). SAEFVIC (Surveillance of Adverse Events Following Vaccination In the Community) is the Victorian, Australia state-based vaccine safety unit. The aim of the study was to review SAEFVIC reports of adverse events following immunisations (AEFI) temporally associated with H1N1/09 vaccines [monovalent and Trivalent Influenza Vaccines (TIV)]. METHODS: 1) Analysis of AEFI related to H1N1/09 vaccines reported to SAEFVIC from September 2009 to December 2010;2) Review of febrile convulsions (ICD-10 code R56.0), in children under 5 years of age presenting to the Royal Children’s hospital (RCH) Melbourne, Emergency Department between 1 March-30 April 2010. The presentation details and immunisation history were clarified by a telephone interview. RESULTS: 1) There were 659 reports of 749 adverse events following H1N1 vaccines. Among the TIV group, Fluvax had the most AEFI reported, with 77 per 100,000 doses distributed. Serious AEFI temporally associated with H1N1/09 vaccines included: 3 deaths, 2 anaphylactic reactions, and 3 GuillainBarre Syndrome. There were 7 reports of drug administration error;2) There were 179 presentations with fever and 67 reported febrile convulsions out of 11025 presentations (0.61%), 11 following H1N1 vaccines. Fluvax? was associated with 55% (6/11) reports. The mean onset time of AEFI was 13.2 hours post vaccination, and there was complete resolution of symptoms in allcases with no significant morbidity. CONCLUSION: Consistent with other Australian states in 2010, there was a TIV brand specific [Fluvax?] increase in febrile convulsions post vaccination. As a result this vaccine is no longer licensed for children <5 years of age. Comprehensive passive and active surveillance for AEFI needs to be incorporated into future pandemic planning.

Autism Spectrum Disorder（ASD） is a pervasive neurodevelopmental condition characterized by variable impairments in communication and social interaction as well as restricted interests and repetitive *** of presentation is a hallmark. With the aim of establishing whether cytokines have utility as potential biomarkers that may de？ne a subgroup of ASD,or function as an objective measure of response to treatment, this review summarizes the role of the immune system, discusses the relationship between the immune system, the brain, and behavior, and presents previouslyidenti？ed immune system abnormalities in ASD, speci？-cally addressing the role of cytokines in these *** roles and identi？cation of biomarkers are also addressed, particularly with respect to cytokine pro？les in ASD.

Purpose:The aim of this study was to define the predictive value of colonic manometry and contrast enema before cecostomy placement in children with defecation disorders. Methods:Medical records,contrast enema,and colonic manometry studies were reviewed for 32 children with defecation disorders who underwent cecostomy placement between 1999 and 2004. Diagnoses included idiopathic constipation (n=13),Hirschsprung’s disease (n=2),cerebral palsy (n=1),imperforate anus (n=6),spinal abnormality (n=6),and anal with spinal abnormality (n =4). Contrast enemas were evaluated for the presence of anatomic abnormalities and the degree of colonic dilatation. Colonic manometry was considered normal when high-amplitude propagating contractions (HAPC) occurred from proximal to distal colon. Clinical success was defined as normal defecation frequency with no or occasional fecal incontinence. Results:Colonic manometry was done on 32 and contrast enema on 24 patients before cecostomy. At follow-up,25 patients (78% ) fulfilled the success criteria. Absence of HAPC throughout the colon was related to unsuccessful outcome (P=0.03). Colonic response with normal HAPC after bisacodyl administration was predictive of success (P=0.03). Presence of colonic dilatation was not associated with colonic dysmotility. Conclusion:Colonic manometry is helpful in predicting the outcome after cecostomy. Patients with generalized colonic dysmotility are less likely to benefit from use of antegrade enemas via cecostomy. Normal colonic response to bisacodyl predicts favorable outcome.

Sequential testis sparing surgery of simultaneous bilateral testicular tumors with different cell types in a Chinese infant:an uncommon presentation

Dear Editor, Patients of bilateral pediatric testicular tumors and bilateral testicular teratoma or yolk sac tumor (YST) were rarely reported, and there was still no standard therapy. However, synchronous bilateral testicular tumors with different histology were a quite rare situation. We reported the first case of successful radical orchiectomy in the left scrotum and testicular sparing surgery in the right testis sequentially. No atrophy or residual tumor in the right testis and no recurrence in the left scrotum were found after 7 years of follow-up. Ethical approval was obtained from the Institutional Review Board of childrens hospital of Shanghai, which was affiliated to the Shanghai Jiao Tong University, School of Medicine, Shanghai, China. Informed consent was obtained from his parents.

目的研究脑膜炎大肠杆菌新基因cglE在细菌穿透血脑屏障中的作用。方法1)PCR扩增cglE基因片段,克隆到原核表达载体pET22b,转化大肠埃希菌BL21(DE3),IPTG诱导表达并对目的蛋白进行纯化及初步鉴定。2)应用生物信息学方法对cglE表达产物进行一级和二级结构预测。结果经Western-blot验证原核表达的CglE蛋白,SDS-PAGE电泳显示其分子量大小约50 000 Da。生物信息学分析表明,该基因与编码二氢硫辛酰胺脱氢酶(Dihydrolipoamide Ddhydrogenase,DLDH)的基因有同源性,与脑膜炎大肠杆菌侵袭素IbeA在二级结构上有相似性。结论建立稳定的原核表达系统并成功表达cglE基因产物,cglE基因可能与能量代谢及细菌穿透血脑屏障有关。目的蛋白CglE的表达及初步鉴定可为研究其功能及脑膜炎大肠杆菌的感染组学奠定了良好的基础。

Alimentary tract duplications in newborns and children: Diagnostic aspects and the role of laparoscopic treatment

Alimentary tract duplications are rare congenital lesions normally diagnosed in newborns and children that can occur anywhere from the mouth to the anus and have a reported incidence of approximately 1 in 4500 life births. Symptoms and clinical presentation vary greatly. The presentation varies according to age and location. The treatment finally is surgical; total resection when possible should be the aim of the intervention. In pediatric surgery minimally invasive surgical procedures became more and more important over the last decades. In consequence the operative procedure on alimentary tract duplications changed in this manner. We review on case reports and clinical reports on minimally invasive surgery in the treatment of alimentary tract duplications, determine the importance of minimally invasive techniques in the treatment of this rare entity and rule out that further studies in the field should be performed.
