Rehabilitative training effects of severe mixed-type cerebral palsy in children One case report

It has often been thought that severe spastic and athetoid cerebral palsy children respond poorly to rehabilitative training effects and have a poor prognosis after school age. The present study included a nine-year-old severe spastic and athetoid cerebral palsy boy, who received 2 years of physical therapy, supplemented with drugs and orthotic devices and who acquired good therapeutic effects. The beneficial rehabilitative effects were as follows： After discharge, primitive reflex disappeared, and both vertical reflex and parachute reflex were found; In the establishment of balance reflex, sitting position was rated as level III, and standing position was rated as level II. The boy could independently walk for 30-50 meters. In the assessment of muscle tone, the level was rated according to the Modified Ashworth Scale and was decreased from three, four to one, two, and the level rated according to the Clonus Scale decreased from V to II. Motor function was noticeably improved. The level was rated according to the Gross Motor Function Classification System and was decreased from V to II. The total score of 88 items of the Gross Motor Function Measure was increased from 6.1% to 72.2%. The Berg Balance Scale score increased from 0 to 42. The Holden Walking Functional Classification level increased from 0 to 3. The score of activities of daily living increased from 9.5 to 70. The symptoms of dysarthria were alleviated from severe to moderate, and those of salivation were alleviated from level V to level I. These results demonstrated that after systemic and scientific treatment and training, the spastic and athetoid cerebral palsy boy had the ability to walk independently and to care for himself.

Clinical Observation on Treatment of Clearing the Governor Vessel and Refreshing the Mind Needling in Neural Development and Remediation of children with cerebral palsy

To investigate the effects of clearing the Governor Vessel and refreshing the mind needling in neural development and remediation of children with cerebral palsy. Methods： A total of 200 cases of children with cerebral palsy were randomly assigned to the treatment group （100 patients） and the control group （100 patients）. The treatment group was given the combined therapy of acupuncture and rehabilitation training, and the chosen acupoints were 13 points of the Governor Vessel, Shenshu （BL 23）, Taixi （KI 3）, Yanglingquan （GB 34）, Zusanli （ST 36） and Sanyinjiao （SP 6）, and points of refreshing the mind were also selected, which included puncturing Shenting （GV 24） toward Qianding （GV 21）, puncturing Qianding （GV 21） toward Baihui （GV 20）, puncturing Baihui （GV 20） toward Naohu （GV 17） and Sishencong （Ex-HN 1）. The control group was only treated with rehabilitation training. A contrastive analysis of the therapeutic effect between acupuncture combined with rehabilitation training and rehabilitation training alone was made after a treatment course of 3 months. The Gross Motor Function Measure （GMFM） and Beijing Gesell Developmental Scale were adopted to assess the neural development and rehabilitation outcomes of the two groups. In addition, skull CT/MRI was adopted to evaluate the plerosis of injured cerebral nerve after treatment. Results： The total effective rate in treatment group was 87% （87/100）, significantly higher than the 55% （55/100） in the control group. The children＇s development quotient （DQ） tested by Gesell Developmental Scale and scores tested by GMFM in the treatment group were obviously higher than those in the control group （P〈0.01）. The improving and curing rates presented by skull CT/MRI in the treatment group were higher than those in the control group （P〈0.01）. cerebral palsy is a multiple disability manifested by motor deficits and impairments of cognition, language, and body perception. To assess if body schema and body image distortion in children and youth with cerebral palsy can be represented through draw-a-person-test, outcome measure of conscious and subconscious body awareness, visual perception and cognition in two age-matched groups, one with cerebral palsy and other with developmental disability, were collected. The outcome was compared within the two groups and with reference data (healthy population) and correlated with the outcome of draw-a-person test. Decreased scores in the draw-a-person test in subjects with cerebral palsy compared both to subjects with developmental disorders and to healthy population had strongly correlated with decreased scores in visual perception. This suggests that draw-a-person test may provide a preliminary guidance for the assessment of visual perception. No statistically significant effect of cognitive abilities on performance in the draw-a-person test was found. The influence of both conscious and subconscious body awareness on the scores in the draw-a-person test was not significant, which confirms that this test alone is not fully reliable to detect the body schema and body image disorder in cerebral palsy.

New Ideas of Treatment for cerebral palsy I Capital Flexion of the Neck： The Key Link in Prematurity Treatment

In a human, the head and spine work together in any kind of posture and movement. Any movement starts from the head through neck flexion and specifically capital flexion. Capital flexion initiates the straightening of the cervical spine which causes the connection of the head on the C1-C2 suboccipital part to the thoracic and lumbar parts. With this, the spine starts to move and postural tone increases. Without construction of the neck or alteration of the axis, as seen with cases of prematurity, postural tone becomes low. Typical features of children with prematurity include low postural tone, altered axis of the head and neck which generates incorrect or ineffective vestibular information and poor cortical movement caused by poor development of capital flexion. Therefore, the most important aspect to consider is the lack of capital flexion causing the absence of some initiation of movements of the spine which leads to further weakness of the neck and trunk.

Serum level changes of insulin-like growth factor-1 and amino acids in children with cerebral palsy following functional exercise plus head acupuncture therapy

BACKGROUND： In the past few years, there were many studies about the pathophysiology of insulin-like growth factor-1 （IGF-1）, as well as glutamic acid （GLU） and γ-aminobutyric acid （GABA） in hypoxic-ischemic brain damage （HIBD）. IGF-1 plays a protective role in brain damage. The over release of excitatory amino acids （EAA） plays an important role in acute neuronal death, which delays neuronal death. The disproportion of increasing of excitatory and inhibitory amino acids can cause different extent HIBD. However, there is rare report about the change of IGF-1 and neurotransmitter level in serum of cerebral palsy （CP） children. OBJECTIVE ： To observe the levels of serum IGF-1, GABA and GLU before and after functional exercise plus head acupuncture therapy and single functional exercise, then study the effective mechanism of IGF-1, GABA and GLU in the occurrence and development of CP, and compare those with normal control group. DESIGN： Case-control study SETTING： College of Rehabilitation Medicine, Jiamusi University, Prevention and Treatment Center of Child cerebral palsy in Heilongjiang Province. PARTICIPANTS： CP group： Sixty CP children came from Prevention and Treatment Center of Child Cerebra palsy in Heilongjiang Province between April 2005 and March 2006 were selected in this study. All the cases were consistent with the diagnostic criteria and the type of cerebral palsy and finally diagnosed with CT and MRI examinations. There were 35 males and 25 females aged from 8 months to 4 years with the mean age o （2.0±0.5） years. And then, they were randomly divided into two groups. Twenty-six cases were received functional exercise, and the other 34 cases were treated with functional exercise and head acupuncture. Objective： To investigate the brain functional compensation of acupuncture in cerebral palsy rehabilitation, Method： All 100 cerebral palsy kids were randomized into a treatment group （50 cases） and a control group （50 cases）. For cases in the treatment group, points of connecting the Governor Vessels and tonifying the kidney were selected： 13 points of the Governor Vessel, Yongquan （Kid 1）, Yanglingquan （GB 34）, Zusanli （ST 36） and Sanyinjiao （SP 6）, and points of refreshing the mind were also selected： puncturing Shenting （GV 24） toward Baihui （GV 20） and puncturing Baihui （GV 20） toward Naohu （GV 17） and Sishencong （Ex-HN 1）. In addition, rehabilitation training was also combined. For cases in the control group, rehabilitation training alone was adopted. Then the clinical effect and improvement through head CT scanning and SPECT were observed. Result： The total effective rate of the treatment group was 84%, significantly higher than 52% of the control group. After treatment, the DQ in the treatment group was significantly higher than the control group （P〈0.01）. Also, the improvement rate of brain maldevelopment and brain atrophy showed by CT scan and restoration rate of brain neuron metabolic function in the treatment group were higher than the control group （P〈0.01）. Conclusion： Acupuncture can promote the compensation of brain function.
