Characterization of Extended Spectrum Beta-lactamase and Carbapenamase Producing Enterobacteriaceae Causing Urinary Tract Infection among Children in Kenya: A Cross-Sectional Study

Introduction: Enterobacteriaceae causing urinary tract infections (UTI) have developed resistance to the commonly used antibiotics due to emergence of Extended Spectrum Beta-Lactamases (ESBLs) and Carbapenamase producing Enterobactericeae which are a public health problem worldwide. This study aims to determine the prevalence and characterize ESBLs and carbapenamase producing Enterobactericeae. Method: A cross-sectional study was carried out in Gertrude’s Children’s hospital, Nairobi. 238 urine samples were collected from patients with urinary symptoms attending the outpatient department within the period 2020-2021. The urine were examined macroscopically and microscopically. Identification and antimicrobial susceptibility testing were done using VITEK® 2 Compact system (BioMérieux). Double disc synergy test and modified hodge tests were done as confirmatory tests for ESBLs and Carbapenamase phenotypes respectively. Polymerase Chain Reaction was used for the detection of blaCTX-M, blaTEM, blaSHV, blaKPC and blaOXA-48 genes. Results: From the 238 children sampled the prevalence of UTI caused by Enterobactericeae was 22.3%. The Enterobacteriaceae species isolated were Escherichia coli (84.9%), Klebsiella pneumoniae (5.66%), Proteus mirabillis (5.66%), Enterobacter aerogenes (1.89%) and Morganella morganii (1.89%). The isolated species were resistant to ampicillin. Meropenem had the highest susceptibility. Only E. coli species had the ESBLs (26.4%) and carbapenamase (1.9%) phenotypes. 100% had BlaCTX-M while 50% had blaTEM resistant gene. There was a significant association (p Conclusion: Ampicillin resistance resulted to use of alternative drugs and Meropenem was the drug of choice where increased resistance to the recommended drugs was noted. Further research on resistant genes is recommended.

From Fostering to Delusional Disorder of Filiation

Introduction: Fostering is a widespread practice in West Africa. It played an essential role in the circulation of children. The failure of its community regulation, not compensated by a legal framework, has made its practice precarious. Observation: A case study was conducted on Dior, who was hospitalised in a psychiatric ward for a delusional disorder of filiation. Dior was entrusted to her maternal grandmother and aunt at ages 2 and 9. During these fosterings, the place of the symbolic father was not substituted, causing a void that challenged her imagination. The outcome was an inscription in a genealogy where she had found herself with prestigious parents. Discussion: Fostering has strongly disrupted the family dynamics in Dior’s case. One of her options was to assume the paternal function, the use of which was perceived as a defiance or even a transgression. This same function would be a starting point of a psychosis whose filial character would only be an attempt at rectification where reality and imagination collide. Conclusion: Our study reveals the importance of stable parental figures at stake in fostering contexts, which constitute a risk factor for the development of a subsequent psychosis.

Vulvar Swelling in a 12-Year-Old Girl: An Early Manifestation of Crohn’s Disease

Vulvar Crohn’s disease (VCD) is a rare complication of Crohn’s disease, especially in pediatric population. An early diagnosis can result difficult if the complication does not present in conjunction with the classic gastrointestinal symptoms that characterize this disease. In this study we present the case of a 12-year-old girl whose initial symptom of Crohn’s disease was a symptomatic vulvar swelling promoted by a rectovaginal fistula. We also provide an overview of Crohn’s disease and the vulvar changes found in the course of this disease.

Genistein stimulates osteoblastic differentiation via p38 MAPK-Cbfa1 pathway in bone marrow culture

Crescentic glomerulonephritis in children:a single centre experience

Background:Crescentic glomerulonephritis(CsGN)is characterized by crescents in 50%or more of glomeruli and clinically by a sudden and progressive decline in renal ***:We evaluated the etiology,clinical features,prognostic factors and long-term outcome of *** January 2000 and December 2010,45 children(26 girls,19 boys)with biopsy-proven CsGN(>50%crescents)were investigated ***:The mean age of the patients was 130.86±33.77 *** mean duration of symptoms prior to diagnosis was 26±12 days(4-40 days).Most of the children had hypertension(62.2%),macroscopic hematuria(73.3%),oligoanuria(44.4%),edema(51.1%)and purpuric rash(40%)at *** final clinical status of the patients was complete remission(n=21),partial remission(n=5)or chronic kidney disease(n=19).Adverse outcomes were significantly associated with a long duration between the onset of symptoms and treatment(P=0.038),the presence of oligoanuria(P=0.006),a severe decreased glomerular filtration rate(GFR75%)(P=0.03),and the ratio of fi brous crescents(P=0.015).Conclusion:The outcome of CsGN in children continues to be poor,and it should be treated as a renal emergency.

Central corneal thickness measurements in premature infants

AIM:To evaluate the central corneal thickness(CCT)measurements of premature infantsMETHODS:The premature infants who were born between 25-34wk of gestational age(GA)were retrieved for the *** CCT measurements were performed within the first 2d of their life under topical anesthesia by using an ultrasonic ***:The mean CCT of 200 eyes was 647.79±63.94μ*** mean CCT of the right and left eyes were similar(647.30±64.72μm and 648.29±63.47μm consecutively).The mean CCT of the girls was 642.08±54.89μm and the boys was 653.07±71.14μm,with no statistically significant difference(P>0.05).But there was a negative correlation of the CCT with GA and birth weight(BW)(P<0.01).CONCLUSION:The CCT values of premature babies were negatively correlated with GA and BW.

Objective The COVID-19 pandemic had an impact on everyday life and in general,reduced the health-related quality of life(HRQoL)of *** this study,we assess the HRQoL of adolescents with type 1 diabetes(T1D)in Germany since the second wave of the COVID-19 pandemic by using self-report and parent-proxy reports,to identify risk factors,to compare to peers and to examine the agreement of HRQoL between parents and their *** A total of 445 adolescents(12–18 years)and 413 parents participated in an anonymous cross-sectional survey conducted at three German diabetes centres from January 2021 to June *** criteria were diabetes duration≥1year and *** HRQoL was assessed by using self-report and parent-proxy report versions of the KIDSCREEN-10 *** The majority of adolescents reported average(75.5%)*** 11.3%of teens reported high and 13.2%low ***’s female gender,older age,higher diabetes burden and parental depression symptoms contributed to lower self-reported HRQoL among *** parent-proxy reports,increasing diabetes burdens,parental depression symptoms,non-migrant status,high education and ketoacidosis contributed to lower scores on teen *** mean scores of the KIDSCREEN-10 index for adolescents did not differ from the German *** comparison to healthy peers during the first wave of the pandemic,adolescents in the current study reported higher *** overall teen-parent agreement was fair although parents reported significantly lower teen HRQoL than adolescents *** HRQoL of most adolescents with T1D during the COVID-19 pandemic was average with parents reporting significantly lower ***-reported and parent-proxy-reported HRQoL and the level of agreement due to different perspectives can provide important information for clinical care and intervention planning.

Osteoimmunomodulation was identified as a new and important strategy to enhance osteogenic differentiation together with other osteogenic ***,approaches regulating osteogenic differentiation and macrophage polarization to remodel an osteoinductive microenvironment are separate and ***,the design and synthesis of one biomaterial that couples the osteogenic performance and immunomodulatory ability is a major challenge for efficient bone *** this study,self-assembled iron-catechin nanoparticles(Fe-cat NPs)were designed based on the coordinated reaction between iron ions and catechin and synthesized via a facile one-pot ***,Fe-cat NPs show intracellular pH-responsive disassembly and release catechin molecules under the low pH of lysosomes after *** strategy delivers catechin intracellularly and then enhances the osteogenic differentiation while inhibits the adipogenic differentiation of human adipose-derived stem cells(hADSCs).More importantly,Fe-cat NPs remodel the osteogenic immune microenvironment by resisting inflammation and promoting M2 polarization of *** a promising metal-organic nanodrug,the intracellular pH-responsive Fe-cat NPs significantly enhance the therapeutic effect of bone regneration by orchestrating osteogenic differentiation and immunomodulation,which may have great potential in bone tissue engineering.

Dear Editor,Interleukin-25(IL-25,also called IL-17E)is an important IL-17 family member that helps initialize and regulate type 2 immunity,exerting an important influence on the pathogenesis of asthma(Corrigan et al.,2011;Barlow et al.,2012).Furthermore,it plays a vital role in initiating type 2 innate lymphoid cell activation and maintenance(Tong and Li,2018)and inducing Th2-type cytokine-mediated allergic airway eosinophilia(Morita et al.,2015).IL-25 expression increases in patients with asthma in different types of lung cells,including lung structural cells(such as endothelium and airway epithelium),eosinophils,T-cells,and mast *** allergen exposure,cells in the bronchial submucosa increase and secrete or respond to IL-25(Corrigan et al.,2011).Plasma IL-25 levels increase in patients with asthma and correlate with lung function(Cheng et al.,2014).The association between IL-25 cells and pulmonary function indicates that IL-25 could be used to monitor asthma *** some mouse models,epithelial IL-25 cytokines are closely related with a range of airway inflammation modalities,including type 2 cytokine production,airway eosinophil inflammation,mucinous epithelial metaplasia,and airway hyperresponsiveness(AHR)(Suzukawa et al.,2012).
