Background: Traumatic Brain Injury (TBI) is a major health problem worldwide. It is the main cause of trauma mortality. Acute subdural hematoma (ASDH) has a reputation for being the most serious of all posttraumatic head injuries. Its frequency is estimated to be 1% to 5% of all head injuries and 22% of severe head injuries. The aim of this study was to assess the epidemiological, clinical and prognosis aspects of ASDH in our department. Material and Methods: This prospective study, from January to December 2019, included 57 patients admitted to the Gabriel Touré Teaching Hospital for ASDH. Patients underwent detailed clinical and radiological evaluation and ASDH was diagnosed by non-contrast computed tomography (CT) scan. Functional outcome was evaluated 6 months after injury by Glasgow Outcome Scale. Results: During the study period, 662 patients were admitted for TBI including 57 (8.61%) cases of ASDH. The mean age was 34 years with extremes of 2 and 77 years. There was a male predominance with 52 (91.2%). The main cause of trauma was motor vehicle accidents in 38 (66.6%) patients. According to the Glasgow coma scale (GCS), 33 (57.9%) patients were classified mild (Table 2). The thickness of the hematoma was more than 1 cm in 13 (22.8%) patients and less than 1 cm in 44 (77%) patients. Surgical treatment was performed in 13 (22.8%) patients. The outcome was favorable in 30 (52.6%) patients and the mortality rate was 33.3%. Conclusion: ASDH remains a difficult challenge because of the high mortality. The mortality rate remains high particularly in patient with poor GCS score at admission.

Trend Analysis in Rainfall, Reference Evapotranspiration and Aridity Index in Southern Senegal: Adaptation to the Vulnerability of Rainfed Rice Cultivation to Climate Change

Rainfall and evapotranspiration are two vital elements for food production under rainfed agriculture. This study aims at investigating the combined changes in these variables in the form of aridly index in the southern Senegal. The temporal trends in annual and monthly (from May to October) aridity index, rainfall and evapotranspiration are examined and adaptation strategies to the vulnerability of rainfed rice cultivation to the changes are developed. The results show a significant decreasing trend in annual rainfall at all study locations for the period 1922-2015. When analyzing the trends in sub-periods, there are two clear patterns in the annual rainfall series: a decreasing trend for the period 1922-1979 and a reversal increasing trend for the period 1980-2015. An increasing trend is also observed in annual reference evapotranspiration. The results reveal that the region will be drier with a significant increase in aridity at the annual and most monthly series. Appropriate adaptation strategies should be implemented to diminish the adverse influence of the increasing aridity on rice productivity for a sustainable agriculture.

Study of the Spatio-Temporal Variability of Rainfall in the Northern Region of Senegal

Senegal is a country of the Sahel. In this region, most of the populations live from agro-pastoral activities. The northern zone of Senegal is strongly influenced by river cultures. And the dynamics of the Senegal River are dependent on rainfall. The rainfall in the area is very closely linked to the dynamics of the atmosphere. The study of the spatio-temporal variability of rainfall in the northern region of Senegal requires quality rainfall observation data. It includes the Ferlo and the Senegal River valley, in particular the regions of Louga (department of Linguère included), Saint-Louis (departments of Dagana and Podor included) and Matam. These stations have been defined since Le Borgne (1988). The difficulty of having quality rain observation data can be resolved by using more accessible and good quality satellite data. Using satellite data, namely MSWEP, CRU, TAMSAT, ARC and PERSIANN, we showed the return of precipitation that appeared in 2000 and the unimodal cycle of precipitation in our study area. These data were validated using the correlation coefficient, the bias, the RMSE and the Nash index with observation data from the Regional Study Center for the Improvement of Adaptation to Drought (CERASS). The CRU data is then retained. Thus, this study made it possible to show the zonal distribution of rainfall in the northern zone of Senegal.

A cerebrole is a patient with a neurological deficit of central origin;these patients very often present disorders of the ion concentration, hence the need for hospitalization in intensive care. The aim of our study was to study hydro-electrolyte disorders in brain-damaged patients, to describe the clinical aspects, to identify the contributing factors and to determine the prognosis of these patients in the intensive care unit of gabriel touré teaching hospital. Material and Methods: It was a prospective cross-sectional study, descriptive of a period of 11 months going from June 2015 to April 2016 in the intensive care unit of CHU Gabriel TOURE we included all the cerebral patients admitted in intensive care in which hydro-ionic disorders have detected on arrival or during hospitalization. Data were collected through transfer sheets, a survey sheet and medical records. Data entry and analysis were done respectively on SPSS software (version 19) and Microsoft Word Starter 2010 software. Results: During our study, out of 450 hospitalization patients, we identified 110 brain-damaged patients and 75 patients were retained in our study, i.e. a prevalence of 16.6%. Male was predominant with 72% with a sex ratio of 2.5 the age group 20 - 50 years was majority with 45.3%, the average age was 38.49 years. The majority of patients (58.7%) were admitted from the emergency department. The most common reason for admission was impairment of consciousness in 94.7% (Table 1) of cases and the Glasgow admission score was less than or equal to 8 in 58.7% (Table 1) of patients. Hydro-electrolyte disturbances were present in 93.3% of patients on admission. Hyponatremia was the most common disorder with 56% (42 cases). Followed by hyperkalaemia 10.7% (8 cases), hypernatremia 9.3% (7 cases), at hypocalcemia 9.3% (7 cases), associated disorders 8% (6 cases). Brain CT was performed in 52% of the patients. Transient Diabetes Induced by L-Asparaginase

Background: Although hyperglycaemia is one of the known side effects of L-asparaginase, its contribution to the development of diabetic ketoacidosis (DKA) is less well known in the literature. Asparaginase is an essential component of treatment protocols for acute lymphoblastic leukaemia (ALL) in combination with other chemotherapeutic drugs. On further evaluation, she was found to have high anion gap metabolic acidosis, hyperglycaemia and ketonuria. In recent decades, the use of these chemotherapeutic agents has led to a decrease in mortality and disease-free survival in ALL. L-asparaginase is one of the chemotherapy protocols used in the treatment of acute lymphoblastic leukaemia (ALL) and can induce hyperglycaemia which is aggravated by the concomitant use of corticosteroids. We report the observation of a 14-year-old girl treated with chemotherapy (GFA LAL protocol) who developed transient diabetes following the use of L-asparaginase. She was treated at the Donka paediatric haemato-oncology unit by a multidisciplinary team including a paediatric oncologist, a psychologist and a diabetologist. The aim of this study was to highlight blood glucose monitoring before and after the use of asparaginase in acute lymphoblastic leukaemia. Conclusion: We conclude that the occurrence of ketoacidosis following the use of asparaginase is a rare event. We recommend close monitoring of blood glucose levels for hyperglycaemia in patients with ALL receiving L-asparaginase.

Sensitivity Analysis of the Penman-Monteith Reference Evapotranspiration to Climatic Variables: Case of Burkina Faso

Sensitivity analysis is important in understanding the effect of each climatic input variable on the variation of the evapotranspiration which is a significant element for hydrological modelling, irrigation programs, and water resources management. This study investigates the responses of evapotranspiration to the variation of maximum and minimum temperature, solar radiation, wind speed and maximum and minimum relative humidity. Daily data from 1998 to 2012 of eight stations are used. For each climatic variable, a variation of 5% up to ±25% was performed in order to evaluate the sensitivity of the evapotranspiration to the input variables. Results show that evapotranspiration is more sensitive to the variation of solar radiation, maximal temperature and wind speed, respectively.

Evolution of People Living with HIV and AIDS in Medical Consultation at 17 ESOPE Sites in Mali

Despite the efforts made by the Malian government with technical and financial partners in the fight against HIV and AIDS, there are no data on the evolution of people with HIV in medical consultation in antiretroviral treatment sites. The objective of this work is to fill this gap by generating factual data describing the evolution of the number of people with HIV in medical consultation in Mali. To achieve this goal, we conducted a cross-sectional study using data from the Coordination Unit of the Sectoral Committee for AIDS Control of the Ministry of Health and Public Hygiene. Sampling was comprehensive including all people living with HIV. The data collected concerned those of the years 1999 to 2014 by the ESOPE software and analyzed with Stata version 12. A Total of 39,619 people were enrolled in the study. And 26,212 people living with HIV in the 17 ESOPE sites of Mali were female. Patients aged 25-34 and 35-44 years were strongly represented in consultation in the years 2007 and 2011, and those aged 15-24 were the least frequent. HIV1 was the most dominant with 97%. The majority of patients presented for initial consultation were clinical stages 2 and 3 in all years. Cases of lost sites seen in the more common in hospitals are 62.79% at the Hospital of Mopti and 57% at the university Hospital of Point G. The number of cases of people living with HIV in medical consultation has progressively augmented from 1999-2007. Then it dropped from 2008-2010. After this number has increased by 2011, it dropped again in 2012-2014. This study made it possible to note the evolution of the patients in medical consultation in ESOPE antiretroviral treatment sites according to the years and events (safe, subsidized by activities by the global fund). The activities of sensitization and communications, case research should be strengthened by involving the community.
