Trend and causes of neonatal mortality in a level Ⅲ children's hospital in shanghai: a 15-year retrospective study

Background To determine the trend and causes of neonatal mortality in a large level Ⅲ neonatal intensive care unit in shanghai during a 15-year period. Methods This is a retrospective, single-centered study. All neonates who died during the period from January 1, 1999 to December 31, 2013 at children's hospital of Fudan University were included. We extracted relevant clinical information from their medical records, analyzed neonatal mortality rate and the characteristics of these patients, and compared neonatal deaths between different periods and populations. Results Among a total of 50,957 admissions during the study period, there were 929 neonatal deaths. The neonatal mortality rate was 1.82%. Trends in neonatal mortality rate showed an increase in the period from 1.0% in 2003 to 2.2% in 2013. The main causes of neonatal mortality were complications of preterm birth (33.6%), congenital anomalies (21.3%), infections (12.6%), and birth asphyxia (9.1%). The proportions of complications of preterm birth (P s (P = 0.018) increased yearly, while the proportions of birth asphyxia (P s (P sed. Proportions of deaths caused by birth asphyxia (P = 0.005) and infections (P sidents. Conclusions Neonatal mortality rate increased from 2003 to 2013 in our study. The proportion of preterm infants in neonatal deaths also increased within the same period. Complications of preterm birth were the main cause of neonatal mortality and the percentage increased year by year. Neonates in the migrating population appeared to be at a higher risk of death during the neonatal period compared to those in the permanent residents, and efforts should be made towards improving perinatal care to prevent infections and birth asphyxia in this vulnerable population.

The coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus-2(sARs-CoV-2)has become a global pandemic[1-5].As of July 31,2020,there were 17,106,007 infected patients and 668,910 deaths worldwide[6,7].National protocols regarding COVID-19 issued by national-level agencies need to be revised in the field of pediatrics[8].From the perspective of hospital management,identitication and set-up of isolation wards and clinics,modification of workflow and processes,management of staff,allocation of medical resources,and formulation of clinical guidelines and protocols in infection prevention and control(IPC)were addressed as response measures to reduce the risk of transmission to healthcare workers and patients[9].In this paper we describe the strategies and practices implemented at the children's hospital of Fudan University in response to this public health *** strategy of response measures is shown in supplementary Fig.1.

Exploration of the financing and management model of a children＇s critical disease security system in China based on the implementation of shanghai children hospital Care Aid

This study is designed to serve as a reference for the establishment of health security systems for children＇s critical diseases. Through analysis of the operation of shanghai children hospital Care Aid （sCHCA）, this study explored the financing model and management of a children＇s critical disease healthcare system and analyzed the possibility of expanding this system to other areas. It is found that a premium as Iow as RMB 7 per capita per year under sCHCA can provide high-level security for children＇s critical diseases. With the good experience in shanghai and based on the current basic medical insurance system for urban residents and the new rural cooperative medical scheme （NRCMs）, it is necessary and feasible to build a health security system for children＇s critical diseases at the national level.

Background saline nasal irrigation is an effective therapy for relieving common cold *** study aimed to investigate and explore the efficacy of physiological seawater nasal irrigation(PsNI)on children with mild and asymptomatic infection with *** This randomized controlled trial was conducted in shanghai,China,and 403 children with mild and asymptomatic infection with Omicron were *** children were allocated into the PsNI group and the control *** primary outcome was the duration of viral shedding(DVs),and the secondary outcome was the change in clinical *** The median age of all participants was 5.59(6.26)years *** DVs was significantly shorter in the PsNI group[2.40(1.13)]than in the control group[3.09(2.14)](P=0.014).The multivariable Cox regression model also showed that patients in the PsNI group had an increased probability of shorter DVs compared with patients in the control group[hazard ratio(HR),1.27;95%confidence interval(CI),1.04–1.55;P=0.017].subgroup analysis suggested that the DVs of patients without full vaccination was significantly reduced in the PsNI *** proportions of runny nose and stuffy nose were apparently reduced in the first three days in the PsNI group or the control group,but there was no evidence showing that PsNI contributes to the benefit compared with the control *** PsNI can reduce the DVs of patients with mild and asymptomatic infection with sARs-CoV-2 Omicron BA.2 variant.

Association of copy number variation in X chromosome-linked PNPLA4 with heterotaxy and congenital heart disease

Background: Heterotaxy (HTX) is a thoracoabdominal organ anomaly syndrome and commonly accompanied by congenital heart disease (CHD). The aim of this study was to analyze rare copy number variations (CNVs) in a HTX/CHD cohort and to examine the potential mechanisms contributing to HTX/***: Chromosome microarray analysis was used to identify rare CNVs in a cohort of 120 unrelated HTX/CHD patients, and available samples from parents were used to confirm the inheritance pattern. Potential candidate genes in CNVs region were prioritized via the DECIPHER database, andPNPLA4 was identified as the leading candidate gene. To validate, we generatedPNPLA4-overexpressing human induced pluripotent stem cell lines as well aspnpla4-overexpressing zebrafish model, followed by a series of transcriptomic, biochemical and cellular ***: seventeen rare CNVs were identified in 15 of the 120 HTX/CHD patients (12.5%). Xp22.31 duplication was one of the inherited CNVs identified in this HTX/CHD cohort, andPNPLA4 in the Xp22.31 was a candidate gene associated with HTX/***4 is expressed in the lateral plate mesoderm, which is known to be critical for left/right embryonic patterning as well as cardiomyocyte differentiation, and in the neural crest cell lineage. Through a series ofin vivo andin vitro analyses at the molecular and cellular levels, we revealed that the biological function ofPNPLA4 is importantly involved in the primary cilia formation and function via its regulation of energy metabolism and mitochondria-mediated ATP ***: Our findings demonstrated a significant association between CNVs and HTX/CHD. Our data strongly suggested that an increased genetic dose ofPNPLA4 due to Xp22.31 duplication is a disease-causing risk factor for HTX/CHD.

Psychological Impact of the 2022 Round COVID-19 Pandemic on China's College students
