Serum ECM1 is a promising biomarker for staging and monitoring fibrosis in patients with chronic hepatitis B

It is critical to assess the extent and progression of liver fibrosis for patients to receive suitable treatments, but its diagnostic methods remain unmet. Extracellular matrix protein 1（ECM1） has previously been reported to be a key factor in the induction and progression of liver fibrosis. However, little is known about the use of ECM1 as a biomarker to evaluate fibrosis. In a CCl4-induced mouse model of liver fibrosis, the present study demonstrated that ECM1 decreased with gradually increasing fibrosis. Using biopsy as a reference, the serum ECM1 levels decreased with increasing fibrosis stage in 247 patients with liver fibrosis, but there were no significant changes between fibrosis stage 2 and stage 0–1. To improve the performance of ECM1, age, platelet count, and ECM1 concentration were combined to calculate an EPA（ECM1-platelet-age） score（ranging from 0 to 10）. The areas under the receiver operating char.cteristic curve of the EPA scores for the detection of F≥2, F≥3, and F4 were 0.6801, 0.7377, and 0.8083, respectively, which showed a comparable or significantly greater diagnostic performance for assessing fibrosis than that of the AST/ALT ratio, APRI score, or FIB-4 score. In HBV patients following antiviral treatment, the dynamics of the EPA score depended on the status of liver fibrosis development. The accuracy of the EPA score in predicting fibrosis regression and progression was 66.00% and 71.43%, respectively, while that of the LSM, another useful method for monitoring hepatic fibrosis changes during treatment, was only 52.00% and 7.14%, respectively. Compared with healthy controls, there were lower levels of serum ECM1 in HBV patients and individuals with HCV infection, MAFLD, ALD, PBC, and DILI. Minimalistic approach to left atrial appendage occlusion guided by cardiac computed tomography angiography

OBJECTIVE To assess the feasibility and safety of the minimalistic approach to left atrial appendage occlusion(LAAO) guided by cardiac computed tomography angiography(CCTA).METHODS Ninety consecutive patients who underwent LAAO, with or without CCTA-guided, were matched(1:2). Each step of the LAAO procedure in the computed tomography(CT) guidance group(CT group) was directed by preprocedural CT planning. In the control group, LAAO was performed.using the standard method. All patients were followed up for 12 months, and device surveillance was conducted using *** A total of 90 patients were included in the analysis, with 30 patients in the CT group and 60 matched patients in the control group. All patients were successfully implanted with Watchman devices. The mean ages for the CT group and the control group were 70.0 ± 9.4 years and 68.4 ± 11.9 years(P = 0.52), respectively. The procedure duration(45.6 ± 10.7 min vs. 58.8 ± 13.0 min,P hospital stay(7.5 ± 2.4 day vs. 9.6 ± 2.8 day, P = 0.001) in the CT group was significantly shorter compared to the control group. However, the total radiation dose was higher in the CT group compared to the control group(904.9 ± 348.0 m Gy vs.711.9 ± 211.2 m Gy, P = 0.002). There were no significant differences in periprocedural pericardial effusion(3.3% vs. 6.3%, P = 0.8) between the two groups. The rate of postprocedural adverse events(13.3% vs. 18.3%, P = 0.55) were comparable between both groups at 12 months *** CCTA is capable of detailed LAAO procedure planning. Minimalistic LAAO with preprocedural CCTA planning was feasible and safe, with shortened procedure time and acceptable increased radiation and contras consumption. For patients with contraindications to general anesthesia and/or transesophageal echocardiography, this promising method may be an alternative to conventional LAAO.

Bioinspired Tactile Sensation Based on Synergistic Microcrack-Bristle Structure Design toward High Mechanical Sensitivity and Direction-Resolving Capability

Natural tactile sensation is complex,which involves not only contact force intensity detection but also the perception of the force direction,the surface texture,and other mechanical ***,the vast majority of the developed tactile sensors can only detect the normal force,but usually cannot resolve shear force or even distinguish the directions of the ***,we present a new paradigm of bioinspired tactile sensors for resolving both the intensity and the directions of mechanical stimulations via synergistic microcrack-bristle structure design and cross-shaped configuration *** microcrack sensing structure gives high mechanical sensitivity to the tactile sensors,and the synergistic bristle structure further amplifies the sensitivity of the *** cross-shaped configuration engineering of the synergistic microcrack-bristle structure further endows the tactile sensors with good capability to detect and distinguish the directions of the applied mechanical *** as-fabricated tactile sensors exhibit a high sensitivity(25.76 N^(−1)),low detection limit(5.4 mN),desirable stability(over 2,500 cycles),and good capability to resolve both mechanical intensity and directional *** promising application scenarios,surface texture recognition and biomimetic path explorations are successfully demonstrated with these tactile *** newly proposed tactile sensation strategy and technology have great potential applications in ingenious tactile sensation and construction of various robotic and bionic prostheses with high operational dexterity.

Surgical techniques to prevent delayed gastric emptying after pancreaticoduodenectomy

Background: Delayed gastric emptying(DGE) is one of the most common complications after pancreaticoduodenectomy(PD). DGE represents impaired gastric motility without significant mechanical obstruction and is associated with an increased length of hospital stay, increased healthcare costs, and a high readmission rate. We reviewed published studies on various technical modifications to reduce the incidence of DGE. Data sources: Studies were identified by searching Pub med.for relevant articles published up to December 2022. The following search terms were used: “pancreaticoduodenectomy”, “pancreaticojejunostomy”, “pancreaticogastrostomy”, “gastric emptying”, “gastroparesis” and “postoperative complications”. The search was limited to English publications. Additional articles were identified by a manual search of references from key articles. Results: In recent years, various surgical procedures and techniques have been explored to reduce the incidence of DGE. Pyloric resection, Billroth II reconstruction, Braun's enteroenterostomy, and antecolic reconstruction may be associated with a decreased incidence of DGE, but more high-powered studies are needed in the future. Neither laparoscopic nor robotic surgery has demonstrated superiority in preventing DGE, and the use of staplers is controversial regarding whether they can reduce the incidence of DGE. Conclusions: Despite many innovations in surgical techniques, there is no surgical procedure that is superior to others to reduce DGE. Further larger prospective randomized studies are needed.

Background: Breakdown of tolerance against the commensal microflora is believ ed to be a major factor in the pathogenesis of inflammatory bowel disease (IBD). Dendritic cells (DC) have been implicated in this process in various animal mod els, but data on human DC in IBD are very limited. Aim: To char.cterise plasmacy toid DC (PDC) and myeloid DC (MDC) in patients with active versus inactive IBD a nd healthy controls. Patients and Methods: Peripheral blood was obtained from 10 6 patients (Crohn’ s disease (CD) n = 49, ulcerative colitis (UC) n = 57) and h ealthy controls (n = 19). Disease activity was scored using themodified Truelove Witts (MTWSI) for UC and the Harvey Bradshaw severity indices (HBSI) for CD. Fo ur colour flow cytometric analysis was used to identify, enumerate, and phenotyp e DC. DC from patients with acute flare ups and healthy controls were cultured a nd stimulated with CpG ODN 2006 or lipopolysacchar.de (LPS). Results: IBD patien ts in remission (PDC UC, 0.39% ; CD, 0.35% ; MDC- 1 UC, 0.23% ; CD, 0.22% of PBMC) have slightly lower numbers of circulating DC compared with healthy con trols (PDC 0.41% , MDC- 1 0.25% of PBMC). In acute flare ups IBD patients ex perience a significant drop of DC (PDC UC, 0.04% ; CD, 0.11% ; MDC- 1 UC, 0.1 1% ; CD, 0.14% of PBMC) that correlates with disease activity (correlation co efficients: PDC MTWSI, 0.93; HBSI, 0.79; MDC- 1 MTWSI, 0.75; HBSI, 0.81). Moreo ver, both express α l4β 7 integrin and display an immature phenotype. Freshly isolated PDC and MDC- 1 from untreated flaring IBD patients express higher base line levels of CD86 which increases further in culture and upon stimulation comp ared with healthy controls. Conclusion: IBD patients lack immature blood DC duri ng flare ups which possibly migrate to the gut. An aberrant response to microbia l surrogate stimuli suggests a disturbed interaction with commensals.

Velamentous Cord Insertion: Interest of Antenatal Diagnosis and Review of the Literature

Velamentous insertion of the umbilical cord corresponds to the insertion of the cord directly on amniotic membranes. It is a rare situation whose frequency varies from 0.5% to 1.69% of single pregnancies. It must be diagnosed during the morphological ultrasound of the 2nd trimester, actively looking for the association with a vasa previa, due to the risk of fetal haemorrhagic threat. We report an antenatal diagnosis of velamentous cord insertion and its management with literature review.
